Heavy legs are most commonly a symptom of poor blood circulation, particularly when the veins in your legs struggle to push blood back up toward your heart. The sensation often worsens after long periods of standing or sitting and tends to improve when you elevate your legs. While the cause is frequently venous insufficiency, heavy legs can also signal other conditions ranging from medication side effects to nerve problems or arterial disease.

Chronic Venous Insufficiency

The most common medical cause of persistently heavy legs is chronic venous insufficiency, or CVI. Your leg veins contain one-way valves that keep blood flowing upward against gravity. When these valves weaken or fail, blood flows backward and pools in the lower legs, creating sustained high pressure in the veins. That elevated pressure is what produces the heavy, achy, dragging sensation.

CVI can develop from damage to the valves in the superficial veins (close to the skin), the deep veins, or the smaller perforating veins that connect the two systems. The result is the same regardless of location: venous hypertension in the lower extremities. Over time, this pressure damages the tiny blood vessels in the surrounding tissue, which is why CVI often progresses from heaviness to visible skin changes like darkening, swelling, itching, and eventually ulcers if left untreated.

A hallmark of CVI is that your symptoms follow a predictable pattern. Legs feel worst at the end of the day or after prolonged standing, and the heaviness noticeably improves when you put your feet up. Visible varicose veins are a related sign. Any varicose vein, by definition, has incompetent valves and abnormal backward blood flow.

How CVI Gets Diagnosed

If your doctor suspects venous insufficiency, the standard test is a duplex ultrasound. This painless scan combines regular ultrasound imaging with Doppler technology to visualize blood flow direction in real time. During the test, a technician may squeeze your calf or ask you to bear down to see whether blood flows backward through the valves. Reflux lasting longer than half a second is generally considered abnormal, and superficial veins wider than 5 millimeters nearly always show reflux. The scan also checks for blood clots, which can cause or worsen venous insufficiency.

Deep Vein Thrombosis

A blood clot in one of the deep leg veins (DVT) can produce sudden heaviness, throbbing pain, and swelling, usually in just one leg. The key features that distinguish DVT from simple tiredness or CVI are that symptoms typically appear in a single leg, the skin over the affected area feels warm, and the area may look red or darker than usual. The veins near the surface may appear swollen and feel hard or tender to the touch.

DVT requires urgent medical attention because a clot can break free and travel to the lungs, causing a pulmonary embolism. If you have leg pain and swelling alongside sudden breathlessness or chest pain, that combination is a medical emergency. Even without chest symptoms, suspected DVT warrants an urgent evaluation, ideally with an ultrasound scan within 24 hours.

Peripheral Artery Disease

While venous problems involve blood getting stuck on its way back to the heart, peripheral artery disease (PAD) restricts blood flowing down to the legs. Narrowed or hardened arteries can’t deliver enough oxygen to the leg muscles, especially during activity. The classic symptom is cramping or aching in the calves, thighs, or buttocks when you walk, which stops when you rest. But many people with PAD describe the sensation less as sharp pain and more as a diffuse heaviness, fatigue, or weakness in the legs that makes walking feel disproportionately difficult.

PAD is most common in people over 50 who smoke or have diabetes, high blood pressure, or high cholesterol. Unlike venous insufficiency, PAD symptoms are triggered by exertion rather than prolonged standing, and elevating your legs doesn’t help (and can actually make arterial symptoms worse).

Medications That Cause Heavy Legs

Several common medications cause fluid retention in the legs, producing heaviness and swelling as side effects. The worst offenders are calcium channel blockers, a widely prescribed class of blood pressure medications. A large meta-analysis found that roughly 25% of people taking these drugs develop peripheral edema, and rates are nearly three times higher at higher doses. The swelling happens because these drugs relax blood vessel walls, allowing more fluid to leak into the surrounding tissue.

Certain diabetes medications in the thiazolidinedione class cause leg edema in 3% to 5% of people when used alone, but the rate climbs to 13% to 16% when combined with insulin. Other drug categories that can contribute include some antidepressants, hormonal therapies like estrogen, corticosteroids, and certain anti-inflammatory painkillers. If your legs started feeling heavy around the same time you began a new medication, that timing is worth mentioning to your prescriber.

Muscle Fatigue and Overuse

Not every case of heavy legs points to a circulatory problem. Intense exercise, particularly running or heavy leg training, depletes stored energy in the muscles and creates microscopic damage that triggers inflammation. The result is legs that feel leaden, stiff, and sluggish for a day or two afterward. This is normal and resolves with rest.

Chronic overtraining without adequate recovery can make heavy legs a persistent issue for athletes. So can iron deficiency, which reduces the blood’s ability to carry oxygen to working muscles. People who menstruate, endurance athletes, and those on restrictive diets are most susceptible to low iron levels. A simple blood test can rule this in or out.

Nerve-Related Causes

Conditions that affect the nerves in the legs can produce sensations that people describe as heaviness. Peripheral neuropathy, common in diabetes and vitamin B12 deficiency, damages the sensory nerves and can cause numbness, tingling, or a weighted feeling in the lower legs and feet. Lumbar spinal stenosis, where the spinal canal narrows and compresses the nerves supplying the legs, often causes heaviness and weakness that worsens with walking and improves when you sit or lean forward. Multiple sclerosis can also produce leg heaviness as an early symptom due to disrupted nerve signaling.

The distinguishing feature of nerve-related heaviness is that it often comes with other neurological symptoms: numbness, pins and needles, burning, or actual weakness where the leg gives way or drags.

Lifestyle Factors That Make It Worse

Several everyday habits amplify the feeling of heavy legs regardless of the underlying cause. Sitting or standing in one position for hours, whether at a desk or on a long flight, allows blood to pool in the lower legs because the calf muscles aren’t contracting to help pump it back up. Excess body weight increases the pressure on leg veins. Hot weather causes blood vessels to dilate, which increases fluid leakage into surrounding tissues.

Pregnancy commonly causes heavy legs for multiple reasons at once: increased blood volume, hormonal changes that relax vein walls, and the weight of the uterus compressing the veins that drain the legs. The heaviness usually resolves within a few months of delivery, though it can leave behind lasting valve damage in some women.

For mild or occasional heaviness, practical measures can make a noticeable difference. Regular walking activates the calf muscle pump and improves venous return. Elevating your legs above heart level for 15 to 20 minutes helps drain pooled blood. Compression stockings apply graduated pressure that supports the veins from outside. Staying hydrated and reducing salt intake limits fluid retention. These strategies work best when heaviness is related to venous pooling or lifestyle factors rather than arterial disease or nerve damage.
