Leaky gut doesn’t have its own unique set of symptoms. Increased intestinal permeability, the medical term for what’s popularly called “leaky gut,” usually results from damage to your intestinal lining, and the symptoms you experience come from that damage and from the body-wide inflammation that follows. This makes it tricky to pin down, because the signs overlap heavily with other digestive conditions and can show up far beyond your gut.

What’s Actually Happening in Your Gut

Your intestinal lining replaces itself every five to seven days, making it one of the most regenerative tissues in your body. That lining is held together by structures called tight junctions, which act like gates between cells, controlling what passes from your intestines into your bloodstream. When those gates malfunction, larger molecules like partially digested food proteins and bacterial fragments slip through into tissue where they don’t belong.

A protein called zonulin plays a central role in opening those gates. Certain triggers, especially gluten in sensitive individuals, cause zonulin levels to rise, which loosens the tight junctions. Once those gaps form, the immune system encounters foreign material it normally wouldn’t see, and it responds with inflammation. Inflammatory signals like TNF-alpha and interferon-gamma then cause even more permeability, creating a self-reinforcing cycle: the initial breach lets in material that triggers inflammation, and that inflammation widens the breach further.

Digestive Symptoms

The most common complaints people associate with leaky gut are digestive. Bloating, excess gas, cramping, and diarrhea are typical, though none of these is specific to intestinal permeability on its own. They reflect the underlying damage to the gut lining and the immune activation happening in the intestinal wall.

Food sensitivities are a particularly telling sign. In controlled testing, researchers have exposed intestinal tissue to food antigens and watched the response in real time. In one study, 22 out of 36 patients showed a reaction within five minutes of exposure: gaps formed in the lining and the spaces between intestinal folds widened. A randomized controlled trial comparing gluten-containing and gluten-free diets found that the gluten group had more bowel movements per day and measurably greater small bowel permeability. The effect was strongest in people carrying specific genetic markers (HLA-DQ2/DQ8), the same genes associated with celiac disease. This doesn’t mean gluten causes leaky gut in everyone, but it illustrates how certain foods can worsen barrier function in susceptible people.

Symptoms Beyond the Gut

What makes leaky gut confusing is that many of its associated symptoms have nothing obvious to do with digestion. When bacterial fragments and undigested proteins cross into the bloodstream, they can trigger low-grade, body-wide inflammation. This chronic inflammatory state has been linked to a range of conditions: obesity, type 2 diabetes, arthritis, fibromyalgia, chronic fatigue syndrome, and asthma. The connection between intestinal permeability and these conditions is well-documented, though researchers are still working out exactly how much of a causal role the gut plays versus simply being one factor among many.

Joint pain and body aches are commonly reported. So are skin problems like eczema, rashes, and acne, which make sense given that inflammation in the bloodstream can affect any tissue. Fatigue that doesn’t improve with rest is another frequent complaint, overlapping with the pattern seen in chronic fatigue syndrome.

Brain Fog, Mood Changes, and the Gut-Brain Axis

Your gut and brain communicate constantly through nerve pathways, immune signals, and hormones. Research from Johns Hopkins has found that irritation in the gastrointestinal system sends signals to the central nervous system that can trigger mood changes. This helps explain why people with irritable bowel syndrome and other functional bowel problems develop depression and anxiety at higher-than-normal rates.

Cognitive symptoms are part of this picture too. Difficulty concentrating, memory lapses, and the general haziness people call “brain fog” have all been reported alongside increased intestinal permeability. Evidence suggests that digestive-system activity can directly affect thinking skills and memory, though the exact mechanisms are still being mapped. What Triggers Intestinal Permeability

Several well-studied factors damage the gut lining or directly open tight junctions:

  • Gluten raises zonulin levels, loosening the seals between intestinal cells. This effect is most pronounced in people with genetic susceptibility but can occur to a lesser degree in others.
  • NSAIDs (ibuprofen, naproxen, aspirin) are known to increase intestinal permeability, especially with regular use. Researchers specifically note that NSAID use must be ruled out when measuring permeability in studies.
  • Alcohol damages the intestinal lining directly and disrupts the balance of gut bacteria that help maintain barrier integrity.
  • Smoking is another established factor that impairs the gut barrier, significant enough that it’s controlled for in clinical permeability testing.
  • Chronic stress and poor sleep affect gut function through the same nerve pathways that connect the gut and brain, potentially weakening barrier function over time.

How Leaky Gut Is Tested

There’s no standard diagnostic test your doctor will run labeled “leaky gut.” The most established research tool is the lactulose-mannitol test. You drink a solution containing two sugars after an overnight fast, then collect all urine for the next five hours. Lactulose is a larger molecule that shouldn’t cross a healthy gut barrier easily, while mannitol is small enough to pass through normally. The ratio of the two in your urine indicates how permeable your intestinal lining is.

In healthy people, the ratio is quite low, typically around 0.03, meaning very little lactulose gets through relative to mannitol. A higher ratio suggests the tight junctions are letting larger molecules pass. However, every lab sets its own cutoff values, and the test is used primarily in research settings rather than routine clinical practice. Most gastroenterologists will focus on identifying and treating whatever underlying condition is damaging the gut lining rather than measuring permeability directly.

How the Gut Repairs Itself

The intestinal lining is the fastest-regenerating tissue in the human body, fully replacing its surface layer every five to seven days. This means that once the source of damage is removed, the physical barrier can rebuild relatively quickly. Eliminating trigger foods, reducing NSAID use, and addressing infections or inflammatory conditions gives the lining a chance to restore itself.

That said, the five-to-seven-day turnover is for normal maintenance. If the inflammatory cycle has become self-sustaining, with permeability driving inflammation and inflammation driving more permeability, recovery takes longer because you need to break that loop. People who make significant dietary and lifestyle changes typically report improvement in digestive symptoms within a few weeks, with systemic symptoms like fatigue, joint pain, and brain fog improving more gradually over weeks to months as inflammation subsides throughout the body.
