Is Toujeo the Same as Lantus? Similarities and Differences

Toujeo and Lantus contain the exact same insulin molecule, but they are not the same product and cannot be used interchangeably. Lantus begins working within 3 to 4 hours of injection and lasts about 24 hours. Toujeo takes roughly 6 hours to begin lowering blood sugar and also lasts around 24 hours, though its activity extends more evenly across that window. That slower onset and steadier release is the main clinical advantage Toujeo was designed to offer.

Lower Risk of Low Blood Sugar

The smoother insulin release with Toujeo translates into fewer episodes of hypoglycemia, which is the most immediate safety concern with any insulin. A meta-analysis of clinical trials found that people with type 2 diabetes using Toujeo had significantly lower rates of both nighttime and total low blood sugar episodes compared to those on Lantus. For people with type 1 diabetes, the safety profile was comparable between the two.

Individual case data illustrates the difference vividly. One published case report tracked a 28-year-old man with diabetes who averaged 4.8 low blood sugar episodes per week on Lantus, including 1.5 severe episodes weekly. While a single case isn’t definitive, this pattern aligns with what the larger trials consistently show.

Blood Sugar Control Is Equivalent

Toujeo’s advantage isn’t that it controls blood sugar better; it’s that it achieves the same control with less risk of dangerous dips.

However, clinical studies show that adults often end up needing 10 to 18% more Toujeo than their previous Lantus dose to achieve the same blood sugar targets. Children typically need about 8% more.
