Tylenol is not an NSAID. Tylenol’s active ingredient, acetaminophen, belongs to a separate class of medications called analgesics and antipyretics, meaning it relieves pain and reduces fever. NSAIDs (non-steroidal anti-inflammatory drugs) include medications like ibuprofen (Advil, Motrin) and naproxen (Aleve). The distinction matters because these drugs work differently in the body, carry different risks, and are better suited for different situations.
How Acetaminophen Differs From NSAIDs
The key difference comes down to inflammation. NSAIDs work by blocking enzymes called COX-1 and COX-2, which produce compounds that trigger pain, fever, and inflammation throughout the body. Because they block these enzymes in inflamed tissue, NSAIDs reduce swelling, redness, and the pain that comes with it.
Acetaminophen also appears to interact with COX enzymes, but only weakly, and primarily in the brain rather than at the site of injury or inflammation. It can reduce pain signals and lower a fever, but it does essentially nothing to reduce swelling. At normal doses, acetaminophen has little effect when levels of inflammatory compounds are high, which is why it’s not effective for conditions driven by significant inflammation, like rheumatoid arthritis.
In practical terms: both can help with a headache or a fever. But if your knee is swollen after a sprain, an NSAID will address the swelling while acetaminophen won’t.
When Each One Works Best
Acetaminophen is a good choice for general pain relief, such as tension headaches, mild body aches, or bringing down a fever. It’s often recommended for people who can’t tolerate NSAIDs due to stomach sensitivity or other reasons.
NSAIDs are the better option when inflammation is part of the problem. This includes muscle strains, joint pain, menstrual cramps, dental pain, and arthritis flare-ups. The anti-inflammatory effect makes a meaningful difference in these situations. NSAIDs also have mild blood-thinning properties that acetaminophen lacks entirely.
Different Risks for Different Organs
One of the most important reasons to know the difference between acetaminophen and NSAIDs is that they stress different parts of the body.
Acetaminophen is processed by the liver. The maximum safe dose for adults is 4,000 mg in 24 hours, but liver damage can occur at or even below that threshold, especially in people who drink alcohol regularly. Acetaminophen is also a hidden ingredient in many combination products, including cold medicines, sleep aids, and prescription painkillers, so it’s easy to take more than you realize.
NSAIDs, on the other hand, are harder on the stomach and kidneys. They can irritate the stomach lining and increase the risk of ulcers and gastrointestinal bleeding, particularly with long-term use. They can also reduce blood flow to the kidneys and raise blood pressure, which makes them a poor fit for people with kidney disease or certain heart conditions.
Can You Take Them Together?
Because acetaminophen and NSAIDs work through different mechanisms and affect different organs, they can generally be taken together for stronger pain relief. Clinical studies in patients with dental pain, postoperative pain, and musculoskeletal conditions have found that combining the two provides additive pain relief. This combination can also allow lower doses of each, which reduces the risk of side effects from either one.
What you should not do is combine two NSAIDs, such as ibuprofen and naproxen, at the same time. That increases stomach and kidney risks without a proportional increase in pain relief.
How to Tell What You’re Taking
The brand names can make this confusing. Here’s a quick breakdown:
- Acetaminophen (not an NSAID): Tylenol, and many store-brand equivalents labeled “acetaminophen”
- Ibuprofen (NSAID): Advil, Motrin
- Naproxen (NSAID): Aleve
- Aspirin (NSAID): Bayer, Bufferin
Some products contain both acetaminophen and an NSAID. Excedrin Extra Strength, for example, combines acetaminophen with aspirin and caffeine. Always check the active ingredients on the Drug Facts label, especially if you’re already taking one of these medications separately. Doubling up on acetaminophen without realizing it is one of the most common causes of accidental overdose.
