Age spots are flat, brown patches of skin caused by years of sun exposure. Medically called solar lentigines, they’re benign clusters of pigment that form when certain skin cells overproduce melanin in response to accumulated ultraviolet damage. They’re extremely common after age 50, though they can appear earlier in people who spend significant time in the sun or use tanning beds.
What Causes Age Spots
Every time ultraviolet light hits your skin, it damages DNA in the outer layer of skin cells. Those damaged cells release chemical signals that tell nearby pigment-producing cells (melanocytes) to ramp up melanin production. Melanin is your skin’s natural sunscreen: it absorbs UV radiation and shields deeper tissue from further harm. In young skin, this process creates an even tan that fades over time.
With decades of repeated UV exposure, the system stops working so neatly. Melanocytes in certain spots begin to multiply and permanently overproduce melanin, creating a visible dark patch that doesn’t fade when you come back indoors. The damage is cumulative, which is why these spots tend to appear on areas that get the most lifetime sun: the backs of the hands, the face, the forearms, the shoulders, and the chest. The term “age spot” is slightly misleading. Age itself isn’t the cause. It’s the total amount of UV exposure accumulated over a lifetime, and that total simply tends to be higher the older you are.
What Age Spots Look Like
Age spots are flat and smooth, round or oval, and range from light tan to medium brown. They can be as small as a freckle or larger than a centimeter across, and several may cluster together in one area. They don’t itch, hurt, or feel different from the surrounding skin. Their color is usually uniform within a single spot.
Two other common skin markings are easy to confuse with age spots. Seborrheic keratoses are brown but slightly raised, with a waxy, scaly, or crusty texture. Freckles tend to be smaller, lighter, and more responsive to seasonal sun exposure, fading noticeably in winter. Neither of these is dangerous, but knowing the difference helps you monitor your skin more accurately.
When a Spot Needs a Closer Look
The vast majority of age spots are completely harmless. The concern is that melanoma, a dangerous form of skin cancer, can sometimes look like a new or changing dark spot. Dermatologists use the ABCDE rule to flag spots that deserve evaluation:
- Asymmetry: one half doesn’t match the other
- Border: edges are ragged, blurred, or uneven
- Color: multiple shades of brown, black, red, or blue within the same spot
- Diameter: larger than about 6 millimeters (the size of a pencil eraser)
- Evolving: the spot is changing in size, shape, or color over weeks or months
A true age spot stays the same. It doesn’t grow quickly, change color, develop irregular borders, or become raised. If any spot on your skin checks one or more of those ABCDE boxes, it’s worth having a dermatologist examine it in person.
Who Gets Them
Prevalence rises sharply with age. Research surveying skin changes across age groups found strikingly high rates of solar lentigines in people over 50, while the spots were relatively uncommon in younger adults. In younger people, interestingly, the amount of daily sun exposure didn’t strongly predict whether someone had age spots, suggesting that genetic factors and skin sensitivity play a role early on. By middle age and beyond, cumulative UV damage overwhelms those individual differences, and the spots become nearly universal in sun-exposed populations.
Studies comparing different Fitzpatrick skin types (a scale from very fair to very dark) in younger adults found no significant difference in age spot rates between types II, III, and IV. Lighter-skinned people aren’t necessarily more prone to developing the spots themselves, though the contrast between the spot and surrounding skin makes them more visually obvious on fair complexions.
Topical Treatments
If age spots bother you cosmetically, several topical products can lighten them over time. None works overnight. Your skin’s outer layer takes roughly four to six weeks to fully turn over, so any treatment that targets pigment needs multiple cycles to show results.
Hydroquinone is the most studied skin-lightening ingredient. At prescription-strength concentrations (typically 4%), it blocks the enzyme melanocytes use to produce melanin. Clinical trials show visible improvement starting around week four, with continued fading through 12 weeks of use. However, the FDA no longer allows over-the-counter sale of hydroquinone in the U.S. due to reports of serious side effects, including rashes, facial swelling, and a condition called ochronosis that causes permanent bluish-gray discoloration. You can still get it through a prescription, where a provider monitors your use.
Retinoids (vitamin A derivatives) speed up skin cell turnover, pushing pigmented cells to the surface faster and replacing them with new, evenly pigmented skin. Studies show retinoids can reduce dark spots by roughly 64% over three to six months of consistent nightly use. Alpha hydroxy acids like glycolic acid work similarly by exfoliating the pigmented outer layer, with noticeable improvement in two to six months. Vitamin C serums and kojic acid (a compound derived from fungi) are gentler alternatives. Kojic acid combined with vitamin C showed significant pigment reduction by week eight in clinical trials, though it worked more slowly than hydroquinone.
Regardless of which product you choose, expect a timeline of 12 to 24 weeks before you see meaningful results with any over-the-counter option.
Professional Removal Options
For faster or more complete removal, dermatologists offer several in-office procedures. Results vary by the depth of pigment and your skin type, but most people see significant clearing.
Laser therapy uses focused light energy to break apart melanin deposits. Q-switched lasers are among the most common types used for pigmented spots. The treated area typically whitens immediately, then forms a thin crust that peels off within a week or two to reveal lighter skin beneath. Full clearance averages around 140 days (roughly five months), and some people need more than one session. Side effects are generally minimal.
Chemical peels use controlled acid solutions to remove the outer layers of skin. The resolution timeline averages about 68 days for pigmentation issues. Superficial peels have little downtime, while medium-depth peels may leave skin red and peeling for a week. Cryotherapy, which involves briefly freezing individual spots with liquid nitrogen, is a quick in-office option for isolated age spots. The frozen skin blisters, peels, and heals lighter over the following weeks. There is a small risk of permanent light spots where the skin was treated, especially on darker skin tones.
Microneedling, which creates tiny punctures to stimulate skin renewal, typically takes two to four months to show results. Prescription-strength topical treatments prescribed alongside procedures can shorten the overall timeline to six to 12 weeks in some cases.
Prevention
Since age spots are the visible record of cumulative UV damage, the most effective prevention strategy is reducing that damage year after year. Current dermatological guidelines recommend a broad-spectrum sunscreen rated SPF 50 or higher, applied 20 minutes before going outside whenever the UV index is 3 or above (which is most of the day in spring and summer across much of the U.S.).
Standard sunscreens block UVA and UVB rays but don’t protect against visible light, which can also trigger pigmentation, particularly in darker skin tones. Tinted sunscreens containing iron oxide provide that additional layer of visible light protection and are specifically recommended for people prone to pigment changes. Pairing sunscreen with a broad-brimmed hat, sunglasses, and sun-protective clothing makes a meaningful difference, especially during peak UV hours between 10 a.m. and 4 p.m.
If you’re already treating existing age spots, sun protection is doubly important. UV exposure during treatment can darken the very pigment you’re trying to fade, erasing weeks of progress. Consistent daily sunscreen use is what separates people who get lasting results from those who see their spots return within a season.
