A honking sound from your dog is almost always one of two things: reverse sneezing or tracheal collapse. Both produce a distinctive noise that owners often compare to a goose honk, but they differ in how serious they are and what you need to do about them. Telling them apart is straightforward once you know what to look and listen for.
Reverse Sneezing: The Most Common Cause
Reverse sneezing is a reflex triggered by irritation in the back of the nose and throat. Instead of pushing air out like a normal sneeze, your dog rapidly pulls air in through the nose in loud, forceful bursts. The soft palate and surrounding muscles spasm, creating a honking or snorting sound that can be alarming the first time you hear it. The episode typically lasts 10 to 30 seconds and then stops on its own.
During a reverse sneeze, your dog will usually stand still, extend their neck, and pull their lips back. Their chest may heave with the effort. The whole point of this reflex is to move mucus from the back of the nasal passages down into the throat where it can be swallowed, clearing the irritation. Common triggers include excitement, dust, pollen, strong smells, pulling on a leash, and sudden temperature changes. Some dogs also develop it from nasal mites or viral infections.
Reverse sneezing is almost always harmless. If your dog has an occasional episode and acts completely normal before and after, there’s generally nothing to treat. Dogs that reverse sneeze frequently, though, may have an underlying irritant worth investigating, like allergies or a foreign body lodged in the nasal passage.
Tracheal Collapse: A More Serious Possibility
Tracheal collapse produces a persistent, harsh, dry cough that sounds strikingly like a goose honking. Unlike reverse sneezing, which comes in brief episodes and resolves, this cough tends to recur throughout the day and gets worse over time. The trachea (windpipe) is held open by C-shaped rings of cartilage. In dogs with tracheal collapse, those rings weaken and flatten, narrowing the airway.
The cough is easily triggered by excitement, physical activity, heat and humidity, inhaled irritants like smoke, or pressure on the neck. You can often provoke it just by gently pressing on the throat. Dogs with tracheal collapse may also gag, retch, or have labored breathing after exertion. The condition tends to be progressive, meaning it gradually worsens over months and years.
How to Tell Them Apart
The key differences come down to timing, posture, and pattern:
- Reverse sneezing is an inward sound. Your dog is inhaling forcefully through the nose, usually standing rigid with their neck extended. Episodes are brief, resolve completely, and your dog acts normal immediately after.
- Tracheal collapse produces an outward cough. It’s dry, repetitive, and often triggered by pulling on a collar, drinking water, or getting excited. It happens repeatedly throughout the day and doesn’t fully resolve between episodes.
If you can, record the sound on your phone. A short video showing your dog during an episode is one of the most useful things you can bring to a vet visit.
Which Dogs Are Most at Risk
Tracheal collapse overwhelmingly affects small and toy breeds. A 2024 study of 110 cases found that Maltese dogs made up nearly 31% of cases, followed by Pomeranians (23%), Poodles (15%), Chihuahuas (7%), and Yorkshire Terriers (5%). Bichon Frises, Shih Tzus, and Schnauzers also appeared. Large-breed dogs can develop it, but it’s rare.
If you own a small-breed dog and the honking cough is recurring, tracheal collapse should be high on your list of possibilities. Middle-aged and older dogs are most commonly affected, though younger dogs can show signs too.
Kennel Cough Can Sound Similar
Kennel cough, an upper respiratory infection, also produces a harsh, honking cough that can sound a lot like tracheal collapse. The difference is that kennel cough comes on suddenly and is often accompanied by other signs of illness: a runny nose or eyes, sneezing, decreased energy, gagging or retching, and occasionally vomiting. It’s highly contagious and typically shows up within days of your dog being around other dogs at a boarding facility, daycare, groomer, or dog park.
Most cases of kennel cough resolve on their own within one to three weeks. But if your dog is very young, very old, or has an existing respiratory condition, the infection can progress to something more serious.
How Tracheal Collapse Is Diagnosed
Standard X-rays of the neck and chest can identify tracheal collapse in roughly 60% of severe cases, but they can miss milder collapse or produce misleading results depending on positioning and timing. The most accurate methods are fluoroscopy (a real-time moving X-ray that captures the trachea during breathing) and direct visualization with a small camera inserted into the airway. These tools aren’t available at every veterinary clinic, so your vet may refer you to a specialist if initial X-rays are inconclusive but tracheal collapse is strongly suspected.
Tracheal collapse is graded on a scale from 1 to 4. Grade 1 means the airway is narrowed by about 25%. Grade 4 means the trachea is essentially flattened shut. The grade matters because it determines treatment.
Treatment for Tracheal Collapse
Medical management works for up to 70% of dogs, especially those with mild to moderate collapse. This typically involves medications to reduce airway spasms and inflammation, weight management, and sometimes mild sedation to control severe coughing and anxiety. Weight loss alone can make a meaningful difference. In one documented case, a dog that lost approximately 25% of its body weight through a structured weight-loss plan showed significant improvement in symptoms within a short period.
When medical management stops working, surgical options exist. One approach places plastic rings or spirals around the outside of the trachea to hold it open. Another uses a stent, a spring-like device placed inside the airway to prop it open from within. Both require a veterinary surgeon with specialized training.
What You Can Do at Home
Whether your dog has been diagnosed with tracheal collapse or you’re managing a dog prone to honking episodes, a few practical changes can reduce how often it happens.
Switch from a neck collar to a harness. When a dog pulls against a collar, the force concentrates directly on the trachea. A chest harness distributes that force across a much larger area, reducing the risk of triggering a coughing episode or worsening an existing tracheal problem. This is especially important for small breeds.
Keep your dog at a healthy weight. Extra body fat puts pressure on the airway and makes every breath harder. If your dog is overweight, even a modest reduction can ease symptoms noticeably. Avoid smoke, strong perfumes, aerosol sprays, and other airborne irritants in your home. On hot, humid days, limit outdoor activity, since heat and humidity are known triggers.
For reverse sneezing episodes, you can sometimes shorten them by gently stroking your dog’s throat or briefly covering their nostrils to encourage a swallow, which resets the soft palate. Stay calm during the episode. Your dog will pick up on your anxiety, which can make things worse.
Signs That Need Urgent Attention
Most honking sounds in dogs aren’t emergencies, but a few signs mean you should get to a vet quickly. Check your dog’s gums and tongue. Healthy gums are pink and moist. Blue or purple gums indicate your dog isn’t getting enough oxygen. Pale or white gums can signal shock or blood loss. Bright red gums may point to heatstroke or systemic inflammation. Any of these color changes, combined with heavy breathing, collapse, or extreme lethargy, is a veterinary emergency.
Persistent coughing that makes it hard for your dog to sleep, eat, or rest comfortably also warrants a prompt vet visit, even if gum color looks normal. A cough that worsens over days rather than improving, or one that starts producing mucus or blood, shouldn’t wait.
