Terramycin is a brand name for oxytetracycline, a tetracycline-class antibiotic that fights bacterial infections. In humans, Terramycin has been used both as an oral antibiotic for systemic infections and as an ophthalmic (eye) ointment for bacterial eye infections. However, its availability for human use has changed significantly over the years, and many people now encounter Terramycin primarily as a veterinary product.
How Oxytetracycline Works
Oxytetracycline works by shutting down a bacterium’s ability to build the proteins it needs to survive and multiply. It does this by attaching to a specific part of the bacterial machinery responsible for reading genetic instructions and assembling proteins. Once the drug locks onto that machinery, the bacterium can no longer produce essential proteins, which stops the infection from spreading.
This mechanism gives oxytetracycline broad-spectrum activity, meaning it works against a wide range of bacteria. It can target both major categories of bacteria (Gram-positive and Gram-negative), as well as less common organisms like Rickettsia (the bacteria behind Rocky Mountain spotted fever) and Mycoplasma (a cause of certain pneumonias). That broad coverage made Terramycin a widely prescribed antibiotic for decades after its introduction in the 1950s.
Human Uses of Terramycin
When oral Terramycin capsules were available for human use, doctors prescribed oxytetracycline for a range of bacterial infections: respiratory tract infections, urinary tract infections, skin infections, and tick-borne diseases. It was part of the standard tetracycline toolkit, alongside drugs like tetracycline itself, doxycycline, and minocycline.
Terramycin was also formulated as an ophthalmic ointment combined with polymyxin B, another antibiotic. This combination treated bacterial eye infections, including conjunctivitis and other infections of the outer eye. According to Pfizer’s labeling, the ointment is applied as a small ribbon (about 1 cm) inside the lower eyelid four to six times daily until the infection clears, which can take anywhere from one day to several weeks depending on severity.
Current Availability in the United States
The oral capsule form of Terramycin for humans has been discontinued in the United States. Its national drug codes are listed as inactivated, meaning it is no longer actively marketed or distributed for human use. Doctors who need a tetracycline antibiotic now typically prescribe doxycycline or minocycline instead, both of which remain widely available and have similar coverage.
The Terramycin ophthalmic ointment most people find in stores or online is labeled “For Animal Use Only” and is commonly used for eye infections in cats, dogs, and horses. While the active ingredients are the same as the human formulation, the veterinary product is not FDA-approved for use in people. A human-labeled version of the ophthalmic ointment does exist in Pfizer’s records, but it is not commonly stocked at pharmacies. When taken orally, about 60% of the dose is absorbed from the digestive tract. Food, dairy products, and antacids reduce absorption further, so it’s typically taken on an empty stomach. These absorption limitations are one reason newer tetracyclines like doxycycline (which absorbs more completely) have largely replaced oxytetracycline in countries where both are available.
Side Effects and Safety Concerns
Oxytetracycline carries the same side effects common to all tetracycline antibiotics. The most frequent are nausea, vomiting, diarrhea, and increased sensitivity to sunlight. These are generally mild and resolve after the medication is stopped.
The most well-known risk involves developing teeth. Tetracyclines bind to calcium in forming teeth and bones, creating characteristic yellow-brown staining that becomes permanent once the teeth erupt. This happens when the drug is taken during pregnancy (second half) or given to children under 8 years old, the period when permanent teeth are still mineralizing. The American Academy of Pediatrics notes that these antibiotics can also interfere with tooth enamel development, leading to weakened or pitted enamel beyond just discoloration. The drugs are also incorporated into fetal bones, though this effect appears to be reversible.
Because of these risks, tetracyclines as a class are avoided during pregnancy and in young children whenever an alternative antibiotic is available. One exception: the AAP now supports short courses (under 21 days) of doxycycline at any age when the clinical situation demands it, since shorter exposure appears far less likely to cause dental problems.
