How Pregnancy Tests Work
Home pregnancy tests detect a hormone called hCG in your urine. Your body produces hCG after a fertilized egg implants in the uterine lining, and levels rise rapidly in early pregnancy. The test strips are designed to turn positive once hCG crosses a specific threshold, typically around 25 mIU/mL. A false positive means something other than a viable pregnancy raised your hCG levels high enough to trigger that result.
Chemical Pregnancy: The Most Common Cause
The single most frequent reason for a “false” positive is a chemical pregnancy, which is an extremely early miscarriage that happens shortly after a fertilized egg implants. In a chemical pregnancy, your body does produce real hCG, and the test correctly detects it. But the pregnancy stops developing before it’s ever visible on an ultrasound, often before your period is even late.
Chemical pregnancies are estimated to account for 13 to 22 percent of all pregnancies. Before today’s highly sensitive tests existed, most people would never have known they were briefly pregnant. They would have simply gotten their period a few days late. Now, because tests can pick up very low levels of hCG, these very early losses are detected more often, creating a situation where you get a positive test followed by bleeding and a negative test days later.
If you test very early (before your missed period) and get a positive result that later turns negative, a chemical pregnancy is the most likely explanation. It’s not a flaw in the test. It’s a pregnancy that was real but ended on its own.
Fertility Medications
If you’re undergoing fertility treatment, certain injectable medications contain hCG itself. These are commonly used to trigger ovulation. Because the test is designed to detect that exact hormone, taking a test too soon after an injection will produce a positive result that has nothing to do with pregnancy.
The general guidance is to wait at least 10 to 14 days after an hCG injection before testing. Your fertility clinic will typically tell you exactly when to test or will use a blood draw instead, which gives a precise hCG number rather than a simple yes-or-no.
Recent Miscarriage or Pregnancy Loss
After a miscarriage, ectopic pregnancy, or abortion, hCG doesn’t vanish from your body overnight. A 2013 study of 443 women who had miscarriages found that hCG levels dropped by 35 to 50 percent within two days and by 66 to 87 percent within seven days. That’s a significant decline, but it still means detectable levels can linger for a week to several weeks after the loss, depending on how far along the pregnancy was.
If you take a pregnancy test during this window, you can get a positive result from the previous pregnancy rather than a new one. This is one reason doctors sometimes monitor hCG levels with blood tests after a loss, to confirm the hormone has fully cleared before interpreting any new results.
Menopause and Perimenopause
This one surprises most people. As estrogen levels drop during menopause, your pituitary gland can start producing small amounts of hCG on its own. Among postmenopausal individuals over age 55, up to 8 percent will have hCG levels high enough to trigger a positive result on a standard test, according to data from the American College of Obstetricians and Gynecologists.
This is why the recommended cutoff for interpreting hCG in that age group is higher (14 international units/L instead of 5). If you’re perimenopausal or postmenopausal and get an unexpected positive test, a blood test that also checks other hormone levels can clarify whether the hCG is coming from the pituitary rather than a pregnancy.
Certain Medical Conditions
Some ovarian conditions and, rarely, certain types of tumors can produce hCG. Chronic kidney disease has also been associated with slightly elevated hCG levels, likely because the kidneys play a role in clearing the hormone from the body and because the pituitary may compensate with extra production.
These causes are uncommon, but they’re worth knowing about. If you get a persistent positive pregnancy test and an ultrasound shows no pregnancy, your doctor will likely investigate further with blood work to determine where the hCG is coming from.
Evaporation Lines: The Technical False Alarm
Not every confusing result is truly a false positive. Sometimes the issue is an evaporation line, a faint, colorless mark that appears on the test strip as urine dries. It looks vaguely like a second line, and it can be easy to mistake for a positive result, especially if you’re anxiously staring at the test.
Evaporation lines appear after the test’s reaction window has passed. Most tests instruct you to read your result within two to five minutes. If you come back to a test 10, 20, or 30 minutes later and see a faint mark that wasn’t there before, that’s likely an evaporation line, not a true positive. A genuine positive line has color (pink or blue, depending on the brand) and shows up within the specified time frame.
