Xanax is not a barbiturate. It belongs to a completely different class of drugs called benzodiazepines. While both drug classes are sedatives that calm the nervous system, they differ in their chemical structure, how they work in the brain, and how dangerous they are in overdose. Xanax (alprazolam) was actually developed and approved by the FDA in 1981 as a safer alternative to barbiturates for treating anxiety.
How Xanax Differs From Barbiturates
Both Xanax and barbiturates target the same brain system. They both enhance the activity of GABA, a chemical messenger that slows down nerve signaling and produces feelings of calm and sedation. But they do this in fundamentally different ways.
Benzodiazepines like Xanax increase how frequently certain channels in nerve cells open in response to GABA. Think of it as making the brain’s natural braking system work more often. Barbiturates, on the other hand, increase how long those same channels stay open each time they activate. This difference matters because barbiturates can essentially force those channels open even without GABA present, which is why they’re far more dangerous at high doses. Benzodiazepines have a built-in ceiling effect: they can only amplify what GABA is already doing, not act on their own.
This is why barbiturates have a notoriously narrow margin of safety. Taking roughly ten times a therapeutic dose of a barbiturate can cause fatal respiratory or cardiovascular failure. Benzodiazepines have a much wider safety margin, which is a major reason they replaced barbiturates for anxiety and insomnia starting in the 1960s and 1970s. When Xanax launched in 1981, its manufacturer Upjohn specifically marketed it on this point, positioning it as a safer option.
Why People Confuse the Two
The confusion is understandable. Xanax and barbiturates produce similar effects: relaxation, drowsiness, reduced anxiety, and slowed reflexes. Both are controlled substances regulated by the DEA. Both carry risks of dependence and withdrawal. And both are classified as central nervous system depressants, which means mixing either one with alcohol or opioids can be life-threatening.
The DEA itself groups them in similar conversations. Its fact sheet on barbiturates lists benzodiazepines like Xanax and Valium as drugs with comparable effects. But similar effects don’t mean the same drug class. Aspirin and ibuprofen both reduce pain and inflammation, but they’re chemically distinct medications with different risk profiles.
Barbiturates Still in Use Today
Barbiturates have largely fallen out of everyday prescribing, but they haven’t disappeared entirely. Six barbiturates remain FDA-approved in the United States, each filling a narrow role that newer drugs haven’t fully replaced.
- Phenobarbital is still widely used for seizures, especially in newborns and children, and remains the most cost-effective epilepsy treatment in lower-resource countries.

Xanax, by contrast, is classified as a high-potency, short-acting benzodiazepine with a half-life of 6 to 27 hours. It’s primarily prescribed for anxiety disorders and panic disorder. For these conditions, benzodiazepines have almost entirely replaced barbiturates.
Xanax on a Drug Test
If you’re wondering whether Xanax would show up as a barbiturate on a drug test, it won’t. Standard urine drug panels test for benzodiazepines and barbiturates separately. They’re distinct categories.
Interestingly, Xanax can actually be tricky to detect even on the benzodiazepine portion of a standard drug screen. Most basic urine tests look for a common breakdown product that many benzodiazepines share, but Xanax doesn’t produce that particular metabolite. This means alprazolam can sometimes be missed entirely on a standard panel, producing a false negative for benzodiazepines. More advanced testing methods can identify it specifically, but a routine screening may not.
Risks Xanax Shares With Barbiturates
Being safer than barbiturates doesn’t make Xanax safe in absolute terms. Physical dependence can develop within weeks of daily use, and withdrawal from benzodiazepines can be severe, potentially involving seizures. Tolerance builds over time, meaning higher doses become necessary for the same effect.
The overdose picture has also shifted. While Xanax alone is less likely to be fatal than a barbiturate overdose, combining it with opioids, alcohol, or other sedatives dramatically increases the risk of fatal respiratory depression. In one study of intentional overdose cases at a major medical center, the most common sedative-hypnotics involved were benzodiazepines like diazepam and alprazolam, with no barbiturate cases at all, reflecting how completely prescribing patterns have changed.
