Licensure is a legal requirement issued by a state government that you must have to practice a healthcare profession. Certification is a voluntary credential issued by a private organization that recognizes specialized knowledge or skill. The core difference: without a license, you cannot legally treat patients, regardless of your education or certifications. Without a certification, you can still practice within your licensed scope, but you may miss out on career advancement and higher pay.
What Licensure Means
Licensure is the state’s grant of legal authority to practice a profession within a defined scope of practice. Each state defines, by statute, the specific tasks and functions a licensed professional can perform. Only individuals authorized by the state can legally carry out those functions. This authority comes from the same constitutional power that allows states to regulate lawyers, barbers, and architects, reserved to the states since the ratification of the Tenth Amendment in 1791.
The key word is “legal.” If a healthcare occupation has a scope of practice defined in state law and only state-authorized individuals can perform those activities, those individuals are licensed, regardless of what title the state uses. Practicing without a license carries serious consequences. In Connecticut, for example, practicing medicine without a license is a class D felony, and each instance of patient contact counts as a separate offense.
Licensure exists primarily to protect patients. When the Supreme Court upheld the power of state medical boards in its 1888 Dent v. West Virginia decision, it noted that “comparatively few can judge the knowledge and skill” of practitioners, making state oversight essential. Today, state medical and nursing boards still describe their primary goal as protecting the public.
What Certification Means
Certification is a process by which a nongovernmental organization recognizes an individual who has met that organization’s predetermined qualifications. The federal government and the National Commission for Certifying Agencies both define it as a voluntary process that identifies qualified professionals to the public and employers. The three hallmarks of certification are that it is voluntary, it is granted by a private body (not the government), and it demonstrates a specific level of knowledge or skill.
The critical distinction: certification by a professional organization, by itself, does not give you the right to practice. You can hold every relevant certification in your field, but without a state license, you cannot legally see patients.
How They Work Together in Practice
Nursing offers the clearest illustration. After graduating from an accredited program, a nurse must pass the National Council Licensure Examination (NCLEX) to be licensed by their state board. That license is the baseline requirement to work as a registered nurse. Once licensed, a nurse can then pursue voluntary certifications to demonstrate expertise in a specialty area.
Common specialty certifications include Critical Care RN (CCRN) for adult or pediatric critical care, Family Nurse Practitioner certification through the American Academy of Nurse Practitioners Certification Board, Psychiatric-Mental Health Nursing certification through ANCC, and Emergency NP certification for nurse practitioners specializing in emergency care. Certified registered nurse anesthetists (CRNAs) must recertify every four years to maintain their credential.
Nurse practitioners follow a two-step path: they first earn their advanced degree, then must be licensed by their state board to practice. Specialty certifications come on top of that licensure, not instead of it.
Portability Across State Lines
Because licenses are issued by individual states, they generally don’t transfer automatically when you move. A nursing license from Texas doesn’t automatically let you practice in New York. The Nurse Licensure Compact (NLC) addresses this for nurses: it’s an agreement among more than 40 states and territories to recognize each other’s RN and LPN licenses, similar to how states honor each other’s driver’s licenses. Nurses with a compact license can practice in any member state without applying for a new license.
Certifications, by contrast, are typically national credentials. A CCRN certification is recognized the same way whether you’re working in Florida or Oregon. This makes certifications inherently more portable, though they still don’t replace the need for a valid state license wherever you practice.
Renewal Requirements
Both licenses and certifications require periodic renewal, but the requirements come from different authorities. State licensing boards set their own continuing education (CE) mandates. Illinois, for example, requires advanced practice registered nurses to complete 80 hours of CE for license renewal, including specific training in topics like implicit bias awareness and sexual harassment prevention. First-time renewals are often exempt from CE requirements.
Certification renewal requirements are set by the certifying organization. CRNAs recertify every four years. Other certifications may require a combination of continuing education hours, clinical practice hours, and sometimes a re-examination. The schedules and requirements vary by specialty and certifying body.
Impact on Salary and Career Growth
Licensure is a prerequisite for employment, so it doesn’t come with a salary “bump” in the way certification does. It’s the floor, not the differentiator. Certification, on the other hand, signals specialized expertise and correlates with measurable pay increases.
On average, healthcare workers who hold a professional certification earn weekly wages that are 4.8% higher than those who don’t. The returns vary by role: community health workers with certification earn 13.7% more per week, medical assistants see a 5.6% increase, and personal care aides gain about 3.3%. Men with certification experience the largest increase at 11.3% higher weekly earnings, while women see a 3.8% gain. White workers with certification earn about 6% more than their uncertified peers, while Black and Hispanic workers see smaller gains of 1.2% and 5%, respectively.
Beyond the numbers, certification can create a pathway for upward mobility when employers value the credential and incorporate it into career ladders. An evaluation of seventeen workforce development programs for healthcare workers found that certification, when paired with employer recognition, leads to meaningful wage increases and advancement opportunities.
