Numbness in the hands has many possible causes, ranging from a pinched nerve to a chronic disease to a medical emergency. The most common culprit is nerve compression, where pressure on a nerve disrupts the signals traveling between your hand and brain. But circulation problems, autoimmune conditions, and even certain medications can produce the same symptom. Which fingers go numb, whether it happens suddenly or gradually, and what else is going on in your body all point toward different explanations.
Nerve Compression at the Wrist or Elbow
The single most common reason for hand numbness is a compressed nerve, and two specific compression points account for the vast majority of cases.
In carpal tunnel syndrome, the median nerve gets squeezed as it passes through a narrow channel in your wrist. This causes numbness, tingling, or burning in the thumb, index finger, and middle finger. Early on, you might only notice it at night or when holding a phone or steering wheel. As it progresses, you may have trouble gripping small objects or feel like your fingers are swollen even when they’re not. Repetitive hand motions, pregnancy, thyroid problems, and wrist injuries all raise the risk.
Cubital tunnel syndrome involves the ulnar nerve, which runs along the inside of your elbow (the “funny bone” area). This one affects different fingers: the pinky and ring finger. The ulnar nerve gives sensation to the little finger and half of the ring finger, so when it’s compressed, those fingers feel like they’ve “fallen asleep,” especially when your elbow is bent for a long time. Leaning on your elbows, sleeping with bent arms, or holding a phone to your ear can all trigger it. Over time, cubital tunnel syndrome can weaken your grip and make it harder to coordinate fine finger movements like typing or playing an instrument. If the nerve stays compressed long enough, the muscles in your hand can actually waste away.
The key difference between these two conditions is which fingers are affected. Thumb, index, and middle finger numbness points to the median nerve at the wrist. Pinky and ring finger numbness points to the ulnar nerve at the elbow.
Diabetes and Nerve Damage
High blood sugar damages nerves over time, a condition called diabetic neuropathy. Peripheral nerve damage is the most common type, and while it usually starts in the feet, it can affect the hands and arms as well. The numbness tends to develop gradually and symmetrically, meaning both hands are affected rather than just one. Some people with diabetes also experience focal nerve damage, which affects a single nerve and can cause numbness or tingling in one hand specifically.
If you already have a diabetes diagnosis and notice hand numbness creeping in, it typically means blood sugar control needs attention. For people who haven’t been diagnosed, unexplained numbness in the hands and feet is sometimes the symptom that leads to a diabetes diagnosis in the first place.
Raynaud’s Disease
If your fingers go numb and change color in cold temperatures, Raynaud’s disease is a likely explanation. In Raynaud’s, the small blood vessels supplying your fingers narrow dramatically when exposed to cold or stress, cutting off blood flow. Your fingers may turn white, then blue, then red as circulation returns. The numbness follows this pattern closely.
Cold temperatures are the most common trigger. Reaching into a freezer, washing your hands in cold water, or stepping outside on a winter day can set off an episode. Emotional stress can trigger it too. Raynaud’s can occur on its own (primary Raynaud’s) or alongside autoimmune conditions like lupus or scleroderma (secondary Raynaud’s). Primary Raynaud’s is more common and generally less severe.
Multiple Sclerosis
Numbness and tingling are among the most common early symptoms of multiple sclerosis (MS). In MS, the immune system attacks the protective coating around nerve fibers in the brain and spinal cord. When this coating is damaged, electrical signals traveling along those nerves slow down or get blocked entirely. Depending on where the damage occurs, the result can be numbness in the hands, visual changes, weakness, bladder problems, or cognitive difficulties.
The numbness from MS tends to come and go in episodes, especially early in the disease. It can affect one hand or both, and it often appears alongside other neurological symptoms rather than in isolation. MS-related numbness feels different from a compressed nerve because it doesn’t follow the pattern of a single nerve. Instead, it may affect an entire hand or arm in a way that doesn’t match any one nerve’s territory.
Chemotherapy and Other Medications
Roughly 30 to 40 percent of cancer patients develop some form of nerve damage during or after chemotherapy. The hands and feet are the areas most commonly affected. This type of numbness can appear during treatment or emerge weeks to months afterward, and in some cases it persists long after treatment ends.
Chemotherapy isn’t the only medication that can cause hand numbness. Certain antibiotics, anti-seizure drugs, and medications used to treat HIV can all damage peripheral nerves. Heavy alcohol use over years produces a similar effect by directly damaging nerve fibers and depleting the B vitamins nerves need to function.
Vitamin Deficiencies
Your nerves need specific nutrients to maintain their protective coating and transmit signals properly. Deficiencies in B12, B6, and folate can all cause numbness and tingling in the hands and feet. B12 deficiency is particularly common in older adults, vegans, and people with digestive conditions that impair nutrient absorption. The numbness from a B12 deficiency typically develops gradually over months and affects both sides equally. Catching it early matters, because prolonged B12 deficiency can cause nerve damage that becomes permanent.
Sudden Numbness as a Stroke Warning
Sudden numbness in one hand or arm, especially when it’s only on one side of the body, can be a sign of stroke. The CDC identifies “sudden numbness or weakness in the face, arm, or leg, especially on one side of the body” as a primary stroke symptom. The word to focus on is “sudden.” Stroke-related numbness doesn’t build up over weeks. It appears within seconds or minutes.
The F.A.S.T. test helps identify a stroke quickly: does one side of the Face droop? Does one Arm drift downward when both are raised? Is Speech slurred? If any of these are present, it’s Time to call 911. Even if the numbness resolves on its own after a few minutes, that may indicate a transient ischemic attack (sometimes called a mini-stroke), which is a warning sign of a full stroke to come. Noting the exact time symptoms first appeared helps medical teams choose the most effective treatment.
How Doctors Pinpoint the Cause
The pattern of your numbness tells a doctor a lot before any testing begins. Which fingers are numb, whether one or both hands are affected, how quickly it started, and what makes it better or worse all narrow down the possibilities significantly. Numbness in specific fingers suggests nerve compression. Symmetrical numbness in both hands suggests a systemic issue like diabetes or a vitamin deficiency. One-sided numbness with other neurological symptoms raises concern for something in the brain or spinal cord.
When testing is needed, nerve conduction studies and electromyography (EMG) are the standard tools. In an EMG, a small needle electrode is inserted into a muscle to record its electrical activity. Damaged muscles show abnormal electrical patterns both at rest and during contraction. These tests can confirm whether a nerve is compressed, identify where the compression is happening, and measure how severe the damage has become. Blood tests can check for diabetes, vitamin deficiencies, thyroid disorders, and autoimmune markers. MRI scans are used when MS or spinal cord problems are suspected.
Because hand numbness has so many possible causes, the combination of your symptoms, medical history, and targeted testing is what leads to the right diagnosis. Gradual numbness that’s been building for months calls for a very different workup than numbness that appeared overnight.
