Xyzal and Zyrtec are so closely related that for most people, they work about equally well. Xyzal (levocetirizine) is literally half of Zyrtec (cetirizine): it’s the active portion of the same molecule, isolated and sold at half the dose. The differences between them are real but small, and which one is “better” depends on what you’re treating and how your body responds.
How the Two Drugs Are Related
Cetirizine, the ingredient in Zyrtec, is a 50/50 mix of two mirror-image molecules. One of those mirrors, levocetirizine, does essentially all the antihistamine work. The other mirror, dextrocetirizine, is about 10 times weaker and contributes almost nothing to allergy relief. Xyzal is just that active half, separated out.
This is why the standard adult dose of Zyrtec is 10 mg while the standard dose of Xyzal is 5 mg. You’re getting roughly the same amount of the ingredient that actually blocks histamine. Think of it like buying a bag of trail mix versus buying just the cashews: the active ingredient is the same, one just comes without the filler.
How They Compare for Nasal Allergies
Clinical trials comparing the two for allergic rhinitis (the sneezing, congestion, and runny nose of seasonal or year-round allergies) generally show similar results. Both significantly reduce total symptom scores compared to placebo. One 12-week trial in children with year-round allergies actually found cetirizine slightly more effective than levocetirizine at reducing overall symptom scores and improving nasal airflow by weeks 8 and 12, though quality-of-life scores were statistically equivalent between the two groups.
That result might seem counterintuitive if levocetirizine is the “active” half. One possible explanation is that the inactive enantiomer isn’t completely inert in every context, or that small differences in how the full molecule is absorbed and distributed give cetirizine a slight edge in some patients. Either way, the gap was modest, and most head-to-head data shows the two performing within a narrow range of each other.
How They Compare for Hives and Skin Reactions
For skin-related allergic responses, the comparison gets more interesting. In a controlled crossover study where researchers triggered skin reactions with histamine, both drugs suppressed the wheal (the raised bump) and flare (the surrounding redness) starting within one hour. Peak suppression was similar: cetirizine blocked about 80% of the wheal and 89% of the flare, while levocetirizine blocked about 84% and 84%, respectively. Statistically, the maximum effect was equivalent.
Where levocetirizine pulled ahead was in sustained suppression over the full dosing period. When researchers measured total histamine blocking across the entire duration of action, levocetirizine at just 2.5 mg outperformed cetirizine at 5 mg. The wheal suppression from levocetirizine also lasted slightly longer on average: 28.4 hours versus 24.4 hours for cetirizine. If you’re dealing with chronic hives and want the most consistent around-the-clock coverage, that sustained effect could matter.
Speed and Duration
Both drugs start working fast. Cetirizine typically kicks in within 20 to 60 minutes and lasts at least 24 hours, with an elimination half-life of about 8.3 hours. Levocetirizine shows significant symptom improvement within one to two hours of a single dose, and its duration of histamine suppression on the skin averages over 26 hours.
In practical terms, you won’t notice a meaningful difference in how quickly either one starts relieving your symptoms. Both are once-daily medications, and both maintain their effect through a full 24-hour cycle for most people.
Side Effects
Because levocetirizine is the purified active half, proponents argue it should cause fewer side effects at equivalent therapeutic doses. The logic is straightforward: you’re taking half the total drug load to get the same antihistamine effect. In practice, both medications share the same side effect profile. Drowsiness is the most common complaint, and both can cause dry mouth and fatigue. Some people do report feeling less sedated on Xyzal than Zyrtec, but this varies from person to person and hasn’t been consistently demonstrated in large trials.
If drowsiness is a problem for you on one, it’s worth trying the other. Individual responses to antihistamines are notoriously unpredictable.
Dosing for Children
Zyrtec has a slight advantage in pediatric flexibility. It can be used in children as young as 6 months (for specific conditions like food or bee sting allergies, at 2.5 mg), with standard dosing of 5 mg daily for ages 2 to 5 and 10 mg daily from age 6 onward. Xyzal is dosed at 2.5 mg daily for ages 2 to 5, 5 mg daily for ages 6 to 11, and 5 to 10 mg daily from age 12. For families with very young children, Zyrtec’s longer track record and broader age labeling may make it the simpler choice.
Cost and Availability
Both are available over the counter and in generic form. Generic cetirizine is one of the cheapest antihistamines you can buy, often under $10 for a 90-day supply at major pharmacies. Generic levocetirizine has become more affordable in recent years but still tends to cost slightly more. If price is a factor and the two work equally well for you, cetirizine is the more economical option.
Which One to Choose
For straightforward seasonal or year-round nasal allergies, most people will do equally well on either drug. Zyrtec (cetirizine) is cheaper, available in more pediatric formulations, and performed at least as well as levocetirizine for nasal symptoms in head-to-head trials. If you’re already using Zyrtec and it controls your symptoms, there’s no compelling reason to switch.
Xyzal (levocetirizine) may have a slight edge for chronic hives, where its longer and more sustained histamine suppression could provide more consistent relief over a 24-hour period. It’s also worth trying if you feel overly drowsy on Zyrtec and want to see if the lower total drug load makes a difference for you. But the honest answer is that these two medications are more alike than they are different, and choosing between them often comes down to personal response, price, and availability.
