Red light therapy is not harmful for most people when used at standard power levels and session lengths. The wavelengths used in red light devices (typically 600 to 700 nm for visible red, and 810 to 1060 nm for near-infrared) are nonionizing, meaning they don’t carry the DNA-damaging energy that ultraviolet light does. That said, real risks exist at high intensities, for certain skin tones, and when combined with specific medications.
How Red Light Differs From Harmful Light
Red and near-infrared light sit at the opposite end of the spectrum from ultraviolet radiation, the type linked to sunburn, premature aging, and skin cancer. Red light devices deliver very low levels of heat and don’t produce UV. This is one reason red light therapy has gained popularity for skin rejuvenation, wound healing, and muscle recovery: it can deliver energy to tissue without the risks associated with sun exposure or tanning beds.
Visible red light (around 660 nm) primarily affects the skin’s surface layers. Near-infrared light (around 850 nm) penetrates deeper into muscle and joint tissue, which is why it’s often chosen for pain relief or recovery. Neither wavelength is ionizing, so they don’t damage DNA the way X-rays or UV radiation can.
When Red Light Can Cause Skin Problems
At typical consumer-device power levels, red light doesn’t burn or injure the skin. Devices designed for home use generally operate between 30 and 100 milliwatts per square centimeter, a range considered effective without overheating tissue. Sessions usually last 10 to 20 minutes per treatment area.
Push the intensity much higher, though, and problems start. One early-stage clinical trial found that high-intensity red LEDs can cause blistering and redness. This is mostly a concern with professional-grade equipment used incorrectly or with cheap, poorly calibrated devices that don’t match their advertised specs.
People with darker skin tones face an additional consideration. The American Academy of Dermatology notes that darker skin is more sensitive to visible light, including red light, and this increased sensitivity can trigger hyperpigmentation. The resulting dark spots can be more intense and longer lasting than those caused by invisible wavelengths like infrared. If you have a medium to dark complexion, starting with shorter sessions and lower power settings is a practical way to gauge your skin’s response before committing to a full routine.
Eye Safety Matters More Than Most People Realize
Your eyes are the part of your body most vulnerable to red and near-infrared light. Research published in Translational Vision Science & Technology tested different light intensities on pigmented retinas and identified 500 mW/cm² as a threshold where damage begins. At that level, roughly one-third of test subjects showed localized photoreceptor damage near the optic nerve, along with pigment changes and sometimes vascular leakage in the retina. Lower intensities (25 and 100 mW/cm²) caused no detectable harm.
Most home devices don’t reach 500 mW/cm² at treatment distance, but staring directly into any bright LED panel is still a bad idea. Opaque, properly fitted goggles are worth wearing during every session. Many red light panels ship with a pair included. If yours didn’t, basic laser safety glasses rated for the correct wavelength range are inexpensive and widely available.
Medications That Increase Your Risk
Certain drugs make your skin more reactive to light, a property called photosensitivity. If you’re taking any of the following, red light therapy could provoke unexpected redness, irritation, or pigment changes:
- Acne medications like isotretinoin
- Antibiotics including tetracyclines and sulfonamides
- Anti-inflammatory drugs such as ibuprofen and naproxen
- Certain antidepressants, particularly tricyclics
- Blood pressure medications and diuretics, especially thiazides
- Oral contraceptives
- Some diabetes medications in the sulfonylurea class
This doesn’t automatically mean you can’t use red light, but it does mean your tolerance threshold is lower. Shorter sessions, greater distance from the panel, and careful attention to how your skin responds afterward are all reasonable precautions.
Pregnancy and Red Light
No published evidence shows red light therapy harms a developing pregnancy, but no large-scale pregnancy-specific studies exist either. The general precautionary guideline is to avoid directing the light at your abdomen, pelvis, or lower back. Treating your face or extremities is considered lower risk. The caution comes from insufficient data, not from documented harm.
What the FDA Actually Regulates
The FDA classifies photobiomodulation devices intended for medical use as Class II medical devices, meaning they require a premarket review before they can be legally sold for therapeutic claims. However, many red light panels on the market are sold as “general wellness” products rather than medical devices. These wellness-category products face far less regulatory scrutiny, so quality and accuracy of power output can vary widely between brands.
If you’re shopping for a device, look for one that has received FDA clearance (not just “FDA registered,” which is a much lower bar). Clearance means the device has been reviewed for safety and performs as advertised. A cleared device paired with the manufacturer’s recommended treatment distance and session time keeps you well within established safety margins.
The Bottom Line on Risk
For the average person using a consumer-grade device at recommended settings, red light therapy carries very little risk. The real dangers come from extremes: too much power, too close to the eyes, or skin that’s been sensitized by medication. Keeping sessions in the 10 to 20 minute range, wearing eye protection, and choosing a device with a power density between 30 and 100 mW/cm² at treatment distance puts you in what researchers consider the effective-but-safe window. People with darker skin should monitor for any new dark spots, and anyone on photosensitizing medication should ease in slowly.
