Neutering isn’t universally good or bad for dogs. It eliminates certain health risks while increasing others, and the tradeoffs depend heavily on your dog’s breed, size, sex, and the age at which the procedure is done. The old advice to neuter every dog at six months is outdated. A 10-year study from UC Davis covering 35 breeds found that some dogs face significantly higher rates of joint disease and cancer when neutered too early, while smaller breeds generally tolerate early neutering with no measurable health consequences.
Joint Problems and Breed Size
The clearest risk from early neutering is joint disease, particularly in medium and large breeds. Sex hormones play a direct role in how growth plates close, and removing those hormones before a dog finishes growing can alter bone development. In Golden Retrievers, males neutered before six months had a 25% rate of joint disorders compared to intact males. German Shepherds neutered before six months showed joint disorder rates of 19% to 20% in both sexes. Female Rottweilers spayed before six months had a striking 43% rate of joint problems, mostly involving the cranial cruciate ligament (the canine equivalent of an ACL tear). In one small sample of Saint Bernards, every female spayed before six months developed a joint disorder.
For smaller breeds, the picture is different. Chihuahuas, Beagles, Pugs, Dachshunds, and most toy breeds showed no increased joint risk from neutering at six months. This makes sense biologically: smaller dogs reach skeletal maturity faster, so the window where missing hormones could affect bone growth is shorter.
Cancer: A Mixed Picture
Neutering eliminates the risk of testicular cancer entirely, and spaying before the first or second heat cycle dramatically reduces mammary cancer risk. These are real, meaningful benefits. But neutering also raises the risk of certain other cancers.
A large survey published in the Journal of the American Veterinary Medical Association found that intact males had a 9% cancer rate, while neutered males had a 28% rate. Intact females had similar numbers compared to spayed females at 26%. Some of this difference reflects the fact that neutered dogs live longer on average (more time to develop cancer), but the pattern holds even when researchers account for age. Prostate cancer, while rare in dogs overall, is roughly 2.8 times more common in neutered males than intact males. This is counterintuitive, since in humans, testosterone drives prostate cancer. In dogs, the relationship works differently.
For mixed-breed dogs, a separate study found no significant increase in cancer rates with neutering across any weight category. The cancer risks appear to be more concentrated in specific purebred lines already predisposed to certain tumor types.
Lifespan and Overall Health
Data from the Dog Aging Project, one of the largest studies of companion dog health, found that sterilization increased life expectancy by about 13.8% in males and 26.3% in females. Neutered dogs were less likely to die from infections and trauma. The lifespan advantage likely comes partly from reduced roaming behavior (fewer car strikes, fewer fights) and partly from eliminating reproductive organ diseases.
That said, a longer life isn’t automatically a healthier one. A survey comparing over 6,000 dogs found that neutered males had orthopedic problems at three times the rate of intact males (21% vs. 7%), and spayed females had similar elevations (19% vs. 3%). Neutered dogs of both sexes were also more likely to be reported as obese: 4% to 5% compared to less than 1% of intact dogs.
Weight Gain After Neutering
Weight gain after neutering is not just an owner management issue. It has a biological basis. Neutering increases food intake and, in females, significantly drops the resting metabolic rate. In one controlled study, neutered animals gained roughly twice as much weight as intact animals over the same period, and a much larger proportion of that weight was fat rather than lean mass. Castrated males gained 22% of their weight as fat compared to about 9% in intact males.
This doesn’t mean neutered dogs are destined to be overweight, but it does mean you’ll likely need to cut portions by 20% to 30% after the procedure and monitor body condition more carefully going forward.
Behavior Changes
Neutering is often recommended for behavioral reasons, but the evidence is more nuanced than most people expect. A large study examining aggression found no association between neutering and aggression toward familiar people or other dogs, regardless of when the procedure was done. Dogs neutered between 7 and 12 months were 26% more likely to show aggression toward strangers, but dogs neutered at other ages showed no such increase.
Neutered males were more likely to have “problematic behaviors” overall: 53% compared to 35% of intact males. Intact males, however, were more likely to exhibit nuisance behaviors like marking and mounting. The behavioral tradeoffs aren’t straightforward, and neutering is not a reliable fix for most behavioral problems. Fear-based aggression and territorial behavior, in particular, tend to persist unchanged after the procedure.
The one consistent behavioral benefit is reduced roaming in male dogs, which lowers the risk of traumatic injuries.
Urinary Incontinence in Females
About 20% of spayed females develop urinary incontinence at some point, compared to less than 1% of intact females. This can happen as early as one week after surgery, though it more commonly appears months or years later. The incontinence results from hormonal changes that weaken the muscles controlling the bladder, not from surgical damage. It’s manageable with medication in most cases, but it doesn’t resolve on its own and requires lifelong treatment.
Male dogs rarely develop incontinence regardless of whether they’re neutered.
Breed-Specific Timing Recommendations
The UC Davis research produced specific guidelines for when to neuter based on breed. The recommendations reflect the point at which the health risks of early neutering become negligible for that breed:
- 6 months (most small and medium breeds): Beagle, Chihuahua, Cocker Spaniel, Corgi, Dachshund, Bulldog, Pug, Shih Tzu, Yorkshire Terrier, Labrador Retriever, Border Collie, Siberian Husky, and others
- 12 months: Australian Shepherd, Boxer, Newfoundland, Rhodesian Ridgeback
- 24 months (large and giant breeds): German Shepherd, Great Dane, Bernese Mountain Dog, Rottweiler, Saint Bernard, Mastiff, Standard Poodle, Irish Wolfhound
- Consider leaving intact: Male Doberman Pinschers and female Golden Retrievers showed enough elevated risk that the researchers suggested not neutering at all
For mixed-breed dogs, body size is a reasonable proxy. Dogs under 20 pounds can generally be neutered at six months. Dogs over 50 pounds benefit from waiting until 12 to 24 months, depending on their expected adult size.
Hormone-Sparing Alternatives
Vasectomy for males and ovary-sparing spay for females are procedures that sterilize dogs while leaving their hormone-producing organs intact. These are not yet widely available, but early data is promising. Dogs that retained their hormones through these procedures had orthopedic and cancer rates similar to intact dogs (about 8% orthopedic issues and 11% cancer), while gonadectomized dogs had rates of 19% to 21% for orthopedic problems and 26% to 28% for cancer.
Complication rates were low: 12% of vasectomized dogs had minor surgical complications like swelling at the incision site, and 7% of ovary-sparing spay patients had similar issues. These procedures do not protect against hormone-driven conditions like mammary tumors or pyometra (uterine infection), so they involve a different set of tradeoffs. Not every veterinarian performs them, and they require a detailed conversation about which risks matter most for your specific dog.
