Garlic offers several well-supported health benefits, from reducing cold frequency to lowering fasting blood sugar. Most of these effects come from sulfur-containing compounds that form when you crush or chop a clove, breaking open its cells and triggering a chemical reaction. The key is knowing which benefits hold up under scrutiny and how to get the most out of every clove.
How Garlic Creates Its Active Compounds
A whole, intact garlic clove is relatively inert. The magic starts when you damage the cells. Crushing, chopping, or mincing garlic releases an enzyme that converts a stored amino acid into a cascade of reactive sulfur compounds, the most well-known being allicin. These compounds are responsible for garlic’s sharp smell and most of its biological effects, including its antioxidant, anti-inflammatory, and antimicrobial properties.
There’s a practical tip worth knowing: let your crushed garlic sit for at least 10 minutes before cooking. Heat deactivates the enzyme that produces these beneficial compounds, so giving it time to work before the garlic hits the pan means more of the good stuff survives. If you toss freshly chopped garlic straight into a hot skillet, you lose a significant portion of its potency.
Fewer Colds and Sick Days
The most striking evidence for garlic involves the common cold. In a 12-week trial of 146 people, those taking a daily garlic capsule reported only 24 colds over the study period, compared to 65 colds in the placebo group. That translated to 111 total sick days for the garlic group versus 366 for placebo. Once someone did catch a cold, recovery time was similar in both groups (about 4.5 to 5.5 days), so garlic appears to help you avoid getting sick rather than speeding up recovery once you are.
A Cochrane review noted that this evidence, while promising, still relies on a limited number of trials. But the size of the difference is hard to ignore, nearly three times fewer colds over three months.
Blood Sugar Benefits for Diabetics
A meta-analysis of clinical trials found that regular garlic intake significantly reduced fasting blood sugar levels. The effect was most pronounced in people with both diabetes and high cholesterol, where the reduction reached statistical significance. For healthy people or those with only one of those conditions, the blood sugar effects were less consistent.
One important caveat: garlic did not significantly lower HbA1c, the marker that reflects average blood sugar control over two to three months. This suggests garlic may help with day-to-day glucose management but isn’t powerful enough on its own to shift long-term blood sugar trends. It’s a useful dietary addition for people already managing their blood sugar through other means, not a replacement for those strategies.
The Cholesterol Question
Early studies generated excitement about garlic’s potential to lower cholesterol. However, a well-designed trial testing three different garlic preparations (fresh garlic, dried powder tablets, and aged garlic extract) found no meaningful effect on cholesterol levels. Harvard Health Publishing’s assessment was blunt: “Save your money,” at least when it comes to using garlic specifically for cholesterol reduction. If you enjoy garlic in your meals, there’s no reason to stop, but don’t count on it to move your lipid numbers.
Antioxidant and Anti-Inflammatory Effects
Garlic’s sulfur compounds act as antioxidants, meaning they help neutralize unstable molecules that damage cells over time. In animal studies, these compounds boosted the body’s own antioxidant defenses, increasing levels of protective enzymes in liver tissue while reducing markers of oxidative damage. They also showed anti-inflammatory effects, which matters because chronic low-grade inflammation underlies many diseases, from heart disease to certain cancers.
These effects have been demonstrated most clearly in lab and animal research. The leap from “protects rat liver cells” to “prevents disease in humans” is a significant one, but the consistency of findings across different garlic compounds and different models of disease is noteworthy. At the very least, garlic contributes to the overall antioxidant capacity of a diet rich in vegetables, herbs, and spices.
How Much Garlic to Eat
Clinical trials and dosage guidelines suggest a range depending on the form. For fresh, raw garlic, 2 to 5 grams daily is the standard recommendation, which works out to roughly one to two average cloves. If you prefer supplements, common doses include 300 to 1,000 milligrams of garlic extract or 2,400 milligrams of aged garlic extract in liquid form. Dried garlic powder falls in the range of 0.4 to 1.2 grams per day.
Raw garlic delivers the broadest spectrum of active compounds, but it can be harsh on the stomach, especially on an empty one. Cooking mellows the flavor and reduces digestive irritation but also diminishes some of the sulfur compounds. Aged garlic extract, which undergoes a long fermentation process, is gentler on the gut and still retains biological activity, just through different compounds than raw garlic.
Who Should Be Careful With Garlic
Garlic has a natural blood-thinning effect. At the amounts you’d use in cooking, this is rarely a problem. But at supplement-level doses, garlic can increase the risk of bleeding in people taking anticoagulant medications like warfarin. UC San Diego Health lists garlic among the supplements that raise bleeding risk due to its antiplatelet activity.
If you’re scheduled for surgery, most guidelines recommend stopping high-dose garlic supplements at least one to two weeks beforehand. People on blood-thinning medications should talk with their pharmacist or doctor before adding garlic supplements to their routine. Garlic used as a food ingredient in normal cooking amounts is generally not a concern, even for these groups.
