There is no medication, eye drop, or lifestyle change that can cure cataracts. Surgery is the only way to remove them, and it is one of the most commonly performed and successful procedures in medicine, with roughly 3 million surgeries done in the United States each year. The clouded natural lens is physically removed and replaced with a clear artificial one, restoring vision in the vast majority of cases.
Why Surgery Is the Only Option
A cataract is not a growth or a film sitting on top of your eye. It’s a gradual clouding of the lens itself, the structure inside your eye that focuses light onto the retina. The proteins in the lens clump together over time, turning it opaque. Because the damage happens within the lens structure, no drug can reverse it. The only fix is to take the old lens out and put a new one in.
The most common technique uses a small probe inserted into the eye that emits ultrasound energy to break the clouded lens into tiny fragments. Those fragments are suctioned out, and a clear artificial lens (called an intraocular lens) is placed in the same capsule where the natural lens sat. The incision is small enough that stitches are rarely needed.
How Well Surgery Works
Cataract surgery has a high success rate. Data from a large UK study found that nearly 95% of eyes with no other eye conditions achieved 20/40 vision or better after surgery, which is the threshold for legally driving without corrective lenses. For people who had other eye problems alongside their cataracts, the rate was still about 80%. A separate U.S. database reported that 57% of patients achieved 20/25 or better vision, meaning most people see quite well after the procedure.
One thing patients often notice immediately is color. A cataract tints everything yellowish or brownish as it worsens. After surgery, colors look noticeably brighter and sharper because light passes through a perfectly clear artificial lens instead of the clouded natural one.
Choosing an Artificial Lens
The artificial lens you receive affects how your vision works day to day, so it’s worth understanding your options before surgery.
- Monofocal lenses correct vision at one distance only. Most people choose to set them for distance vision (helpful for driving, watching TV), then wear reading glasses for close-up tasks. These are the standard lens and are typically covered by insurance.
- Multifocal lenses have several focal zones built in, allowing your brain to select the right focus for different tasks like reading or driving. They can reduce or eliminate the need for glasses, though some people need time to adjust to them.
- Toric lenses are designed for people with astigmatism. They correct the uneven curvature of the eye that causes blurred or distorted vision and can reduce glare and halos. Toric lenses come in monofocal and multifocal versions.
Multifocal and toric lenses typically cost more out of pocket. Medicare Part B covers cataract surgery with a conventional (monofocal) lens, but upgrading to a premium lens usually means paying the difference yourself.
What Recovery Looks Like
Full recovery takes about four weeks, but most people notice clearer vision within a few days. The procedure itself is outpatient, meaning you go home the same day. You’ll wear an eye shield while sleeping for the first stretch of recovery and use prescribed eye drops to prevent infection and control inflammation.
During the healing period, you’ll need to avoid getting water, soap, or shampoo in the operated eye. Rubbing or pressing on the eye is off limits. Swimming, heavy lifting, bending over, and wearing eye makeup are all activities your surgeon will give you a specific green light for, usually within a few weeks. Most people return to normal routines, including driving, relatively quickly.
Can Cataracts Come Back After Surgery?
A cataract itself cannot return because the natural lens has been permanently removed. However, something called posterior capsule opacification can develop, and it’s common enough that people sometimes call it a “secondary cataract.” This happens when the thin membrane that holds the artificial lens in place becomes cloudy over time. It occurs in 20% to 50% of patients within two to five years of surgery.
The fix is a quick laser procedure that creates a small opening in the cloudy membrane, restoring clear vision. It takes only a few minutes, is painless, and is done in a doctor’s office. The cloudiness does not return after this treatment.
Slowing Cataracts Before Surgery
While nothing reverses cataracts, certain habits can slow their progression and delay the point where surgery becomes necessary. Ultraviolet light exposure is one of the strongest modifiable risk factors. Wearing sunglasses that block 99% to 100% of both UV-A and UV-B rays, along with a brimmed hat, makes a meaningful difference over years of cumulative exposure.
Smoking accelerates cataract formation, and quitting reduces ongoing risk. A diet rich in antioxidants, particularly vitamins A, C, and E, along with lutein and zeaxanthin (found in leafy greens, eggs, and colorful fruits), supports lens health. People with diabetes face a higher risk of cataracts, so keeping blood sugar well controlled is another important lever. Certain medications, especially long-term corticosteroids, also increase risk.
None of these strategies will make an existing cataract disappear. But for someone with early-stage clouding who isn’t ready for surgery, they can buy meaningful time before the lens becomes cloudy enough to interfere with daily life.
