Tadalafil and Viagra (sildenafil) work the same way and treat the same condition, but they differ meaningfully in how long they last, how you take them, and what side effects you might experience. Both belong to the same drug class and block the same enzyme to increase blood flow, making erections easier to achieve when you’re aroused. The biggest practical difference: sildenafil works for about 4 to 6 hours, while tadalafil can last up to 36 hours.
How They Work
Both tadalafil and sildenafil block an enzyme that breaks down the chemical signal responsible for relaxing blood vessels in the penis. By slowing that breakdown, both drugs help blood flow in and sustain an erection during sexual arousal. Neither drug creates arousal on its own or works as an aphrodisiac.
The key biochemical difference is in how precisely each drug targets that one enzyme versus related enzymes elsewhere in the body. Sildenafil also partially affects a closely related enzyme found in the retina, which is why some men notice a temporary blue tint to their vision. Tadalafil largely avoids the retina but affects a similar enzyme in skeletal muscle, which explains why it’s more likely to cause muscle aches and back pain.
Duration: The Biggest Practical Difference
Sildenafil reaches its peak concentration in about an hour, and most men find it works within 30 minutes. Its effects typically fade within 4 to 6 hours. That means you need to plan around it, taking it roughly an hour before sexual activity.
Tadalafil lasts far longer, with a duration of action stretching up to 36 hours. This extended window is why it’s sometimes called “the weekend pill.” You can take it Friday evening and still have its support on Sunday morning. That longer duration is the single most common reason men prefer it in head-to-head studies. In one crossover trial where men tried both drugs, 69% preferred tadalafil, and the top reason was the ability to get an erection long after taking the medication. Men described less time pressure and more spontaneity.
The 31% who preferred sildenafil most often cited reliability, with 57% of that group saying their top reason was being “able to get an erection every time.” Some men like the predictability of a shorter-acting drug that’s clearly “on” and then “off.”
Daily Dosing Option
Because tadalafil stays active in the body so much longer, it’s available as a low-dose daily pill. At a small daily dose taken at the same time each day, tadalafil maintains a steady level in your system so you don’t need to plan around a pill at all. This option doesn’t exist for sildenafil, which is only taken as needed before sex.
The daily approach is also FDA-approved for treating an enlarged prostate (benign prostatic hyperplasia, or BPH), a condition that causes frequent urination, weak stream, and difficulty emptying the bladder. Sildenafil has no equivalent approval. So if you have both erectile dysfunction and urinary symptoms from an enlarged prostate, tadalafil can address both with one daily pill.
Food and Timing
Sildenafil’s prescribing information notes that a high-fat meal can reduce its peak blood concentration by about 29% and delay absorption by roughly an hour. In practice, clinical studies suggest this doesn’t dramatically reduce effectiveness for most men, but many doctors still recommend taking it on a lighter stomach when possible.
Tadalafil is less sensitive to food. A high-fat meal can slightly reduce its peak concentration, but it doesn’t delay how quickly it’s absorbed. Because tadalafil’s window of action is so long, minor shifts in absorption timing matter much less than they do with a 4-hour drug.
Side Effects
The most common side effects overlap: headache, indigestion, and nasal congestion occur at similar rates with both drugs. Where they diverge is informative.
- Flushing (facial redness and warmth) is more common with sildenafil.
- Muscle aches and back pain are more common with tadalafil, typically appearing 12 to 24 hours after taking it and resolving within a couple of days. This happens because tadalafil interacts with a related enzyme found in skeletal muscle tissue.
- Vision changes (a blue-green tint or increased light sensitivity) occasionally occur with sildenafil but are rare with tadalafil.
Neither drug’s side effects are dangerous for most men. They tend to be mild and temporary, and they often decrease with continued use as your body adjusts.
The Same Critical Safety Rule
Both tadalafil and sildenafil carry the same serious contraindication: never combine either one with nitrate medications, such as nitroglycerin tablets or patches prescribed for chest pain. Both drugs amplify the blood-pressure-lowering effect of nitrates, which can cause a dangerous drop in blood pressure. With sildenafil, this interaction clears relatively quickly because the drug leaves your system in hours. With tadalafil, the interaction lasts longer. Research shows the blood pressure interaction between tadalafil and nitroglycerin persists for a full 24 hours, though it’s no longer detectable at 48 hours. If you take any form of nitrate for heart disease, neither drug is safe to use.
Which One Is Better?
Neither drug consistently outperforms the other in clinical effectiveness. Meta-analyses comparing the two find no significant difference in the percentage of men who achieve successful erections. The choice comes down to how each one fits your life.
Tadalafil suits men who want flexibility, spontaneity, and the option to not think about timing. It’s also the better choice if you have urinary symptoms from an enlarged prostate. Sildenafil may appeal to men who prefer a shorter-acting drug with a clear on-off window, or who experience bothersome back pain or muscle aches from tadalafil. Both are available as generics, which has made cost less of a deciding factor than it once was.
Many men try both before settling on one. In crossover studies where participants experienced each drug firsthand, preferences split roughly 70/30 in favor of tadalafil, but that still means nearly a third of men preferred sildenafil for their own reasons. The best choice is the one that matches how you want to use it.
