Ethics in healthcare is a set of moral principles that guide how medical professionals treat patients, make difficult decisions, and distribute limited resources. Four core principles form the foundation: autonomy (respecting a patient’s right to make their own decisions), beneficence (acting in the patient’s best interest), non-maleficence (avoiding harm), and justice (treating people fairly). These principles shape everything from a routine doctor’s visit to life-or-death decisions in intensive care.
The Four Core Principles
The ideas of beneficence and non-maleficence trace back to the Hippocratic tradition of “help and do no harm.” A physician has an obligation to protect patients, remove conditions that could cause harm, and help those with disabilities. Non-maleficence sets the boundary: even well-intentioned treatment shouldn’t expose a patient to unnecessary risk or suffering. These two principles often work together, but they can also conflict. A chemotherapy regimen, for instance, causes significant harm in the short term while aiming for long-term benefit.
Autonomy and justice evolved later. Autonomy means patients have the right to make informed choices about their own bodies and care, even choices their doctors disagree with. Justice means healthcare resources and treatment should be distributed fairly and equitably, without favoring one group over another.
Informed Consent and Truth-Telling
Informed consent is one of the most direct expressions of autonomy. For consent to be valid, a patient must have the mental capacity to make medical decisions, must be free from coercion, and must understand the benefits and risks of the proposed treatment. Consent must also be specific to the procedure in question, current (not given months ago for a different situation), and revocable at any time. A patient who agrees to surgery on Monday can change their mind on Tuesday morning, and that decision must be respected.
Truth-telling is closely linked to consent. You can’t make an informed decision if your doctor is withholding key information. There is a concept called “therapeutic privilege,” where a clinician withholds information believing that full disclosure would cause the patient serious psychological harm. Some courts have recognized this as legally permissible, but ethically it remains on shaky ground. Withholding information from a competent patient violates autonomy and, in most cases, does more harm than good by eroding trust.
Confidentiality and Its Limits
Healthcare providers have a professional duty to keep what patients tell them private. This ethical obligation predates any law and exists because patients need to share sensitive information honestly for treatment to work. In the U.S., the HIPAA Privacy Rule sets a legal floor for these protections, preventing unauthorized disclosure of medical information. State laws can add stricter protections on top of HIPAA, but they can’t offer less.
Confidentiality is not absolute, though. Providers can or must break it in several situations: when a patient poses an imminent risk of harm to themselves or others, when child abuse or neglect is suspected, when certain communicable diseases must be reported to public health authorities, or when disclosure is required by court order. HIPAA specifically allows disclosure without consent to avert a serious and imminent threat to health or safety, as long as the provider acts in good faith and consistent with professional standards.
Fair Allocation of Scarce Resources
Justice becomes most urgent when resources run out. During the COVID-19 pandemic, hospitals facing ventilator shortages had to decide which patients received life-sustaining treatment. Many triage frameworks prioritized patients with the best chance of short-term survival, using clinical scoring systems to predict mortality. The goal was straightforward: save the most lives possible.
But that approach raised serious equity concerns. Scoring systems that rely on existing organ function or chronic illness severity can disadvantage patients from marginalized communities who already face higher rates of conditions like heart failure or diabetes due to systemic inequities in healthcare access. Several proposals emerged to address this. One involved applying a health equity adjustment factor to triage scores, accounting for racial disparities in baseline health. Another suggested a reserve system, setting aside a portion of scarce resources specifically for patients from disadvantaged populations. These frameworks reflect the tension at the heart of distributive justice: maximizing overall benefit while ensuring no group bears a disproportionate burden.
End-of-Life Decisions
Some of the most difficult ethical questions arise when treatment stops working. Medical futility describes a situation where continued intervention has a very low probability of achieving any meaningful clinical benefit, or where it causes more suffering than good. Key indicators include failure of two or more organ systems, advanced heart failure, or a disseminated cancer that no longer responds to treatment. When these conditions are present, the central question shifts from “Can we keep treating?” to “Should we?”
The distinction between a clinical effect and a patient benefit is critical here. A ventilator can keep lungs inflating, but if the underlying organ failure is irreversible and the patient will never leave intensive care, that clinical effect doesn’t translate into a benefit for the person. Prolonging ineffective treatment violates the standard of good medical practice. When futile therapy is limited, either by withdrawing existing treatments or withholding new ones, care doesn’t stop. It transitions to palliative care focused on relieving pain and reducing suffering.
This is where the principle of double effect comes in. Medications given to manage pain in a dying patient may, at high doses, hasten death. Ethically, this is considered acceptable when the intent is to relieve suffering, the act itself (giving pain medication) is inherently good, and the benefit of comfort is proportional to the risk of a shortened life. The negative outcome is foreseen but not intended.
Genetic Testing and the Duty to Warn
Genetic testing has created a new kind of ethical conflict. When a patient learns they carry a hereditary mutation linked to cancer or another serious disease, that information is relevant to their blood relatives, who may share the same risk. If the patient chooses not to tell family members, the clinician faces a genuine dilemma: the duty of confidentiality to the patient pulls in one direction, while the potential to prevent serious harm in a relative pulls in the other.
There’s no clean answer. Disclosing genetic results to a relative without the patient’s permission could prevent a preventable cancer. But breaches of confidentiality can also cause real damage: strained family relationships, stigmatization, or discrimination. In one reported case, a mother with a BRCA mutation forbade her daughters from telling an unmarried sibling, fearing it would affect the sister’s marriage prospects. Beyond individual cases, routine breaches of genetic privacy could deter people from seeking genetic testing or counseling at all, undermining the broader public health value of genomic medicine.
AI and Algorithmic Bias
Artificial intelligence is increasingly used in healthcare, from reading imaging scans to predicting which patients need the most intensive follow-up. AI can diagnose certain diseases from scans with higher accuracy and speed than human radiologists. But these tools carry ethical risks that map directly onto the same principles governing all of healthcare.
The most pressing concern is bias. AI systems learn from historical data, and if that data reflects existing disparities, the algorithm reproduces them. A widely cited example found that a commercial algorithm used by hospitals to identify patients needing extra care used healthcare spending as a proxy for illness severity. Because Black patients historically had less spent on their care due to access barriers, the algorithm systematically underestimated their medical needs compared to white patients with similar levels of chronic disease.
Many AI tools also function as “black boxes,” producing recommendations without showing how they reached a conclusion. This lack of transparency makes it nearly impossible to identify and correct biases after deployment. The ethical framework for AI in healthcare mirrors the traditional principles: beneficence and non-maleficence require that AI tools actually help patients without causing harm through errors or bias. Autonomy requires transparency about when and how AI is being used in a patient’s care. Justice demands that AI-driven tools don’t widen existing health disparities. And across all of these, human oversight remains essential. AI should augment clinical judgment, not replace it.
Ethics Committees
When ethical conflicts arise in a hospital, they rarely have obvious solutions. Ethics committees exist to help navigate these situations. A typical committee includes a mix of clinician scientists, basic researchers, a legal expert, a social scientist, a philosopher or ethicist, and at least one layperson from the community. This multidisciplinary makeup is intentional: medical decisions with ethical dimensions shouldn’t be resolved by physicians alone.
These committees serve several functions. They consult on individual cases when patients, families, or clinicians disagree about the right course of action. They review institutional policies to ensure they align with ethical standards. They provide education to staff on emerging ethical issues. And they sometimes mediate disputes, helping all parties understand the values and trade-offs involved before a decision is made.
