Vyvanse is not dangerous for the heart in most people, but it does place mild, measurable stress on your cardiovascular system every day you take it. The medication raises blood pressure by an average of 2 to 4 mmHg and heart rate by about 3 to 6 beats per minute. For a healthy person, those changes are small. For someone with an underlying heart condition, they can be significant.
The real question isn’t whether Vyvanse affects the heart (it does), but whether those effects translate into serious problems over months and years of use. The answer depends heavily on your starting cardiovascular health.
What Vyvanse Does to Your Heart
Vyvanse (lisdexamfetamine) is a stimulant, and stimulants activate the same “fight or flight” system that kicks in when you’re startled or stressed. This speeds up your heart rate and tightens blood vessels slightly, pushing blood pressure up. Those effects begin within an hour or two of taking the medication and persist through the day as the drug is active.
For context, the average blood pressure increase of 2 to 4 mmHg is roughly equivalent to drinking a cup of coffee. The heart rate bump of 3 to 6 beats per minute is similarly modest. Most people never notice these changes. But they compound over time, especially if your blood pressure or heart rate was already on the higher end before starting the medication.
The Long-Term Picture
A large study published in the Journal of the American College of Cardiology in 2024 looked at long-term cardiovascular outcomes in adults taking ADHD medications. Compared to people who had previously used stimulants but stopped, those taking at least a standard daily dose had a 10-year absolute risk of heart failure of 1.2%, versus 0.7% in the group that had stopped. That translates to roughly 70% higher relative risk of heart failure with ongoing use.
Those numbers deserve some perspective. A 1.2% risk over 10 years means that about 1 in 83 long-term users developed heart failure during the study period. That’s low in absolute terms, but it’s not zero, and the gap between users and non-users is real. The risk appears to grow with higher doses and longer duration of use.
Separate data from the Agency for Healthcare Research and Quality tracked over 844,000 person-years of follow-up among young and middle-aged adults. The rate of sudden cardiac death among current ADHD medication users was 0.30 per 1,000 person-years, identical to the overall study population rate. In other words, current users in that study did not have a noticeably elevated rate of sudden cardiac death compared to the broader group.
Who Should Avoid Vyvanse
The FDA’s prescribing label is specific about who should not take this medication. You should avoid Vyvanse if you have known structural heart abnormalities, cardiomyopathy (a disease of the heart muscle), serious heart rhythm disorders, or coronary artery disease. These conditions make the heart vulnerable to the additional stress that stimulants create, and rare but serious events like heart attack, stroke, and sudden death have been reported in adults taking stimulants at normal doses.
In children, the risk profile gets particular attention. Sudden death has been reported in pediatric patients who had structural cardiac abnormalities or other serious heart problems. This is why prescribers are expected to screen for cardiac disease before writing the first prescription, including asking about family history of sudden death or dangerous heart rhythms.
It’s worth noting that Vyvanse does not carry a boxed warning (the FDA’s most serious label warning) for heart risks. Its only boxed warning relates to the potential for abuse and dependence. The cardiovascular concerns are listed one tier below, in the “Warnings and Precautions” section.
What Monitoring Looks Like
The American Heart Association recommends that anyone taking ADHD stimulants have their blood pressure and pulse checked within the first 1 to 3 months, then at follow-up visits every 6 to 12 months. During dose changes, monitoring should happen more frequently. This isn’t complicated testing. It’s the standard blood pressure cuff and pulse check you’d get at any office visit.
What you’re watching for at home is also straightforward. Chest pain during physical activity, fainting or near-fainting episodes, and a racing or irregular heartbeat are all signals to bring up with your prescriber promptly. These symptoms are uncommon, but they can indicate that the medication is putting more strain on your heart than expected.
Risk Factors That Change the Equation
Your individual risk on Vyvanse depends on what you bring to the table before the first pill. High blood pressure, even the “borderline” kind that doesn’t yet require its own medication, means you’re starting from a higher baseline. The 2 to 4 mmHg bump from Vyvanse matters more when you’re already at 135/85 than when you’re at 110/70.
Age is another factor. The cardiovascular system becomes less flexible over time, and blood vessels stiffen naturally with age. Adults in their 40s and 50s starting Vyvanse carry more background cardiovascular risk than a teenager does, which is part of why the long-term heart failure data in adults warrants attention. Obesity, diabetes, smoking, and a family history of early heart disease all layer additional risk on top of whatever the stimulant itself contributes.
Caffeine intake also plays a role that people often overlook. Caffeine and Vyvanse both raise heart rate and blood pressure through overlapping pathways. If you’re drinking several cups of coffee alongside your stimulant, the combined cardiovascular load is larger than either one alone.
Balancing Benefit Against Risk
Untreated ADHD carries its own health costs. People with unmanaged ADHD have higher rates of accidents, substance use, job instability, and chronic stress, all of which affect cardiovascular health indirectly. The decision to take or continue Vyvanse isn’t just about the medication’s direct effect on your heart. It’s about whether the overall picture, including the benefits of having your ADHD managed, tips in favor of treatment or against it.
For most people with a healthy heart, the cardiovascular effects of Vyvanse are measurable but modest. Regular monitoring catches problems early, and dose adjustments or switching to a non-stimulant option are available if blood pressure or heart rate climbs too high. The people who face genuinely elevated risk are those with pre-existing heart conditions, and for that group, stimulants are best avoided entirely.
