Its effects then taper gradually, with a half-life of roughly 12 hours, meaning it takes about that long for half the drug to leave your system. The primary byproduct your body creates while breaking it down lingers a bit longer, with an 18-hour half-life, but that byproduct is inactive and doesn’t produce additional sedation.
Available Strengths and Forms
Standard lorazepam tablets come in three strengths: 0.5 mg, 1 mg, and 2 mg. These are the most commonly prescribed versions, taken by mouth. The concentrated oral solution (Lorazepam Intensol) delivers the same drug in liquid form, which can be easier for people who have difficulty swallowing pills. Injectable lorazepam is reserved for hospital settings, most often for patients experiencing prolonged seizures that need to be stopped quickly.
Common and Serious Side Effects
The most frequent side effects are drowsiness, dizziness, weakness, and unsteadiness. For many people these are mild and fade as the body adjusts, but they can impair driving and other tasks that require alertness.
Less common but more serious reactions include skin rash, hives, swelling of the face or mouth, wheezing, shortness of breath, and yellowing of the skin or eyes. Any of these warrant immediate medical attention. Some people also experience unusual side effects that seem to run counter to the drug’s purpose: anxiety, agitation, aggression, memory problems, difficulty concentrating, or sleep disturbances. Hallucinations, seizures, and changes in mental health have been reported as well, particularly during dose changes or withdrawal.

Combining lorazepam with opioids or other drugs that slow the central nervous system is particularly dangerous. The combination can cause severe breathing problems and, in some cases, death. The same concern applies to alcohol, which amplifies lorazepam’s sedative effects. People who are also taking medication for opioid addiction need especially careful management of both prescriptions.
Generic vs. Brand: Practical Differences
Because the active ingredient is chemically identical, the main difference between generic lorazepam and brand-name Ativan comes down to cost and inactive ingredients. Generic versions are almost always cheaper, sometimes significantly so.
