What Are the 4 Stages of Cirrhosis of the Liver?

The four stages of cirrhosis describe how the disease progresses from a silent, manageable condition to one with serious, life-threatening complications. Stages 1 and 2 fall under what doctors call “compensated” cirrhosis, meaning your liver is scarred but still functioning well enough that you have no major symptoms. Stages 3 and 4 are “decompensated” cirrhosis, where the liver can no longer keep up and visible complications develop. The difference in outlook between these two halves is dramatic: median survival with compensated cirrhosis is more than 12 years, while decompensated cirrhosis drops that to roughly 2 years.

How Cirrhosis Is Staged

The four-stage system is based on two key complications: varices (enlarged, fragile veins in the esophagus or stomach caused by rising pressure in the liver’s blood supply) and ascites (fluid buildup in the abdomen). Each stage is defined by whether these complications are present and whether they’ve caused an emergency like bleeding. The stages are progressive, meaning they generally move in one direction, but they aren’t locked in. If the underlying cause of liver damage is addressed, such as stopping alcohol use or treating a hepatitis infection, it’s possible for decompensated cirrhosis to regress back to a compensated state.

Stage 1: No Varices, No Ascites

Stage 1 is the earliest point at which someone has cirrhosis. The liver has significant scarring, but there are no enlarged veins in the digestive tract and no fluid collecting in the abdomen. Most people at this stage feel completely normal and may not know anything is wrong. Cirrhosis is often discovered incidentally through blood work or imaging done for another reason.

Because the liver has remarkable reserve capacity, it continues handling its hundreds of daily functions, including filtering toxins, producing clotting factors, and processing nutrients. The one-year survival rate at this stage, corresponding to a Child-Pugh Class A score, is close to 100%. The priority here is identifying and treating whatever is causing the scarring to prevent progression.

Stage 2: Varices Without Bleeding or Ascites

In Stage 2, rising pressure in the portal vein (the major blood vessel feeding the liver) has caused varices to develop. These swollen veins most commonly appear in the lower esophagus or upper stomach. You still feel well and likely have no symptoms, so this stage is still classified as compensated. Varices are typically found during a routine upper endoscopy, a screening procedure recommended once cirrhosis is diagnosed.

The risk at this stage is that varices can rupture and bleed, which would immediately push the disease into Stage 4. To reduce that risk, doctors often prescribe medication to lower portal vein pressure. Some people at this stage notice mild, nonspecific symptoms like fatigue or easy bruising, but many remain entirely asymptomatic.

Stage 3: Ascites Develops

Stage 3 marks the crossing point into decompensated cirrhosis. The defining feature is ascites, the accumulation of fluid in the abdominal cavity. You may notice your belly swelling, your pants fitting tighter, or unexplained weight gain over days or weeks. Ascites develops because the scarred liver raises pressure in surrounding blood vessels while simultaneously producing less albumin, the protein that helps keep fluid inside your blood vessels.

Along with ascites, other signs of decompensation can appear at this stage. Jaundice, a yellowing of the skin and eyes, indicates the liver is struggling to process bilirubin. Hepatic encephalopathy, which causes confusion, forgetfulness, personality changes, or in severe cases a reduced level of consciousness, signals that toxins normally cleared by the liver are building up and affecting the brain. Not every person at Stage 3 develops all of these complications, but the presence of any one of them means the liver’s functional reserve has been exceeded.

The shift in prognosis is significant. Five-year survival for people with moderate liver damage (Child-Pugh Class B) is roughly 60 to 75%, and it falls to 34 to 50% for those with severe damage (Class C). Fluid management through dietary salt restriction and diuretics becomes a central part of daily life at this point.

Stage 4: Variceal Bleeding

Stage 4 is defined by gastrointestinal bleeding from ruptured varices, with or without ascites. This is a medical emergency. Signs include vomiting blood (which can look bright red or resemble coffee grounds), black tarry stools, lightheadedness, and a rapid heart rate. A single episode of variceal bleeding carries a significant mortality risk, and the likelihood of rebleeding is high without intervention.

At this stage, treatment shifts toward stabilizing the immediate crisis and then managing the underlying portal hypertension to prevent future bleeds. For many people at Stage 4, a liver transplant evaluation becomes a central conversation. The one-year survival rate for Class C cirrhosis, which most Stage 4 patients fall into, is approximately 45%.

How Doctors Measure Liver Damage

Two tools help doctors assess where someone falls on this spectrum. The Child-Pugh score uses five measurements: bilirubin levels, albumin levels, how quickly blood clots, the presence of ascites, and the degree of encephalopathy. Each factor earns one to three points, and the total determines your class. Class A (5 to 6 points) corresponds to a liver that’s still functioning normally despite scarring. Class B (7 to 9 points) indicates moderate damage. Class C (10 to 15 points) means severe damage.

A FibroScan, which uses ultrasound waves to measure liver stiffness, is another common tool. It’s especially useful for detecting cirrhosis before symptoms appear. The stiffness threshold that indicates cirrhosis (fibrosis score F4) varies by the underlying disease. For fatty liver disease, a reading of 14 kPa or higher suggests cirrhosis. For alcohol-related liver disease, the threshold is 19 kPa or higher. For hepatitis C, it’s 14 kPa or higher. These numbers help catch the disease early, often while it’s still in Stage 1 or 2 and most responsive to treatment.

Can Cirrhosis Be Reversed?

The scarring itself becomes increasingly difficult to reverse as it accumulates, but the clinical stage of cirrhosis can improve. This is one of the most important things to understand about the four-stage system: decompensation is not always permanent. If the cause of the liver damage is removed, the liver can recover enough function to move back from a decompensated to a compensated state. The most well-studied example is alcohol-related cirrhosis, where sustained abstinence can lead to “recompensation,” meaning ascites resolves, liver function improves, and the prognosis shifts dramatically.

Similarly, successful treatment of hepatitis B or C can halt and partially reverse liver damage. For people with fatty liver disease, weight loss and metabolic improvements can reduce liver inflammation and slow or stop progression. The earlier in the staging sequence these changes happen, the better the outcome. Stage 1 and 2 offer the widest window for intervention, which is why screening and early detection matter so much even though these stages produce few or no symptoms.
