Tresiba and Lantus are not the same insulin. They contain different active ingredients, work through different mechanisms in the body, and last for different lengths of time. Both are long-acting basal insulins prescribed to keep blood sugar steady between meals and overnight, but the similarities largely end there.
Different Insulins, Different Molecules
Tresiba’s active ingredient is insulin degludec, while Lantus contains insulin glargine. Both are synthetic versions of human insulin, but they’ve been engineered differently at the molecular level to slow their absorption after injection.
When you inject Lantus, it forms tiny clusters called microprecipitates in the tissue under your skin. These clusters dissolve gradually, releasing insulin at a relatively steady rate over about 24 hours. Tresiba takes a completely different approach: after injection, it forms long chains of linked insulin molecules. Individual insulin units slowly peel off from these chains, creating an ultra-long release that lasts up to 42 hours. That’s nearly twice the duration of Lantus.
This difference in duration has a practical benefit. Lantus needs to be taken at roughly the same time every day to maintain consistent coverage. Tresiba offers more flexibility because its longer action creates an overlapping buffer. If you’re someone who struggles with a rigid injection schedule, that extra forgiveness can matter.
How Blood Sugar Control Compares
Both insulins do a good job of controlling fasting blood sugar, and clinical trials have generally shown similar reductions in A1C between the two. The real difference shows up in the consistency of that control throughout the day and night.
Tresiba produces a flatter, more stable glucose-lowering effect with less variability from dose to dose. Lantus can have slight peaks and valleys in its activity, particularly toward the end of its 24-hour window. For many people this variation is minor and clinically insignificant, but for those who are sensitive to insulin fluctuations, the steadier profile of Tresiba can make a noticeable difference in day-to-day blood sugar readings.
Nighttime Low Blood Sugar Risk
One of the clearest advantages Tresiba has over Lantus is a lower rate of nighttime hypoglycemia, which is the type of low blood sugar that happens while you’re asleep and can be particularly dangerous because you may not wake up to treat it.
In a head-to-head clinical trial of people with type 1 diabetes who were prone to severe nighttime lows, Tresiba reduced nocturnal episodes by 28% compared to Lantus when measured at a blood sugar threshold of about 70 mg/dL. At a more serious threshold (around 54 mg/dL), the reduction was 37%. Severe hypoglycemia across the entire day dropped by 35%. In absolute terms, that translated to roughly 0.3 fewer severe episodes per person per year, which is meaningful when even one severe low can result in a medical emergency.
This lower hypoglycemia risk is one of the main clinical reasons a doctor might recommend switching from Lantus to Tresiba, especially for people who experience frequent or unpredictable lows.
Dosing and Switching Between Them
If your doctor decides to switch you from Lantus to Tresiba, the conversion is straightforward. Adults typically start Tresiba at the same number of units they were taking of Lantus, a 1:1 dose conversion. Your doctor will then adjust the dose based on how your blood sugar responds over the following days and weeks.
Tresiba is injected once daily, and because of its long duration, you have more flexibility in timing. The manufacturer notes that doses can be given at varying times of day if needed, as long as there are at least 8 hours between injections. Lantus is also once daily but works best when taken at a consistent time.
Both insulins are approved for type 1 and type 2 diabetes. Tresiba is FDA-approved for patients 1 year of age and older. Both are available as injectable pens.
Cost and Insurance Differences
Because Tresiba is a newer insulin (approved in 2015, compared to Lantus in 2000), it tends to carry a higher list price. However, actual out-of-pocket costs depend heavily on your insurance plan’s formulary. Some plans prefer Lantus or its biosimilar alternatives, while others may cover Tresiba at a comparable copay tier. Lantus also has biosimilar versions available, which can be significantly cheaper. Tresiba does not yet have a biosimilar on the market.
If cost is a factor, it’s worth checking whether your insurance covers one more favorably than the other before requesting a switch. Manufacturer savings programs exist for both, though eligibility varies.
Which One Is Better
Neither insulin is universally better. Both effectively control blood sugar over 24 hours, and for many people the day-to-day experience of using them feels similar. Tresiba offers a more stable action profile, greater dosing flexibility, and a lower risk of nighttime lows. Lantus has a longer track record, broader availability of cheaper biosimilar options, and works perfectly well for the majority of people who use it.
The choice between them usually comes down to how well your current insulin is working. If you’re on Lantus with good control and no problematic lows, there may be little reason to switch. If you’re experiencing unpredictable blood sugar swings, frequent nighttime lows, or struggling with a strict injection schedule, Tresiba addresses each of those specific problems.
