Neither oat milk nor cow’s milk is universally “better.” Each one wins in different categories, and the right choice depends on what matters most to you: protein, heart health, blood sugar, environmental impact, or dietary restrictions. Here’s how they actually compare, nutrient by nutrient.
Calories, Protein, Fat, and Sugar
Per one-cup (240 ml) serving, the basic numbers look quite different:
- Protein: Cow’s milk (1%) delivers 8 grams; oat milk provides only 3 grams.
- Total fat: Oat milk has 4 grams; low-fat cow’s milk has 2 grams.
- Sugar: Cow’s milk contains 12 grams of naturally occurring lactose; unsweetened oat milk has about 3 grams.
That protein gap is significant. If you swap your daily glass of cow’s milk for oat milk, you lose 5 grams of protein per serving. Over a day with multiple servings (in cereal, coffee, cooking), that deficit adds up fast. The quality of the protein differs too. Cow’s milk protein, a mix of casein and whey, is among the most digestible and complete proteins available. Oat protein scores between 43 and 69 on the PDCAAS scale (a measure of protein quality), largely because it’s low in the essential amino acid lysine. Cow’s milk protein scores near the top of that same scale.
If you’re relying on milk as a protein source, especially for growing children or older adults trying to maintain muscle, cow’s milk has a clear advantage.
Blood Sugar Response
This is one area where cow’s milk pulls ahead by a wide margin, and most people don’t realize it. Cow’s milk has a glycemic index between 20 and 34, depending on fat content. Oat milk’s glycemic index sits around 69, which is in the medium-to-high range. That means oat milk raises blood sugar significantly faster than cow’s milk does.
The reason is straightforward: when oats are processed into milk, their starches break down into simpler sugars that your body absorbs quickly. Even though the sugar content on the label looks low, the type of carbohydrate matters. If you have prediabetes, type 2 diabetes, or you’re watching your blood sugar for any reason, this is worth knowing. Drinking oat milk in your morning coffee alongside a carb-heavy breakfast could push your glucose higher than you’d expect from the nutrition label alone.
Heart Health and Cholesterol
Oat milk’s standout ingredient is beta-glucan, a soluble fiber found naturally in oats. Beta-glucan binds to cholesterol in the gut and helps carry it out of the body. Commercial oat milk contains roughly 0.5 grams of beta-glucan per 100 ml, so a standard cup gives you about 1.2 grams. A study from Lund University found that men with moderately high cholesterol who drank 750 ml of oat milk daily for five weeks saw reductions in both total cholesterol and LDL (“bad”) cholesterol compared to a rice milk control group.
You’d need to drink a fair amount of oat milk to hit the 3 grams of beta-glucan per day that most health guidelines associate with meaningful cholesterol reduction. But even smaller amounts contribute, especially as part of a diet already rich in soluble fiber from other sources like beans, barley, and whole oats.
Cow’s milk, particularly whole milk, contains saturated fat, which can raise LDL cholesterol when consumed in large amounts. Switching to oat milk is one practical way to cut saturated fat intake while adding a cholesterol-lowering fiber you won’t find in dairy.
Calcium and Bone Health
Most commercial oat milks are fortified with calcium to match the amount in cow’s milk, so the numbers on the label often look similar. But the calcium in cow’s milk is naturally occurring and has a well-documented absorption rate of about 32%. The calcium added to plant milks may not be absorbed as efficiently.
There’s also a practical problem: fortified calcium tends to settle to the bottom of the carton. Even with vigorous shaking, you may not get the amount listed on the label in every pour. If you rely on oat milk as your primary calcium source, shaking the carton thoroughly every time matters more than you’d think. Some brands use calcium carbonate, others use tricalcium phosphate, and their absorption rates vary.
For people who drink oat milk exclusively, getting calcium from other foods like leafy greens, tofu, canned sardines, or fortified orange juice helps close any gap.
What About the Additives?
Scan the ingredient list on most oat milks and you’ll find oils (sunflower or canola/rapeseed), dipotassium phosphate, and sometimes gums or emulsifiers. These have generated concern on social media, but the alarm is largely overblown.
The canola or rapeseed oil added for creaminess is low in saturated fat, high in unsaturated fat, and relatively rich in omega-3 fatty acids. It actually has cardiometabolic benefits, helping reduce LDL and total cholesterol. The small amount in a serving of oat milk isn’t a health concern.
Dipotassium phosphate, used to keep oat milk from curdling in hot coffee, is safe for the general population and provides about 20% of your daily phosphorus needs. The one exception: people with kidney disease need to monitor phosphorus intake carefully, since impaired kidneys can’t clear excess phosphorus efficiently.
A more legitimate concern is glyphosate, the herbicide commonly sprayed on oat crops before harvest. The Environmental Working Group has reported detectable glyphosate residue in many oat-based products. Choosing organic oat milk reduces this exposure, though the health significance of trace glyphosate in food remains debated.
Gluten and Allergy Considerations
Oat milk is naturally free of lactose and dairy proteins, making it a solid choice for people with lactose intolerance or a cow’s milk allergy. It’s also free of soy and nuts, which gives it an edge over many other plant milks for people managing multiple allergies.
However, oats carry a high risk of cross-contamination with wheat, barley, and rye during growing and processing. If you have celiac disease, standard oat milk is not safe. The National Celiac Association recommends choosing only oat milk labeled gluten-free, ideally certified by a third party, which means it’s been tested to contain fewer than 20 parts per million of gluten. A regular gluten-free label on the package meets the FDA minimum, but third-party certification involves more rigorous testing.
Environmental Footprint
If environmental impact drives your choice, oat milk wins decisively. Dairy milk produces an average of about 1.29 kg of CO2 per kilogram of milk. Plant-based milks as a category produce far less, with most estimates placing oat milk’s carbon footprint at roughly one-third of dairy’s. Oat milk also requires significantly less water and land than dairy production.
Among plant milks, oat milk isn’t the absolute lowest-impact option (coconut milk edges it out on carbon), but it’s widely available, grows well in temperate climates without irrigation, and avoids some of the water-intensity problems associated with almond milk.
Which One Should You Choose?
Choose cow’s milk if your priority is protein, stable blood sugar, or reliable calcium absorption. It’s a nutritionally dense, minimally processed food that delivers complete protein and naturally occurring vitamins and minerals in every glass.
Choose oat milk if you’re avoiding dairy for digestive, ethical, or environmental reasons, or if you’re specifically trying to lower cholesterol. Just be aware that you’ll need to get your protein elsewhere, and unsweetened versions are worth seeking out to keep added sugars low.
For many people, the best approach is using both. Cow’s milk where protein and nutrition density matter (smoothies, post-workout, kids’ meals) and oat milk where taste and dietary preference take priority (coffee, cereal, baking). The “better” milk is the one that fits the rest of what you eat.
