Acetaminophen works differently from anti-inflammatory drugs like ibuprofen and naproxen. Those medications, called NSAIDs, block an enzyme called COX at the site of injury or inflammation. This is what actually reduces swelling, redness, and warmth in your joints. Acetaminophen has only a very weak effect on that enzyme at the pain site. The guidelines note that clinical trial effect sizes for acetaminophen are very small, meaning few people who take it experience meaningful benefit. Meta-analyses have suggested that acetaminophen used alone may be no better than a placebo for most people over the long term. Members of the organization’s own patient panel stated that, for most individuals, acetaminophen is simply ineffective for osteoarthritis.
So why does it still appear in the guidelines at all? Because some people cannot tolerate NSAIDs. If you have kidney problems, stomach ulcers, a history of gastrointestinal bleeding, or you take blood thinners, NSAIDs may be off the table. In those cases, acetaminophen may be appropriate for short-term or occasional use as a second-choice option, not because it works particularly well, but because the alternatives carry risks you can’t afford.
When NSAIDs Are the Better Choice

The FDA sets the maximum at 4,000 mg per day for adults, though many manufacturers and physicians recommend staying closer to 3,000 mg. The ACR guidelines specifically flag that regular monitoring for liver damage is needed if you take acetaminophen consistently, particularly near that upper limit.
The risk of liver damage is the main safety concern. Acetaminophen is processed by the liver, and exceeding the recommended dose, even modestly, can cause serious harm. This risk increases significantly if you have a history of liver disease or drink three or more alcoholic beverages a day. It also climbs if you’re unknowingly doubling up: acetaminophen is an ingredient in hundreds of combination products, including cold medicines, sleep aids, and prescription painkillers. Always check the active ingredients on every medication you take.
Because Tylenol Arthritis caplets are extended-release, they should never be crushed or split. Doing so releases the full dose at once rather than gradually, which can spike blood levels of acetaminophen beyond what your liver can safely handle.
Choosing the Right OTC Pain Reliever
The simplest way to think about it: if your pain involves visible or felt inflammation (swelling, warmth, stiffness that improves with movement), an NSAID is the more targeted choice. If your pain is more of a dull ache without obvious swelling, or if you can’t take NSAIDs safely, acetaminophen can take the edge off, though the clinical evidence suggests it won’t do much more than that for most people with arthritis.
