Strep throat is caused by bacteria, not a virus. Specifically, it’s caused by a type of bacterium called group A Streptococcus (also known as Streptococcus pyogenes). This distinction matters because bacterial infections like strep respond to antibiotics, while viral sore throats do not. Most sore throats are actually viral, which is why telling the two apart is so important.

Why the Confusion With Viruses

The reason so many people search this question is that strep throat and a viral sore throat can feel similar at first. Both cause throat pain and can come with a fever. But the underlying causes are completely different, and so is the treatment. Viruses like adenovirus, rhinovirus, and the flu are responsible for the majority of sore throats. These infections run their course on their own and don’t need antibiotics. Strep throat, on the other hand, is a bacterial infection that requires antibiotic treatment to clear up safely.

How to Tell Strep Apart From a Viral Sore Throat

The symptom patterns for bacterial and viral sore throats are surprisingly distinct once you know what to look for. Strep throat tends to come on suddenly and often includes fever, swollen lymph nodes in the neck, headache, loss of appetite, and belly pain (especially in children). If you look at the back of your throat, you may see bright red tonsils with white patches.

The key giveaway is what’s missing. Strep throat typically does not cause coughing, sneezing, or a runny nose. If you have a sore throat along with those cold-like symptoms, plus red or watery eyes and mild body aches, a virus is the far more likely cause.

Because strep throat is bacterial, antibiotics are the standard treatment. The first choice is typically penicillin or amoxicillin, taken for 10 days. If you have a penicillin allergy, several alternatives are available. Finishing the full course matters even if you feel better within a few days, because stopping early can leave bacteria behind and increase the risk of complications.

One practical benefit of starting antibiotics: you become much less contagious quickly. Most people are no longer contagious within 12 hours of their first dose. Schools and child care centers typically allow kids to return after that 12-hour window, as long as symptoms are improving.

What Happens if Strep Goes Untreated

This is the main reason the bacteria-versus-virus question matters so much. A viral sore throat is unpleasant but generally harmless. Untreated strep throat, however, can lead to serious inflammatory complications. These include:

  • Rheumatic fever: a condition that can damage the heart, joints, nervous system, and skin
  • Scarlet fever: a strep-related illness that causes a distinctive red rash
  • Kidney inflammation: a condition called poststreptococcal glomerulonephritis that affects how the kidneys filter blood
  • Reactive arthritis: painful joint inflammation triggered by the infection

These complications are uncommon when strep is treated with antibiotics, but they’re a real risk when the infection is ignored or mistaken for a simple cold. Rheumatic fever in particular can cause lasting heart damage, which is why strep throat in children is taken seriously even when symptoms seem mild.

Who Gets Strep Throat Most Often

Strep throat is most common in children between ages 5 and 15, though adults can get it too. It spreads through respiratory droplets, so close contact in schools, daycare, and households drives most transmission. Adults who live or work with school-age children have a higher exposure risk.

If you’ve had strep once, you can get it again. A previous infection doesn’t create lasting immunity because there are many different strains of group A Streptococcus. There’s no vaccine for it, so handwashing, avoiding shared utensils, and staying home during the contagious period are the main ways to limit spread.
