What Are Liver Spots? Causes, Symptoms and Treatment

Liver spots are flat, tan-to-dark-brown patches of skin caused by years of sun exposure. They have nothing to do with your liver. The name stuck from an old, incorrect belief that liver problems caused the discoloration, but these spots are purely a skin response to ultraviolet light. Dermatologists call them solar lentigines, and they affect more than 90% of fair-skinned adults over age 50.

What Liver Spots Look Like

Liver spots are flat, painless, and range from freckle-sized to about half an inch across. Their color spans from light tan to dark brown, and they tend to cluster on skin that has seen the most sun over your lifetime: the backs of your hands, tops of your feet, face, shoulders, and upper back. They can appear individually or in groups, and they don’t fade on their own once they’ve formed.

The spots have clearly defined edges and a uniform color throughout. They don’t itch, don’t rise above the surrounding skin, and don’t change shape over time. That consistency is one of the key features that separates them from something more concerning.

What Happens Inside the Skin

Your skin contains pigment-producing cells called melanocytes. Under normal conditions, these cells make melanin in response to sunlight and distribute it evenly to surrounding skin cells, which is the process behind a tan. With liver spots, this system becomes locally disrupted.

The current understanding is that the problem starts not with the melanocytes themselves but with the surrounding skin cells (keratinocytes). After decades of UV exposure, these cells malfunction and lose their ability to properly cycle through their normal growth and turnover process. Research published in Karger’s Dermatopathology journal suggests that a specific signaling pathway in these cells breaks down, causing melanin to accumulate in the deepest layer of the skin rather than being shed naturally. The pigment builds up in both the outer skin layers and, eventually, deeper tissue, creating a visible dark patch that persists.

Notably, melanocytes in liver spots are only slightly increased in number. They’re more active than usual, but the real issue is that the surrounding skin can no longer move pigment through its layers and shed it efficiently. The hyperpigmentation you see is a consequence of that breakdown, not the primary cause.

Why UV Exposure Is the Main Cause

Ultraviolet radiation is the dominant trigger. When UV light hits your skin, a protein called p53 acts as a damage sensor and kicks off a chain reaction that tells melanocytes to ramp up melanin production. This is the body’s attempt to shield DNA from further radiation damage. In small doses, the system works as intended and produces a temporary tan.

Over years and decades, repeated UV exposure causes cumulative damage. The skin cells that coordinate pigment distribution become permanently altered. Collagen and elastic fibers in the deeper skin layers degrade (a process called actinic elastosis), and the architecture of the outer skin changes. Some areas develop thickened, club-shaped ridges in the epidermis, while others thin out. Both patterns are found in liver spots, and both reflect long-term structural damage from the sun.

This is why liver spots appear almost exclusively on sun-exposed areas and rarely on skin that stays covered. It also explains why people who use tanning beds develop them earlier.

Who Gets Them and When

Fair-skinned individuals are most susceptible. People with skin types that burn easily and tan minimally or not at all face the highest risk. Among this group, the incidence climbs steeply with age, exceeding 90% after age 50.

That said, liver spots can appear in people of any skin tone who have significant cumulative sun exposure. They tend to show up earlier in people who spent years working outdoors, lived in high-UV climates, or had frequent sunburns in childhood and adolescence. Genetics also play a role in how quickly and severely your skin responds to UV damage.

How Aging Makes Spots Worse

Sun damage is the trigger, but the aging process itself makes liver spots more likely to appear and harder to reverse. As you get older, damaged cells that should have been cleared away by the immune system start accumulating in your skin. These “senescent” cells stop dividing but don’t die. Instead, they release a cocktail of inflammatory and tissue-remodeling signals that disrupt the skin around them.

This secretory activity from aged cells ramps up melanocyte behavior in nearby tissue, driving excess pigment production. It also degrades the structural integrity of the surrounding skin, creating a feedback loop: damaged skin retains more pigment, which attracts more immune cells, which release more inflammatory signals. The result is spots that deepen in color over time and become increasingly resistant to fading.

How to Tell Them Apart From Skin Cancer

Liver spots are benign, but they appear on the same sun-damaged skin where melanoma and other skin cancers develop. The visual overlap can be unsettling. Dermatologists use a straightforward checklist to evaluate suspicious spots:

  • Asymmetry: One half of the spot doesn’t match the other
  • Border: The edges are jagged, blurred, or irregular
  • Color: The spot contains multiple shades or uneven coloring
  • Diameter: It’s larger than a pencil eraser (about 6 millimeters)
  • Evolving: It has changed in size, shape, or color over recent months

A true liver spot is symmetrical, has smooth borders, is a single uniform color, and stays the same over time. If any spot on your body fails one or more of these checks, a dermatologist can examine it and perform a biopsy if needed. Distinguishing benign from malignant skin changes takes clinical training, so don’t rely on self-diagnosis for anything that looks off.

Preventing New Spots

Since UV exposure is the primary cause, sun protection is the most effective prevention strategy. The American Academy of Dermatology recommends broad-spectrum sunscreen with SPF 30 or higher, reapplied every two hours and after swimming or sweating. Tinted sunscreens containing iron oxide offer an additional advantage because they block visible light, which also contributes to hyperpigmentation.

Protective clothing, wide-brimmed hats, and seeking shade during peak sun hours (roughly 10 a.m. to 4 p.m.) reduce UV exposure further. These measures won’t reverse existing spots, but they slow the formation of new ones and help prevent current spots from darkening.

Treatment Options That Work

Liver spots don’t require treatment for medical reasons, but many people want to lighten or remove them for cosmetic purposes. The two main approaches are topical products and in-office procedures.

Topical Treatments

Prescription creams containing hydroquinone (a bleaching agent) and tretinoin (a vitamin A derivative that speeds skin cell turnover) are the standard first-line treatment. In clinical studies, a regimen combining 4% hydroquinone with 0.05% tretinoin cream showed significant reductions in pigmentation starting at week four, with 100% of patients showing measurable improvement by week eight. Full treatment courses typically run 12 to 24 weeks. Over-the-counter options with lower concentrations of these ingredients are available but work more slowly.

These creams can cause dryness, redness, and peeling, especially in the first few weeks. Your skin also becomes more sensitive to sunlight during treatment, making daily sunscreen use essential.

In-Office Procedures

For faster or more complete removal, dermatologists offer two primary procedures. Cryotherapy involves applying liquid nitrogen to each spot for a few seconds, which destroys the pigmented cells. The treated area typically blisters, scabs, and heals over one to two weeks, leaving lighter skin underneath. Laser therapy uses targeted light energy to break apart pigment deposits without damaging surrounding tissue.

Both procedures are typically done in multiple sessions spread over several months. A comparative study found that courses of eight sessions over four months produced meaningful results for both approaches. Cryotherapy carries a small risk of permanent lightening beyond the original spot, while laser treatment tends to be more precise but costs more per session.

Regardless of which treatment you choose, new spots will continue to form if you don’t protect your skin from further UV exposure. Treatment removes existing damage but doesn’t change how your skin responds to sunlight going forward.
