Fasting comes in many forms, from skipping breakfast a few days a week to going without both food and water during religious observance. The most widely practiced types fall into a few categories: time-restricted eating, modified calorie fasts, extended fasts, dry fasts, and fasting-mimicking diets. Each works differently, carries different risks, and suits different goals.
Time-Restricted Eating
Time-restricted eating is the most common entry point into fasting. The idea is simple: you compress all your eating into a set window each day and consume nothing with calories outside of it. The 16:8 method is the most popular version. You eat during an eight-hour window and fast for the remaining 16 hours. In practice, this often looks like skipping breakfast and eating between noon and 8 p.m., though you can shift the window to fit your schedule.
A more aggressive version is OMAD, or “one meal a day,” which shrinks the eating window to roughly one hour. OMAD delivers a longer daily fast but can make it difficult to get adequate nutrition in a single sitting, especially for people with higher calorie needs.
Circadian Rhythm Fasting
This is a variation of time-restricted eating that aligns your eating window with daylight hours rather than letting you choose any window you like. The logic is rooted in how your body’s hormones shift throughout the day. Hormones involved in hunger, fat storage, and blood sugar regulation all fluctuate on a roughly 24-hour cycle. Insulin sensitivity, for example, tends to be highest earlier in the day and drops in the evening. Eating when these hormones are optimally aligned means food is more likely to be efficiently metabolized and less likely to be stored as fat. Research suggests that eating on a schedule misaligned with your circadian rhythm, like consistently eating late at night, increases the risk of obesity and insulin resistance.
The 5:2 Method
Rather than fasting every day, the 5:2 approach has you eat normally five days a week and dramatically reduce calories on the other two. On those two days, you limit yourself to one meal of about 500 to 600 calories. The fasting days don’t need to be consecutive, and most people space them out (say, Monday and Thursday) to make the pattern more sustainable. This method appeals to people who find daily fasting windows too restrictive but still want the metabolic benefits of periodic calorie reduction.
Extended Fasting
Extended fasts last 24 hours or longer, sometimes stretching to 48 or 72 hours. During these fasts, you consume only water, and sometimes electrolytes or black coffee. The primary draw of longer fasts is a deeper shift in how your body fuels itself. After your liver’s stored glucose is depleted (typically within 12 to 24 hours), your body begins breaking down fat for energy and producing ketones. Blood ketone levels in this state generally range from 0.5 to 3 mmol/L, a range known as nutritional ketosis.
Animal studies show that autophagy, the process by which cells break down and recycle damaged components, ramps up after about 24 hours of fasting and peaks around 48 hours. No conclusive human studies have pinpointed the exact timeline, but this cellular cleanup process is one of the main reasons people pursue longer fasts.
The risks scale with duration. Going too long without food can signal starvation to your body and actually encourage increased fat storage afterward. Fasts lasting more than seven days carry a risk of refeeding syndrome, a dangerous shift in electrolytes that can occur when food is reintroduced too quickly after prolonged deprivation. For most people, fasts beyond 72 hours offer diminishing returns and increasing danger.
Fasting-Mimicking Diets
A fasting-mimicking diet lets you eat small amounts of carefully chosen food while still triggering many of the biological responses associated with a full fast. A typical protocol lasts five days. On the first day, you eat roughly 40% to 50% of your normal calorie intake, then drop to just 10% to 20% for the remaining four days. The macronutrient balance matters: approximately 10% protein, 45% fat, and 45% carbohydrates. Keeping protein low is key, because protein intake activates growth-signaling pathways that suppress the cellular repair processes fasting is meant to trigger.
This approach was designed for people who want some of the metabolic benefits of a multi-day fast without actually going without food entirely. It’s typically done once a month or once a quarter rather than on a weekly basis.
Dry Fasting
Dry fasting means abstaining from both food and water. It’s practiced most widely in religious contexts. During Ramadan, Muslims fast from dawn to sunset for approximately 30 days, consuming no food or liquid during daylight hours. Bahá’à fasting follows a similar pattern for 19 days in March, with daily fasting periods of 10 to 13 hours.
Without water intake, the body activates hormonal systems that conserve water by retaining sodium and reducing urine output. Studies on intermittent dry fasting during religious observance have found notable effects on fat and glucose metabolism without critical impacts on hydration or kidney function, at least within the 10- to 16-hour daily windows that these traditions typically involve. Continuous dry fasting for 24 hours or longer is a different matter: serum osmolality (a marker of dehydration) begins to rise, and the risks climb quickly. Extended dry fasting is significantly more dangerous than water fasting of the same duration.
Other Religious and Cultural Fasts
Beyond Ramadan and Bahá’à fasting, many traditions incorporate fasting in distinct ways. Orthodox Christians observe several fasting periods throughout the year, often abstaining from animal products rather than all food. Some Hindu fasts involve eating only fruits or a single meal. Jewish fasts like Yom Kippur require a full 25-hour abstention from food and water. The Daniel Fast, drawn from the Book of Daniel, is essentially a plant-based whole-foods diet maintained for 21 days, closer to a dietary restriction than a true fast.
These practices vary enormously in what’s restricted, how long the fast lasts, and whether water is permitted. The metabolic effects depend heavily on those specifics. A fast that eliminates meat but allows grains and vegetables will have very different physiological consequences than a complete dry fast lasting a full day.
Who Should Avoid Fasting
Fasting is not safe for everyone. People with diabetes face the most immediate risk, since fasting can cause dangerous drops in blood sugar, especially if they’re on medication that lowers glucose. Pregnancy adds another layer of concern. People with cardiovascular disease, chronic high blood pressure, liver or kidney disease, clotting disorders, or peptic ulcers are also at elevated risk during fasting. Children and adolescents who are still growing, and anyone with a history of eating disorders, should generally avoid structured fasting protocols entirely.
How to Choose a Fasting Method
Your starting point matters more than finding the “best” method. If you’ve never fasted intentionally, 16:8 time-restricted eating is the gentlest on-ramp. You’re essentially just formalizing the overnight fast most people already do. If daily restriction feels too rigid, the 5:2 method gives you more flexibility on most days while still providing periodic metabolic benefits.
Extended fasts and fasting-mimicking diets are tools for people who already have experience with shorter fasts and specific goals like deeper autophagy or metabolic resetting. They require more planning, carry more risk, and generally shouldn’t be undertaken casually. Dry fasting, outside of established religious traditions with built-in safeguards like daily refeeding, is the highest-risk category and the one with the least margin for error.
Whatever method you consider, the length and type of fast you can safely do depends heavily on your current health, medications, and nutritional status. The benefits of any fasting protocol come from consistency over time, not from pushing a single fast as long as possible.
