The pouch physically presses against the gum tissue for extended periods, and the nicotine itself acts as a chemical irritant. Repeating this in the same spot day after day compounds the damage. Once gum tissue recedes, it does not grow back on its own, and exposed tooth roots become more vulnerable to sensitivity and decay.
