Is Smoking Weed Better Than Vaping Nicotine?

Neither smoking weed nor vaping nicotine is safe, but they damage your body in different ways. Smoking marijuana exposes your lungs to combustion byproducts, tar, and fine particulate matter at levels that exceed even tobacco cigarettes. Vaping nicotine avoids combustion but still delivers toxic metals, inflammatory chemicals, and one of the most addictive substances known. The honest answer is that you’re choosing between two distinct sets of risks, not picking a “better” option.

Lung Damage: Smoke vs. Aerosol

Burning marijuana produces more fine particulate matter (PM2.5) per minute than a tobacco cigarette. Research from UC San Francisco measured PM2.5 emissions across different methods and found that joints released 7.8 milligrams per minute, bongs released 5.2 mg/min, and hand pipes released 4.2 mg/min. A tobacco cigarette, by comparison, released 2.2 mg/min. Cannabis vape pens fell in between at 3.4 mg/min. When people smoked a bong in a living room, the average PM2.5 concentration hit 1,300 micrograms per cubic meter, which is 37 times higher than the EPA’s 24-hour safety standard.

Nicotine vaping avoids combustion entirely, which means no tar and significantly less particulate matter. But “less” doesn’t mean “none.” A study published in the journal Thorax compared the lung effects of vaping cannabinoid (CBD) aerosol versus nicotine aerosol in mice and found that both caused inflammatory infiltration in lung tissue, with immune cells clustering around airways and blood vessels. The CBD aerosol caused roughly twice the oxidative stress and significantly more damage to the lung barrier than nicotine aerosol. Proteins leaked from the lungs into surrounding fluid at nearly double the rate with CBD vaping compared to nicotine vaping.

This matters because many people assume “vaping weed” is the clean alternative to smoking it. The delivery method helps, but the substance you’re inhaling still drives a large portion of the harm.

The EVALI Outbreak and Contamination Risks

In 2019 and 2020, a wave of severe lung injuries tied to vaping hospitalized 2,807 people across all 50 states. The CDC identified vitamin E acetate, an oily additive used to dilute THC vape cartridges, as the primary culprit. It was found in lung fluid samples from 48 out of 51 patients tested but in none of the healthy comparison group. Nearly all cases involved THC-containing products, particularly those purchased from unregulated sources.

Nicotine vapes weren’t immune to contamination concerns either. Research has shown that the metal heating coils in vape devices can release nickel, chromium, and lead into the aerosol you inhale. For cannabis vapes specifically, the American Chemical Society found that some unregulated THC cartridges contained 100 times more lead than regulated products, far exceeding accepted safety limits. If you’re vaping anything from an unregulated source, the contamination risk climbs sharply.

Heart and Blood Vessel Effects

Marijuana smoke contains many of the same chemical toxins found in tobacco smoke, and smoking any substance promotes inflammation in the arteries. A study tracked cannabis users over three years and found they were six times more likely to have a heart attack compared to non-users. In a separate four-year study, daily cannabis users faced 25% greater odds of a heart attack and were 42% more likely to have a stroke compared to people who never used cannabis. Even weekly use carried a small but measurable increase: 3% higher heart attack risk and 5% higher stroke risk.

Nicotine raises heart rate and blood pressure acutely every time you use it, which stresses your cardiovascular system over time. The long-term cardiovascular data on nicotine vaping specifically is still limited because the products haven’t been widely used for as long. But nicotine itself is a known vasoconstrictor that narrows blood vessels, and chronic use keeps your cardiovascular system under persistent low-grade stress.

Brain Effects in Young Users

Both substances thin the brain’s gray matter in adolescents and young adults, but they target somewhat different regions. Research on people aged roughly 16 to 22 found that nicotine users had thinner cortex in areas tied to motor control and executive function, including parts of the frontal lobe responsible for planning, decision-making, and impulse control. Cannabis users showed thinning in an overlapping but distinct region involved in language processing and complex reasoning.

When young people used both substances together, the interaction was unexpected. In some brain regions, cannabis users who also used nicotine actually had thicker cortex than cannabis-only users, suggesting the two substances may interact in ways that aren’t simply additive. Researchers couldn’t determine whether this interaction translated to better or worse cognitive performance because the study didn’t include cognitive testing. What’s clear is that both substances alter brain structure during a period when the brain is still developing, and the effects aren’t identical.

Addiction: A Major Difference

This is where the two substances diverge most sharply. Nicotine hooks roughly 30% of people who use it regularly. Cannabis hooks about 9%. That three-to-one gap makes nicotine one of the most addictive commonly used substances, on par with or exceeding many harder drugs in terms of how reliably it creates dependence.

Vaping nicotine can make this worse, not better. Modern vape devices deliver nicotine efficiently and allow near-constant use throughout the day, which keeps blood nicotine levels consistently high and deepens physical dependence. Cannabis dependence is real and can involve irritability, sleep disruption, and cravings when you stop, but the withdrawal is generally less intense and shorter-lived than nicotine withdrawal.

Effects on Your Mouth

Smoking marijuana dries out your mouth by reducing saliva production. Saliva is your mouth’s primary defense against bacteria, so chronic dry mouth creates conditions for cavities, gum disease, and bad breath to flourish. The tar and combustion chemicals in marijuana smoke also directly irritate gum tissue, and long-term use has been linked to chronic gum inflammation and tooth loss.

Nicotine vaping also reduces blood flow to the gums and can mask early signs of gum disease by suppressing the bleeding that would normally alert you to a problem. The propylene glycol in vape liquid draws moisture from oral tissues, contributing to its own version of dry mouth. Both habits are bad for your teeth, though through somewhat different mechanisms.

Secondhand Exposure

If you’re around other people, the difference is substantial. Smoking a joint in a room sends PM2.5 concentrations to levels the EPA classifies as hazardous. Even in a cannabis store that only allowed vaporizing and dabbing (no combustion), average PM2.5 levels measured 84 micrograms per cubic meter, still above the EPA safety threshold. Nicotine vaping produces visible aerosol that dissipates quickly and contains far fewer particulate matter emissions than any form of smoking, though it’s not zero exposure for bystanders.

What You’re Really Choosing Between

Smoking weed delivers more particulate matter to your lungs, more cardiovascular stress, and more secondhand harm to people around you. Vaping nicotine is less acutely damaging to lung tissue but carries a much higher addiction risk, exposes you to toxic metals from heating elements, and keeps your cardiovascular system under constant low-level strain. If you’re young, both substances alter your brain structure in ways that may affect cognition long-term.

The framing of “better” implies one is acceptable. In practice, the risks are just distributed differently. Smoking weed hits harder on the lungs and heart in the short term. Vaping nicotine locks you into a daily habit that’s notoriously difficult to quit, with cardiovascular and respiratory consequences that accumulate over years of use. Choosing between them based on which is “safer” misses the more useful question, which is what specific risks you’re willing to accept and which ones you aren’t.
