Hands that go numb when you lie down are almost always caused by nerve compression, and the position of your wrists, elbows, or neck during sleep is usually the trigger. The numbness happens because lying down changes how pressure is distributed across the nerves that run from your neck through your arms and into your fingers. In most cases, adjusting your sleep position or wearing a simple wrist splint at night resolves the problem, but persistent or worsening numbness can signal an underlying condition worth investigating.
Carpal Tunnel Syndrome: The Most Common Cause
Carpal tunnel syndrome is the single most frequent reason people wake up with numb hands. The median nerve runs through a narrow passageway in your wrist called the carpal tunnel, and when that space tightens, the nerve gets squeezed. During sleep, most people naturally curl their wrists inward or make a fist, which narrows the tunnel further and cuts off normal nerve signaling. The telltale pattern is numbness in the thumb, index, middle, and ring fingers, but not the pinky.
You might notice the numbness wakes you in the middle of the night, or your fingers feel stiff and tingly first thing in the morning. Shaking your hands out usually brings temporary relief. Over time, untreated compression can lead to weaker grip strength and difficulty with fine motor tasks like buttoning a shirt.
Ulnar Nerve Compression at the Elbow
If the numbness is concentrated in your ring finger and pinky, the problem is more likely at your elbow than your wrist. The ulnar nerve wraps around a bony bump on the inside of your elbow, and when you sleep with your elbow bent, the nerve stretches over that ridge. This stretching irritates the nerve and reduces its blood supply. Many people sleep with their elbows bent tightly, sometimes tucked under a pillow or beneath their head, which keeps the nerve under tension for hours.
Beyond numbness, cubital tunnel syndrome can cause an aching pain on the inside of the elbow and, over time, weaken your grip and coordination. Typing, playing an instrument, or even opening a jar may become harder. The American Academy of Orthopaedic Surgeons recommends keeping your elbow straight during sleep. A simple fix: wrap a towel around your straightened elbow before bed, or wear an elbow pad backwards to prevent you from bending it overnight.
Neck Problems That Radiate to Your Hands
Sometimes the compression isn’t happening in your arm at all. It’s in your neck. A pinched nerve in the cervical spine (cervical radiculopathy) can send numbness, tingling, or pain radiating all the way down your arm and into your hand. This happens most often because of age-related wear. As the discs between your vertebrae lose height, the bones move closer together and form bone spurs that narrow the openings where nerves exit the spine.
A herniated disc can do the same thing. The soft material inside the disc pushes outward and presses on a nearby nerve root. In more than half of cervical radiculopathy cases, the C7 nerve root is affected, with the C6 root involved in roughly a quarter of cases. Lying down can shift spinal alignment just enough to increase or decrease pressure on these nerves, which is why you might feel fine during the day but notice symptoms at night. The numbness pattern depends on which nerve root is compressed, and it often affects the shoulder, arm, and specific fingers on one side.
Arm Position and Thoracic Outlet Syndrome
If you sleep with your arms overhead or tucked under your head, you may be compressing the bundle of nerves and blood vessels that pass through the narrow space between your collarbone and first rib. This is called thoracic outlet syndrome. The neurogenic form, which involves compression of the brachial plexus (the nerve network that controls movement and sensation in your arm and hand), causes pain, tingling, and numbness that typically worsen when you raise your arms.
A vascular form also exists. When the artery supplying blood to your arm gets compressed, reduced circulation can cause numbness, tingling, and skin that feels cool to the touch. When the vein is compressed instead, blood can’t drain properly from the arm, leading to swelling and a heavy feeling. All forms tend to affect one side at a time.
Systemic Conditions That Make It Worse
Certain health conditions can damage nerves throughout your body, making them more vulnerable to compression during sleep. Diabetes is the most common culprit. Sustained high blood sugar and elevated triglycerides gradually damage the small blood vessels that feed your nerves, leading to peripheral neuropathy. Symptoms include burning, tingling, pins-and-needles sensations, and numbness in the hands and feet that are often worse at night.
Vitamin B12 deficiency is another frequent contributor, and it can develop silently over years. People taking metformin for diabetes are at higher risk for low B12 levels. Thyroid disorders, kidney disease, and alcohol use disorder can also cause nerve damage that shows up as nighttime numbness. If your hand numbness is accompanied by similar symptoms in your feet, or if it affects both hands symmetrically, a systemic cause is more likely than a localized compression problem.
Sleep Positions That Reduce Numbness
The way you position your arms, wrists, and neck during sleep has an outsized effect on nerve health. Your head weighs roughly 10 pounds, and resting it on your hand or forearm compresses nerves in both locations at once. A few principles can make a significant difference:
- Side sleepers: Place a pillow in front of you to support the entire arm, keeping your elbow from bending past 90 degrees and your wrist and fingers flat in a neutral position.
- Back sleepers: Keep your arms at your sides or resting on pillows. Avoid folding them across your chest, which bends both elbows and wrists.
- Stomach sleepers: This is the hardest position to make nerve-friendly. It’s difficult to sleep face-down without tucking your elbows underneath you or placing your arms overhead, both of which compress nerves.
Avoid making a fist while you sleep. Curling the fingers jams the tendons and muscles into the carpal tunnel, increasing pressure on the median nerve. Keeping your hand flat on a pillow helps maintain a neutral wrist position.
Do Wrist Splints Actually Help?
Wearing a wrist splint at night is one of the most accessible and well-supported treatments for sleep-related hand numbness caused by carpal tunnel syndrome. The splint holds your wrist in a neutral position, preventing the flexion that compresses the median nerve. In a randomized controlled trial of active workers with carpal tunnel symptoms, those who wore customized night splints for six weeks experienced significant reductions in wrist, hand, and finger discomfort compared to a control group that received only ergonomic education.
The improvements held up at 12 months. Notably, the splinted group improved regardless of how much nerve impairment they had at baseline, while the control group only improved among those whose nerve function was still normal. Night splints didn’t reduce the rate of eventual surgery, but they did provide meaningful symptom relief, especially in the early stages. Over-the-counter splints from a pharmacy work for many people, though a custom-fitted one from a hand therapist provides a better fit.
Signs That Need Medical Attention
Occasional hand numbness that resolves within minutes of changing position is common and usually harmless. But certain patterns point to something that needs evaluation. Numbness that persists after you wake up and move around suggests ongoing nerve compression rather than just positional pressure. Progressive weakness in your grip, visible shrinking of the muscles at the base of your thumb or between your knuckles, and difficulty with coordination tasks like typing or handling small objects all indicate nerve damage that may worsen without treatment.
Numbness that appears suddenly on one side of your body, especially if accompanied by facial drooping, confusion, or difficulty speaking, can signal a stroke and requires emergency care. Numbness affecting both hands and both feet in a “stocking-and-glove” pattern suggests peripheral neuropathy and warrants blood work to check for diabetes, B12 deficiency, or thyroid problems. If nighttime numbness is new and you’re pregnant, carpal tunnel symptoms are common during pregnancy due to fluid retention but should still be discussed with your provider to rule out other causes.
