There is no cure for feline leukemia virus (FeLV). No drug, surgery, or therapy can eliminate the virus once a cat is infected. However, the diagnosis is not an automatic death sentence. Some cats clear the virus on their own, others live for years with proper management, and experimental treatments are showing early promise for specific complications.
Why FeLV Can’t Be Eliminated
FeLV is a retrovirus, meaning it inserts its genetic material directly into the cat’s own DNA. Once that integration happens, the virus becomes part of the cat’s genome, and no current treatment can selectively remove it. This is similar to why HIV in humans can be managed but not cured. The viral DNA sits inside cells and can reactivate, making true eradication essentially impossible with today’s medicine.
Some antiviral therapies can reduce the amount of virus circulating in a cat’s bloodstream, but as the Cornell Feline Health Center notes, these treatments may cause significant side effects and aren’t effective or available in all cases. They manage the viral load rather than clear the infection.
Not Every Infected Cat Gets Sick
One of the most important things to understand about FeLV is that infection doesn’t follow a single path. Cats exposed to the virus fall into three broad categories, and the outcome varies dramatically depending on which one applies to your cat.
Abortive infection is the best-case scenario. The cat’s immune system fights off the virus immediately after initial exposure, before it can spread beyond the local lymph tissue. The only trace left is antibodies. These cats don’t get sick and don’t carry the virus going forward.
Regressive infection means the immune system stopped the virus before or shortly after it reached the bone marrow. The cat isn’t actively shedding virus and won’t test positive on a standard screening test, but viral DNA remains embedded in cells. These cats can live relatively normal lives, though reactivation is possible under stress or immune suppression. They can still develop serious complications: in one Brazilian study, 45% of cats with regressive infections presented with anemia and about 9% developed lymphoma.
Progressive infection is the most serious outcome. The immune system failed to contain the virus, which continues replicating and circulating in the bloodstream. These cats test positive on both standard and confirmatory tests, actively shed the virus, and face the highest risk of developing fatal complications. In the same study, cats with progressive infection showed lymphoma in 38.5% of cases, anemia in 24.4%, and leukemia (cancer of the blood cells) in about 18%.
How Long FeLV-Positive Cats Live
Survival times vary widely based on the type of infection and how early the cat receives supportive care. A study of cats at a referral clinic in Italy found that the median survival time for FeLV-positive cats was about 714 days, roughly two years. For comparison, cats without either FeLV or FIV had a median survival of nearly 11 years. Cats with regressive infections generally live longer than those with progressive infections, sometimes significantly so.
Some progressively infected cats stay symptom-free for years before developing problems. The European Advisory Board on Cat Diseases notes that while life expectancy is reduced for these cats, an asymptomatic period of several years is not unusual. Keeping an FeLV-positive cat indoors, minimizing stress, and staying on top of regular veterinary checkups all help extend that window.
What Treatment Actually Looks Like
Since there’s no way to eliminate the virus itself, treatment focuses entirely on managing whatever problems arise. That means antibiotics when secondary bacterial infections take hold, blood transfusions for severe anemia, and sometimes chemotherapy if lymphoma develops. The approach is reactive: your vet addresses each complication as it appears rather than treating the underlying virus.
Keeping your cat indoors serves two purposes. It protects other cats from exposure (FeLV spreads through saliva, nasal secretions, and shared food bowls) and it shields your cat from infections their compromised immune system might struggle to fight. Stress reduction also matters, particularly for cats with regressive infections, because immune suppression from chronic stress can trigger viral reactivation.
Routine wellness visits become more important for FeLV-positive cats. Catching anemia, weight loss, or swollen lymph nodes early gives your vet more options and typically leads to better outcomes for each individual complication.
Experimental Therapies
Stem cell therapy has shown early but limited promise. In one case report, a four-year-old FeLV-positive cat with severe anemia that wasn’t responding to standard treatment received 20 million mesenchymal stromal cells through combined delivery routes. Within a month, the cat’s blood counts improved significantly, and it remained clinically stable without further therapy for 32 months. That’s an encouraging result, but it’s a single case, not a proven treatment. No studies have yet tested this approach in larger groups of FeLV-infected cats with bone marrow problems.
In human medicine, similar stem cell approaches have shown modest success for comparable blood disorders, with about a third of patients achieving meaningful improvement in one study. Whether those results translate to cats remains an open question. For now, stem cell therapy for FeLV-related complications is not widely available and remains firmly in the experimental category.
Getting an Accurate Diagnosis
If your cat tested positive on an initial screening test, it’s worth understanding what that result means before assuming the worst. The standard screening test detects a viral protein in the blood and catches both progressive and some regressive infections. A positive result should always be confirmed with a second type of test that looks for the virus inside white blood cells. If both tests are positive, the cat almost certainly has a progressive infection. If the first test is positive but the second is negative, the infection may be regressive, and retesting in a few weeks can help clarify.
This distinction matters because it directly affects prognosis. A cat confirmed as progressively infected faces a meaningfully different future than one whose immune system has already contained the virus to a regressive state. Knowing which category your cat falls into helps you and your vet plan appropriate monitoring and care.
Prevention for Other Cats
A vaccine for FeLV exists and is recommended for all kittens and for adult cats with outdoor access or exposure to cats of unknown status. The vaccine doesn’t help cats already infected, but it substantially reduces the risk for uninfected cats. If you have both FeLV-positive and negative cats in the same household, keeping them separated and not sharing food bowls, water dishes, or litter boxes reduces transmission risk. The virus is fragile outside the body and doesn’t survive long in the environment, so casual contact with contaminated surfaces is a lower risk than direct cat-to-cat contact through grooming, biting, or sharing dishes.
