Newborn Waking Up Every Hour: Causes and What Helps

Your newborn is waking every hour because their biology is designed that way. A newborn’s stomach holds only about 20 ml at birth, which empties in roughly one hour when fed breast milk. Their sleep cycles last just 45 to 60 minutes, and they spend far more time in light, easily disrupted sleep than adults do. Hourly waking in the first weeks of life isn’t a sleep problem. It’s a newborn functioning exactly as expected.

That said, understanding why it happens can help you figure out what’s normal, what might be making things worse, and when the pattern will finally shift.

Their Stomach Is Tiny

At birth, a newborn’s stomach holds roughly 20 ml, about four teaspoons. That small volume digests quickly, especially breast milk, which passes through the stomach in about an hour. A published analysis of neonatal stomach physiology found that this 20 ml capacity translates directly to a feeding interval of approximately one hour for a term newborn, matching both gastric emptying time and the normal newborn sleep cycle.

The stomach grows over the first few weeks. By day 10, it holds closer to 60 ml, and by a month it may reach 80 to 150 ml. As capacity increases, your baby can take in more at each feeding and go longer between them. But in those early days, the math simply doesn’t support long stretches of sleep. Your baby wakes because they’re genuinely hungry again.

Newborn Sleep Cycles Are Half the Length of Yours

An adult sleep cycle runs about 90 minutes. A newborn’s cycle lasts 45 to 60 minutes, and a much larger portion of that time is spent in active (REM) sleep. During active sleep, babies twitch, make sounds, flutter their eyelids, and breathe irregularly. They’re also far easier to wake.

At the end of every short cycle, your baby briefly surfaces toward wakefulness. Adults do this too, but we’ve learned to roll over and fall back asleep without fully waking up. Newborns haven’t developed that skill yet. If anything feels off at that transition point (hunger, a wet diaper, a sudden noise, discomfort), they wake completely. When sleep cycles are only 45 minutes long and the baby is spending most of that time in light sleep, the opportunities for full waking come frequently.

They Don’t Have a Body Clock Yet

Newborns don’t produce melatonin, the hormone that tells the body it’s nighttime. The pineal gland is present at birth but isn’t capable of synthesizing melatonin on a day-night schedule until somewhere between 4 and 6 months of age. Research tracking melatonin in infants found that a circadian sleep-wake rhythm typically doesn’t emerge until the second month of life at the earliest, with a stable pattern not appearing in most babies until 13 to 15 weeks.

Until that internal clock develops, your baby has no biological distinction between 2 p.m. and 2 a.m. They sleep and wake in short bursts around the clock, driven by hunger and comfort rather than light and darkness. This is one of the main reasons nighttime feels so relentless. Your baby isn’t ignoring nighttime cues; they literally can’t detect them yet.

The Startle Reflex Wakes Them Up

Newborns are born with the Moro reflex, an involuntary response where they suddenly fling their arms outward and then pull them back in. It fires when a baby’s balance system detects a sensation of falling, which can happen simply from being laid down on their back. The reflex sends an emergency signal to the brainstem, and the resulting jolt often wakes the baby completely.

This is one reason your baby may seem to wake the instant you set them down. It’s not that they prefer your arms (though they do). It’s that the physical act of transitioning from being held to lying flat triggers a reflex they can’t control. The Moro reflex typically fades by 6 months of age. In the meantime, swaddling with arms snug can reduce the startle response, though you should stop swaddling once your baby shows any signs of rolling.

Growth Spurts and Cluster Feeding

Even within the baseline of frequent waking, there are periods when it intensifies. Growth spurts trigger cluster feeding, where your baby wants to eat almost constantly for a stretch of hours or even a day or two. These spurts commonly happen around 2 to 3 weeks, 6 weeks, 3 months, and 6 months, though every baby’s timing varies.

During a growth spurt, a baby who had started spacing feedings out to every two hours may suddenly want to eat every 30 to 45 minutes. This isn’t a sign of low milk supply or that something is wrong. The increased demand signals the body to produce more milk (if breastfeeding) and provides the extra calories the baby needs. Growth spurts typically last two to three days before the pattern settles again.

Reflux and Physical Discomfort

Sometimes hourly waking has a layer of physical discomfort on top of normal biology. Reflux is common in newborns because the muscle at the top of the stomach is still immature. Most babies spit up visibly, but some experience silent reflux, where stomach acid rises into the esophagus without any milk coming up. Signs of silent reflux include coughing or hiccupping during feeds, swallowing or gulping after burping, and crying that doesn’t settle, especially when laid flat.

Gas can also cause frequent waking. If your baby draws their knees up, arches their back, or seems uncomfortable specifically after feeding, trapped gas may be shortening their sleep stretches. Keeping your baby upright for 15 to 20 minutes after a feed and pacing bottle feeds more slowly can help with both reflux and gas.

The Overtired Trap

It sounds counterintuitive, but a newborn who stays awake too long actually has a harder time falling and staying asleep. When a baby is kept up past their window of sleepiness, their body produces stress hormones that create a wired, alert state. You’ll notice it as fussiness, jerky movements, and an inability to settle even though they’re clearly exhausted.

Newborns can typically handle only 45 to 90 minutes of wakefulness before they need to sleep again. If your baby has been up for two hours and is fighting sleep, they’ve likely crossed into overtired territory. Watching for early tired cues (yawning, turning away from stimulation, zoning out) and acting on them quickly can prevent the cycle from compounding overnight.

What Actually Helps Right Now

You can’t change the size of your newborn’s stomach or speed up melatonin production, but you can reduce the things that add unnecessary waking on top of the biological baseline.

  • Room temperature: Keep the room between 68 and 72°F. Overheating is a common cause of restless sleep. Skip blankets, hats indoors, and weighted sleepers or swaddles.
  • Sleep surface: Use a firm, flat mattress in a safety-approved crib or bassinet, in the same room where you sleep. Room-sharing (not bed-sharing) is recommended for at least the first 6 months.
  • Swaddling: A snug swaddle reduces Moro reflex waking. Stop swaddling at the first sign of rolling.
  • Light exposure during the day: Even though your baby can’t produce melatonin yet, daytime light and nighttime darkness help set the foundation for circadian rhythm development over the coming weeks.
  • Boring nighttime feeds: Keep lights dim, voices low, and interaction minimal during overnight feeds. This won’t change the frequency of waking now, but it helps your baby start associating darkness with sleep as their brain matures.

When Hourly Waking Starts to Improve

The shift doesn’t happen all at once, but there are real biological milestones behind it. As stomach capacity grows in the first month, feeding intervals start to stretch. By 6 to 8 weeks, some babies begin offering one longer stretch of 3 to 4 hours, often in the first part of the night. Around 2 to 3 months, the circadian system starts coming online, and you’ll notice more sleep clustering at night.

Then, around 4 months, sleep architecture itself changes. The brain reorganizes from the two-stage newborn pattern into the more complex adult-like stages, cycling through light sleep, deep sleep, and REM. This transition often causes a temporary spike in waking (the well-known 4-month sleep regression), but it’s actually a sign that the sleep system is maturing. On the other side of it, your baby is neurologically capable of longer consolidated stretches for the first time.

Hourly waking in a newborn is exhausting, but it has an expiration date built into your baby’s development. Every week brings a slightly bigger stomach, a slightly more mature nervous system, and one step closer to a body clock that knows the difference between night and day.
