Pregabalin and gabapentin are closely related medications that work through the same basic mechanism, but they differ in ways that matter for pain control, side effects, cost, and how reliably the body absorbs them. Both belong to a class sometimes called gabapentinoids, and they share enough structural similarity that they are often treated as interchangeable in casual conversation. In practice, their differences in absorption, potency, and regulatory approval mean they are not simply two versions of the same pill.
How Both Drugs Work
Pregabalin and gabapentin bind to the same target in the nervous system: a specific part of voltage-gated calcium channels known as the alpha-2-delta subunit. When they latch onto that site, calcium flow into nerve terminals drops, which in turn dials down the release of several chemical signals involved in transmitting pain and excitatory nerve impulses.1SpringerLink (Pain Therapy). Calcium Channel α(2)δ Ligands Mirogabalin, Pregabalin, and Gabapentin: Advancements in Diabetic Peripheral Neuropathic Pain Therapeutics This shared mechanism is why they are prescribed for many of the same conditions: nerve pain, epilepsy, and certain anxiety and pain syndromes. The important differences between them are not about what they do once they reach the brain, but about how they get there and how predictably they behave once swallowed.
Absorption and Predictability
The single biggest pharmacological difference between these two drugs is how the gut absorbs them. Gabapentin relies on a transport system in the small intestine that has a ceiling. As you increase the dose, absorption does not keep pace. Its bioavailability (the fraction that actually reaches your bloodstream) drops from about 60% at a daily dose of 900 mg down to roughly 33% at 3,600 mg per day.2PubMed. A comparison of the pharmacokinetics and pharmacodynamics of pregabalin and gabapentin This means doubling the dose does not double the drug’s effect, and blood levels can vary unpredictably from person to person and even from dose to dose in the same person.
Pregabalin, by contrast, is absorbed through a different set of amino acid transporters, and its absorption scales proportionally with the dose.3PubMed. Mucosal uptake of gabapentin (neurontin) vs. pregabalin in the small intestine Bioavailability stays at 90% or above regardless of how much you take, and peak blood levels arrive within about an hour. For clinicians, that makes dosing more straightforward: if they increase the prescription, they can predict how much more drug will be circulating. Gabapentin’s saturable absorption is also why it is typically dosed three times a day, while pregabalin can often be given twice daily.
Neuropathic Pain
Neuropathic pain, the burning or shooting pain caused by nerve damage from diabetes, shingles, spinal injuries, or other conditions, is the territory where both drugs see the most use. A 2024 systematic review and meta-analysis pooling 14 studies and over 3,300 patients found that pregabalin outperformed gabapentin on pain scales through 12 to 14 weeks, improved quality-of-life scores, resulted in more days with no or only mild pain, and led to lower opioid use alongside treatment.4PubMed Central. Pregabalin vs. gabapentin in the treatment of neuropathic pain: a comprehensive systematic review and meta-analysis of effectiveness and safety The same analysis also found that gabapentin was associated with a higher rate of nausea and vomiting compared to pregabalin.
For diabetic peripheral neuropathy specifically, a retrospective study comparing pregabalin, gabapentin, and duloxetine found that after six weeks, patients on gabapentin had significantly higher pain scores than those on pregabalin.5PubMed. Comparative analysis of the therapeutic effects of pregabalin, gabapentin, and duloxetine in diabetic peripheral neuropathy: A retrospective study That said, duloxetine was at least as effective as pregabalin in that study, so the comparison is not just between the two gabapentinoids; a different class of drug may outperform both for certain patients.
In postherpetic neuralgia, the nerve pain that lingers after a shingles outbreak, a separate meta-analysis of randomized trials concluded that pregabalin had a better overall therapeutic effect than gabapentin, though gabapentin had a lower incidence of adverse events.6PubMed Central. A Meta-analysis of Randomized Controlled Trials Comparing the Efficacy and Safety of Pregabalin and Gabapentin in the Treatment of Postherpetic Neuralgia That trade-off, slightly better pain relief versus a slightly cleaner side-effect profile, recurs throughout the gabapentinoid literature.
Epilepsy
Both drugs are approved as add-on therapy for focal (partial) seizures, and this was actually gabapentin’s original indication when it first came to market. A head-to-head randomized trial comparing pregabalin and gabapentin as adjunctive treatments found essentially no difference. The median reduction in 28-day seizure rate was about 59% for pregabalin and 57% for gabapentin, and the 50% responder rates were nearly identical at around 56% and 58%, respectively.7PubMed Central. Adjunctive pregabalin vs gabapentin for focal seizures For seizure control, then, the two drugs appear to be genuinely interchangeable in a way they are not for pain.
Fibromyalgia
Pregabalin is the only gabapentinoid with FDA approval for fibromyalgia, and the evidence base is much deeper for it. A systematic review found seven trials of pregabalin and only one of gabapentin for fibromyalgia. Both reduced mean pain scores compared to placebo at a modest rate: pregabalin by roughly 38% to 50%, and gabapentin by about 51% in the single available trial.8PubMed. Systematic review of the comparative effectiveness of antiepileptic drugs for fibromyalgia Without more head-to-head data, it is impossible to say definitively that one is better than the other for fibromyalgia. In practice, gabapentin is sometimes tried off-label when pregabalin is too expensive or poorly tolerated, but the evidence supporting that swap is thin.
Generalized Anxiety Disorder
Pregabalin has a licensed indication for generalized anxiety disorder (GAD) in several countries outside the United States, and the evidence behind that approval is reasonably strong. A meta-analysis published in Molecular Psychiatry found pregabalin significantly more effective than placebo for GAD, with a moderate effect size that held up under sensitivity analyses excluding patients with psychiatric comorbidities or those taking rescue medications.9PubMed Central. Gabapentin and pregabalin in bipolar disorder, anxiety states, and insomnia: Systematic review, meta-analysis, and rationale Gabapentin has been studied far less for anxiety, and the data that does exist is mostly in the context of social anxiety or perioperative anxiety rather than chronic GAD. If anxiety is a primary treatment goal, pregabalin has a much more established track record.
Restless Legs Syndrome
Restless legs syndrome is another condition where both drugs are used, though the evidence here involves a prodrug form of gabapentin called gabapentin enacarbil, which was designed to overcome gabapentin’s absorption problems. A network meta-analysis of 35 studies found that gabapentin enacarbil, pregabalin, and rotigotine (a dopamine agonist patch) all reduced restless legs symptoms to a similar degree, with no significant differences between the three active treatments.10PubMed. Gabapentin enacarbil, pregabalin and rotigotine are equally effective in restless legs syndrome: a comparative meta-analysis Standard gabapentin is used off-label for this condition too, but the erratic absorption makes dosing less reliable than with the enacarbil formulation or pregabalin.
Side Effects
The most common side effects of both drugs are dizziness, drowsiness, dry mouth, weight gain, and swelling in the hands or feet. In fibromyalgia trials, these side effects led roughly one in four patients to quit treatment at higher doses.11PubMed. Gabapentin and pregabalin in the treatment of fibromyalgia: a systematic review and a meta-analysis The side-effect profiles overlap heavily, but the relative rates shift depending on the condition and dose being studied. In postherpetic neuralgia trials, gabapentin had a statistically lower incidence of adverse events overall.12PubMed Central. A Meta-analysis of Randomized Controlled Trials Comparing the Efficacy and Safety of Pregabalin and Gabapentin in the Treatment of Postherpetic Neuralgia In the broader neuropathic pain meta-analysis, gabapentin was linked to more nausea and vomiting.13PubMed Central. Pregabalin vs. gabapentin in the treatment of neuropathic pain: a comprehensive systematic review and meta-analysis of effectiveness and safety
One practical difference: because pregabalin reaches higher and more predictable blood levels, its central nervous system side effects (dizziness, sedation) tend to be more noticeable when you first start it or when the dose goes up. Gabapentin’s slower, less complete absorption can ironically soften the initial side-effect hit, even though the eventual steady-state side effects are similar. Clinicians sometimes start patients on gabapentin first specifically because the ramp-up feels gentler.
Respiratory Risk When Combined with Opioids
Regulatory agencies have issued warnings about combining either gabapentinoid with opioids because of the risk of respiratory depression. The concern is backed by both experimental and observational data. A review in Expert Opinion on Drug Safety reported that co-exposure to gabapentinoids and opioids was associated with increased odds of respiratory depression or opioid-related death, with odds ratios around 1.3 in perioperative and chronic pain settings, and a hazard ratio of 3.4 in opioid maintenance treatment.14PubMed. Non-opioid antinociceptive drugs: risk of respiratory depression and death related to concomitant use of gabapentinoids in addition to opioids This appears to apply to both pregabalin and gabapentin, with experimental evidence suggesting a single dose of either drug can reverse the respiratory tolerance that chronic opioid users have built up. This is especially relevant for people on methadone or buprenorphine maintenance for opioid use disorder.
Abuse Potential and Dependence
Pregabalin is a Schedule V controlled substance in the United States; gabapentin is not federally scheduled, though several states have added their own scheduling. A systematic review found that the addictive power of both drugs is limited compared to classic substances of abuse, but pregabalin appeared somewhat more prone to misuse. The differences showed up in the magnitude of dependence symptoms, the rate of transitioning from prescription use to self-administration, and how long self-administration persisted.15PubMed. How addictive are gabapentin and pregabalin? A systematic review The population most at risk consists of people with a current or past substance use disorder, particularly those involving opioids or multiple drugs. Among patients without that history, confirmed cases of behavioral dependence were vanishingly rare.
The reason for pregabalin’s higher misuse potential likely traces back to those same pharmacokinetic properties that make it clinically attractive: faster absorption, higher bioavailability, and a quicker onset of the calm, mildly euphoric feeling some users report. Gabapentin’s sluggish, saturable absorption makes it harder to get a rapid subjective “hit” from a large dose.
Withdrawal and Tapering
Both drugs can cause withdrawal symptoms if stopped abruptly after regular use. Symptoms typically begin within 12 hours to 7 days after stopping the medication and include anxiety, restlessness, insomnia, sweating, palpitations, and headache.16PubMed Central. Pregabalin withdrawal in patients without psychiatric disorders taking a regular dose of pregabalin: A case series and literature review Case reports confirm that these withdrawal effects occur even in patients without psychiatric disorders who were taking standard prescribed doses of pregabalin.
Gradual dose reduction is the standard advice for discontinuing either drug. The usual recommendation is to taper over four to six weeks as an outpatient, reducing the dose by about 20% at weekly intervals, or over five to seven days if discontinuation is being managed in a hospital.17PubMed Central. Gabapentinoids: a therapeutic review – Section: Withdrawal from gabapentinoids For gabapentin, documented cases of genuine addiction (beyond physical dependence) have occurred overwhelmingly in people with a history of alcohol, cocaine, or opioid abuse, often at doses well above 3,000 mg per day.18PubMed. Gabapentin: Abuse, Dependence, and Withdrawal
Kidney Function and Dose Adjustments
Neither drug is processed by the liver. Both are eliminated essentially unchanged through the kidneys, which means there are no interactions with the liver enzyme systems that metabolize most other drugs.19PubMed Central. Rational dosing of gabapentin and pregabalin in chronic kidney disease – Section: Pharmacokinetics and renal handling This is a genuine advantage for patients taking multiple other medications, since drug-drug interactions are minimal for both gabapentinoids.
However, the reliance on kidney clearance means that anyone with reduced kidney function will accumulate the drugs more quickly and experience stronger effects, including more side effects, at the same dose. Gabapentin’s total clearance in healthy adults is about 100 mL per minute, roughly matching the normal kidney filtration rate. Pregabalin’s clearance is somewhat lower, in the range of 67 to 81 mL per minute, suggesting some degree of reabsorption by the kidneys.20PubMed Central. Rational dosing of gabapentin and pregabalin in chronic kidney disease – Section: Pharmacokinetics and renal handling Patients on hemodialysis lose about 35% of gabapentin and 50% to 60% of pregabalin per session, so supplemental doses after dialysis are generally needed. Dose reductions based on kidney function are essential for both drugs and should be calculated by a prescriber, not estimated by the patient.
Switching Between the Two Drugs
Clinicians frequently switch patients from gabapentin to pregabalin when gabapentin’s erratic absorption seems to be causing inconsistent pain control, or in the other direction when cost is a barrier. Pharmacokinetic modeling has studied direct overnight switches and brief overlap transitions at three dose pairings: gabapentin 900 mg/day to pregabalin 150 mg/day, gabapentin 1,800 mg/day to pregabalin 300 mg/day, and gabapentin 3,600 mg/day to pregabalin 600 mg/day.21PubMed. Gabapentin to pregabalin therapy transition: a pharmacokinetic simulation These roughly 6:1 dose ratios are commonly used as a starting framework, though individual responses vary. When switching in either direction, a brief cross-taper, reducing one while introducing the other, is often smoother than an abrupt swap.
Cost and Access
Gabapentin went generic in the United States in 2004; pregabalin’s patent expired in 2019. Before pregabalin became generic, the cost difference was enormous. Among injured workers tracked over a multi-year period, the average drug cost per claimant prescribed gabapentin actually fell 22% (from about $612 to $480), while the average cost per pregabalin claim rose 224% (from about $911 to nearly $2,950) even as the proportion of claimants on pregabalin plummeted by roughly 80%.22Journal of Occupational and Environmental Medicine. Gabapentinoid and Opioid Utilization and Cost Trends Among Injured Workers That data reflects the brand-name era, and the gap has narrowed considerably now that generic pregabalin is available. Still, gabapentin remains the cheaper option in most pharmacies and under most insurance formularies, which is a major reason it continues to be prescribed far more often despite pregabalin’s pharmacokinetic advantages.
Pregnancy and Reproductive Safety
The safety data for both drugs in pregnancy is concerning but incomplete. A systematic review found that pregabalin exposure during pregnancy was associated with increased risks of overall congenital anomalies, including anomalies of the nervous system, eyes, and urinary and genital tract, as well as higher rates of miscarriage and stillbirth. Gabapentin exposure was linked to increased risks of preterm birth, preeclampsia, and babies born small for gestational age.23PubMed. Systematic review of gabapentinoid use during pregnancy and its impact on pregnancy and childhood outcomes: A ConcePTION study A European case-control study also found a significantly elevated risk of ventricular septal defects (a type of heart defect) with first-trimester gabapentin or pregabalin exposure, roughly two and a half times the rate seen in unexposed pregnancies.24PubMed Central. Gabapentin and Pregabalin Use in First Trimester of Pregnancy and Risk of Specific Congenital Anomalies: An IMI ConcePTION European Case-Malformed Control Study
The challenge with interpreting these results is that the underlying conditions being treated, particularly epilepsy, also carry independent pregnancy risks. Separating the drug effect from the disease effect is difficult. Neither gabapentin nor pregabalin should be considered safe during pregnancy, and anyone taking either drug who is planning a pregnancy or discovers they are pregnant should discuss alternatives with their prescriber rather than stopping abruptly.
Cognitive Effects in Older Adults
Both drugs cause drowsiness and dizziness, side effects that carry extra weight for older adults who are already at risk of falls and cognitive decline. A large Taiwanese population study tracked people with and without gabapentinoid exposure and found that those who had used gabapentin or pregabalin had about a 45% higher rate of developing dementia after adjusting for conditions like diabetes, hypertension, stroke, and depression. The risk was dose-dependent: people in the highest cumulative exposure group had roughly two and a half times the dementia risk compared to the lowest exposure group, and the association was actually strongest in patients under 50.25PubMed Central. The association between Gabapentin or Pregabalin use and the risk of dementia: an analysis of the National Health Insurance Research Database in Taiwan
This is observational data, not a clinical trial, and it cannot prove that the drugs themselves cause dementia. People prescribed gabapentinoids often have conditions (chronic pain, epilepsy, neuropathy) that independently affect cognition. But the dose-response pattern is worth knowing, especially for older patients or those expecting long-term use. This is an area where the evidence is still accumulating, and neither drug has been cleared of concern.
