CNAs, or Certified Nursing Assistants, are healthcare workers who provide hands-on, everyday care to patients in nursing homes, hospitals, and other medical settings. They help people with basic needs like bathing, eating, and getting around, working under the supervision of licensed nurses. With over 580,000 employed in nursing homes alone, CNAs make up more than a third of the nursing home workforce and serve as the primary point of daily contact between patients and the healthcare system.
What CNAs Do Every Day
The core of a CNA’s job revolves around what healthcare professionals call activities of daily living, or ADLs. These are the fundamental tasks that patients need help with when they’re too sick, injured, or elderly to manage on their own: bathing, dressing, eating, using the bathroom, and moving from a bed to a wheelchair or walker. CNAs also provide oral and denture care, assist with medical equipment, and handle light housekeeping in patient rooms.
Beyond physical care, CNAs monitor patient health by taking vital signs like blood pressure, temperature, and pulse. They observe and report changes in a patient’s condition to the nurses overseeing their care. This monitoring role matters because CNAs spend more direct time with patients than almost anyone else on the care team, which means they’re often the first to notice when something is off.
Emotional support is a significant part of the job too. Patients in nursing homes or hospitals are frequently anxious, confused, or frustrated, and CNAs provide consistent daily comfort and companionship.
What CNAs Cannot Do
CNAs have a clearly defined scope of practice that limits them to basic patient care. They cannot administer medications, perform wound care or dressing changes, start IV lines, or make decisions about a patient’s treatment plan. Those tasks fall to Licensed Practical Nurses (LPNs) and Registered Nurses (RNs), who have more extensive training and licensure. CNAs also do not conduct patient assessments or contribute to formal care planning. Their role is to carry out the care plan that licensed nurses create and to flag any concerns they observe.
Training and Certification Requirements
Becoming a CNA is one of the fastest entry points into healthcare. Federal law, established through the Omnibus Budget Reconciliation Act of 1987, requires a minimum of 100 hours of training for nurse aides working in nursing homes. That breaks down into at least 70 hours of classroom and lab instruction plus 30 hours of supervised clinical training with actual residents in a licensed nursing facility. During those clinical hours, trainees must work under the direct supervision of a registered nurse.
Many states set their own requirements above the federal minimum, so training programs can range from about four to twelve weeks depending on where you live. After completing an approved program, candidates must pass a competency evaluation, which typically includes both a written (or oral) exam and a skills demonstration. Passing places you on your state’s nurse aide registry, which is required to work legally as a CNA.
To keep your certification active, most states require you to work a minimum number of paid nursing hours during each registration period. North Carolina, for example, requires at least eight hours of nursing or nursing-related services supervised by an RN. Private duty employment generally does not count toward renewal. If your certification lapses, you may need to retake the competency exam or complete additional training.
Where CNAs Work
Nursing homes are the single largest employer of CNAs. Nursing assistants make up 36% of the entire nursing home workforce, outnumbering every other job category in those facilities. But nursing homes aren’t the only option. CNAs also work in hospitals, assisted living communities, rehabilitation centers, home health agencies, and hospice programs. The setting you choose affects your daily routine significantly. A CNA in a hospital may work with patients recovering from surgery who need short-term help, while a CNA in a long-term care facility builds relationships with the same residents over months or years.
Pay and Job Outlook
The median annual wage for nursing assistants was $39,530 in May 2024, which works out to roughly $18.96 per hour. Pay varies by setting and location. CNAs working in hospitals or metropolitan areas typically earn more than those in rural nursing homes. Some states with higher costs of living also offer wages well above the national median.
Demand for CNAs remains strong. The aging U.S. population continues to drive the need for long-term care, and nursing homes, hospitals, and home health agencies consistently report difficulty filling these positions. For people looking to enter healthcare quickly, the combination of short training timelines and steady demand makes the CNA role an accessible starting point.
Skills That Matter Most
Physical stamina is non-negotiable. CNAs spend most of their shifts on their feet, lifting and repositioning patients, and moving between rooms. The work is demanding on your body in ways that office jobs simply aren’t.
Patience ranks as one of the most important soft skills. Sick patients can be irritable, and elderly patients may be confused or suspicious of the people trying to help them. Responding with calm and kindness, shift after shift, is harder than it sounds. Flexibility matters too, since care needs can change without warning and schedules often include nights, weekends, and holidays. Strong communication skills help you report patient changes accurately to the nurses supervising your work, and the ability to function as part of a team is essential in every healthcare setting.
Career Advancement From CNA
Many people use the CNA role as a stepping stone. After gaining experience, a common next step is becoming a Licensed Practical Nurse, which expands your scope of practice to include medication administration, wound care, and patient assessment. LPN programs typically take about one year to complete.
Others go further and pursue a Registered Nurse degree through an associate or bachelor’s program. The hands-on patient care experience from working as a CNA gives you a practical foundation that many nursing students lack, and some nursing programs give preference to applicants with CNA experience. The transition from CNA to LPN or RN involves significantly more education and clinical training, but it also comes with higher pay, greater responsibility, and more career options.
