Is Vietnamese Coffee Healthy? Benefits and Risks

Vietnamese coffee can be a healthy drink, but it depends heavily on how it’s prepared and what goes into the cup. The coffee itself, typically brewed from robusta beans through a small metal filter called a phin, delivers a strong dose of antioxidants and caffeine. The potential health concerns come from three places: the sweetened condensed milk that’s traditionally added, the unfiltered brewing method, and the quality of the beans themselves.

What Makes Vietnamese Coffee Different

Most Vietnamese coffee uses robusta beans rather than the arabica beans common in Western coffee shops. Robusta contains nearly twice as much caffeine per weight and higher concentrations of chlorogenic acids, which are the main antioxidant compounds in coffee. A traditional phin-brewed cup produces a small, concentrated serving of about 2 ounces with roughly 66 mg of caffeine. A double serving (4 ounces) lands around 130 mg, which is comparable to a standard 8-ounce cup of American drip coffee at 80 to 120 mg.

The phin filter is a slow-drip metal device that sits on top of the cup. Unlike a paper filter, it doesn’t trap the oily compounds in coffee. This matters for health reasons covered below.

The Robusta Bean Advantage

Robusta’s higher chlorogenic acid content is genuinely beneficial. These compounds help regulate blood sugar and reduce inflammation. In a study comparing robusta and arabica coffee in a rat model of type 2 diabetes, both types improved insulin sensitivity and reduced liver fat. But robusta performed better on several measures: it was more effective at controlling long-term blood sugar markers, improved lipid profiles, and increased levels of adiponectin, a hormone that helps regulate metabolism. Robusta also suppressed a liver enzyme involved in releasing stored glucose into the bloodstream at nearly three times the rate of the control group.

None of this means Vietnamese coffee is a diabetes treatment. But the robusta beans at its core have a metabolic profile that’s at least as favorable as arabica, and possibly more so.

The Condensed Milk Problem

Traditional Vietnamese coffee, whether served hot (cà phê sữa nóng) or iced (cà phê sữa đá), is made with sweetened condensed milk. This is where the nutritional picture shifts. Just one ounce of sweetened condensed milk, roughly two tablespoons, adds 90 calories and over 15 grams of sugar. Many servings use more than that.

For context, the American Heart Association recommends no more than 25 grams of added sugar per day for women and 36 grams for men. A single Vietnamese iced coffee can eat up half or more of that limit before you’ve had breakfast. If you’re drinking one daily, the sugar and calorie load adds up fast.

Black Vietnamese coffee brewed through a phin has essentially zero calories. The health gap between black and sweetened versions is enormous. If you enjoy the strong flavor of robusta but want to avoid the sugar, you can swap condensed milk for unsweetened coconut milk, which adds healthy medium-chain fats and a creamy texture without the sugar hit, or simply drink it black.

Egg Coffee and Other Variants

Cà phê trứng, or egg coffee, is a Hanoi specialty made by whipping egg yolks with condensed milk into a custard-like foam layered over strong coffee. It’s delicious, but it’s closer to a dessert than a health drink. A typical 8-ounce serving runs 250 to 350 calories, with 25 to 35 grams of carbohydrates (mostly sugar), 12 to 18 grams of fat, and 8 to 12 grams of protein from the eggs and dairy. The protein content is a genuine upside compared to regular sweetened coffee, but the calorie count makes this an occasional treat rather than a daily habit for most people.

Unfiltered Brewing and Cholesterol

The phin filter produces what’s technically “unfiltered” coffee, similar to French press or Turkish coffee. Metal filters allow oily compounds called diterpenes (cafestol and kahweol) to pass into the cup. These compounds raise LDL cholesterol and can elevate liver enzymes. In one study, consuming coffee grounds daily for three weeks increased total cholesterol by about 25 mg/dL and raised a liver enzyme marker significantly compared to controls.

For most people drinking one cup a day, this effect is modest. But if you already have high cholesterol, or you’re drinking multiple phin-brewed cups daily, it’s worth knowing that switching to a paper-filtered brewing method would remove most of these compounds. Pouring your phin coffee through a paper filter is a simple workaround that preserves the flavor while reducing diterpene exposure.

Caffeine and Blood Pressure

Because robusta beans pack roughly twice the caffeine of arabica, Vietnamese coffee can hit harder than you’d expect from a small cup. Caffeine raises blood pressure by increasing resistance in blood vessels, an effect that starts within 15 minutes of drinking and lasts 3 to 4 hours. At typical dietary doses (90 to 350 mg, or one to four cups), blood pressure can rise anywhere from 3/4 mmHg to 14/13 mmHg.

For people with normal blood pressure, this temporary bump usually returns to baseline within about 90 minutes. For people with hypertension, the effect is larger and lasts longer. A single dose equivalent to 2 to 3 cups of coffee raised diastolic blood pressure by 10 mmHg for a full three hours in people with high blood pressure. The effects of caffeine and stress are additive, meaning your morning Vietnamese coffee before a tense meeting could push your blood pressure higher than either factor alone.

If you’re sensitive to caffeine or managing blood pressure, sticking to a single traditional serving (2 ounces from one phin) keeps your intake at a moderate 66 mg, well below the threshold where blood pressure effects plateau.

Bean Quality Is a Real Concern

One issue that doesn’t get enough attention outside Vietnam is the prevalence of adulterated coffee. Historically, when beans were scarce before the 1990s, roasters stretched supplies by mixing in corn, soy, rice wine, butter, and even fish sauce. That practice became embedded in the industry, and some manufacturers still exploit it. Inspectors have found products with as little as 10 percent real coffee, bulked out with soybean powder, corn starch, and foaming agents to mimic the dark, thick appearance consumers associate with “strong” coffee.

In one inspection, over 100 kilograms of coffee powder sold in grocery stores showed caffeine levels of 0.41 to 0.46 percent, far below the expected rate of over 1 percent for genuine coffee. One store alone had produced and sold 344 tons of substandard coffee products in less than a year. Some instant coffee products on the Vietnamese market contain no real coffee at all.

This doesn’t apply to all Vietnamese coffee, and reputable specialty brands (both in Vietnam and exported) use 100 percent robusta or arabica beans. But if you’re buying Vietnamese coffee for its health benefits, sourcing matters. Look for brands that are transparent about their bean origin and processing. If a pre-ground Vietnamese coffee is unusually cheap or produces an unnaturally thick, foamy brew, that’s a red flag.

The Bottom Line on Vietnamese Coffee

Black Vietnamese phin coffee made from quality robusta beans is a genuinely healthy drink. It’s rich in antioxidants, supports metabolic health, and delivers caffeine efficiently in a small serving. The traditional preparation with sweetened condensed milk turns it into a high-sugar indulgence. The unfiltered brewing adds a small cholesterol risk for heavy drinkers. And the higher caffeine content of robusta means you should be more mindful of intake if blood pressure is a concern. The coffee itself isn’t the problem. What you add to it, how much you drink, and what’s actually in the beans are what determine whether it helps or hurts.
