The most common side effect of Lantus (insulin glargine) is low blood sugar, or hypoglycemia. Other major side effects include injection site reactions, weight gain, low potassium levels, and, rarely, serious allergic reactions. Most people tolerate Lantus well, but understanding these risks helps you recognize problems early and manage them effectively.
Low Blood Sugar (Hypoglycemia)
Hypoglycemia is the single most frequent adverse reaction to Lantus and all insulin products. It happens when there’s more insulin in your system than your body needs relative to the food you’ve eaten and energy you’ve burned. Blood sugar below 70 mg/dL is the standard threshold, and at that level you may start noticing symptoms: tremors or shaking, sweating, cold or clammy skin, dizziness, confusion, anxiety, and a rapid heartbeat.
Mild episodes are easy to manage with fast-acting carbohydrates like juice or glucose tablets. Severe hypoglycemia is a different situation entirely. It means your blood sugar has dropped low enough that you can’t treat it yourself and need help from another person. At its worst, severe hypoglycemia can cause unconsciousness, seizures, and, in rare cases, permanent brain damage or death. The risk increases with tighter blood sugar control, so people aiming for aggressive glucose targets need to be especially vigilant about monitoring.
Certain medications can make hypoglycemia harder to detect. Beta-blockers, for instance, can mask the racing heartbeat and tremors that normally alert you to a drop. Other drugs increase the risk of low blood sugar by amplifying insulin’s effect. These include ACE inhibitors, certain antidepressants like fluoxetine, and some antibiotics. If you take any of these alongside Lantus, more frequent blood sugar checks are a practical safeguard.
Injection Site Reactions
Pain, redness, and irritation at the injection site are common and typically mild. A more significant concern is lipohypertrophy, a buildup of fatty tissue under the skin caused by injecting into the same spot repeatedly. This affects as many as 64% of people who inject insulin at some point. The lumps can range from golf-ball to fist-sized, and they often feel firm, rubbery, or somewhat numb compared to surrounding skin. They tend to appear in the belly or thigh, the most popular injection areas.
Lipohypertrophy isn’t just a cosmetic issue. Injecting into these thickened areas changes how your body absorbs insulin, often leading to unpredictable blood sugar levels. The FDA has also flagged a related condition called localized cutaneous amyloidosis, where abnormal protein deposits form at overused injection sites. Both problems can cause hyperglycemia because insulin absorption becomes erratic. Switching to a fresh injection site can then cause an unexpected drop in blood sugar as the insulin absorbs normally again. Rotating your injection sites consistently is the simplest way to prevent these complications.
Weight Gain
Some weight gain is a recognized side effect of Lantus and insulin therapy in general. Insulin helps your cells absorb glucose, and when blood sugar control improves, your body retains calories it was previously losing through urine. The gain is usually gradual rather than dramatic, but it can be frustrating, especially for people with type 2 diabetes who are already working to manage their weight. Regular physical activity and attention to portion sizes can help offset this effect.
Low Potassium (Hypokalemia)
Insulin drives potassium from the bloodstream into cells, which can lower your blood potassium levels. This is a class-wide effect of all insulin products, not unique to Lantus. Mild drops often go unnoticed, but more significant drops can cause muscle cramps, unusual fatigue, weakness, constipation, and a fast or irregular heartbeat. The risk is higher if you’re already taking medications that lower potassium, such as certain diuretics (water pills). Your doctor will typically monitor potassium through routine blood work.
Allergic Reactions
Local allergic reactions at the injection site, like redness, itching, or a small raised area, occur in a notable percentage of insulin users. These are usually mild and resolve on their own. Systemic (whole-body) allergic reactions are far less common, reported in roughly 1 to 2% of insulin-treated patients. When they do occur, symptoms can include widespread hives, swelling of the face, lips, or throat, shortness of breath, and a drop in blood pressure. In extreme cases, this progresses to full anaphylaxis, which is a medical emergency. Anyone who experiences swelling around the face or difficulty breathing after injecting Lantus should seek immediate medical attention.
Medication Errors and Regimen Changes
One underappreciated risk with Lantus is accidental mix-ups. Insulin products come in similar-looking pens and vials, and injecting the wrong type or dose can cause dangerous blood sugar swings. The FDA specifically recommends checking the insulin label before every injection. This sounds simple, but medication errors remain a documented cause of hypoglycemia in real-world use.
Switching insulin types, changing your dose, or even changing your injection site can also destabilize blood sugar temporarily. Moving from a site with lipohypertrophy to healthy skin, for example, can cause a sudden increase in absorption and an unexpected low. Any change to your insulin routine warrants closer monitoring for a period afterward.
Safety During Pregnancy and Breastfeeding
Published studies have not found a clear link between Lantus and birth defects or developmental problems. The bigger risk during pregnancy is poorly controlled diabetes itself. Women with an HbA1c above 7% face a 6 to 10% risk of major birth defects, and that risk climbs to 20 to 25% when HbA1c exceeds 10%. Lantus is used during pregnancy when the benefit of blood sugar control outweighs potential risks. For breastfeeding, there is limited data on whether insulin glargine passes into breast milk, though the body’s own natural insulin is present in human milk.
Use in Children
Lantus is approved for pediatric use and has been studied in children aged 6 and older with type 1 diabetes. The side effect profile is similar to what adults experience, with hypoglycemia being the primary concern. Children may be less reliable at recognizing or communicating early symptoms of low blood sugar, so caregivers should be familiar with the signs: shakiness, irritability, confusion, pallor, and sweating.
