The first signs of hip dysplasia in dogs are often subtle changes in movement: a swaying gait in the hind end, slowness getting up from the floor, or a distinctive “bunny hop” when running where both back legs move together instead of alternating. Many dogs show these signs between 6 and 12 months of age, though some don’t display obvious symptoms until much later in life when arthritis has developed in the joint.

What Happens Inside the Joint

A healthy hip is a ball-and-socket joint where the top of the thigh bone (the ball) fits snugly into a cup-shaped socket in the pelvis. In a dog with hip dysplasia, that socket is too shallow or the surrounding tissues are too loose, so the ball doesn’t sit securely. Every time the dog moves, the joint shifts partially out of position. Over time, this repeated shifting damages the cartilage that cushions the joint, wears down the bone underneath, and triggers the formation of bony spurs around the joint. That process is what eventually causes pain and stiffness.

Because the condition starts with loose-fitting joints rather than visible damage, dogs can be dysplastic long before they show any outward signs. This is why early detection matters so much.

Movement Changes to Watch For

The most recognizable early sign is the bunny hop. Instead of extending each hind leg independently while running or climbing stairs, a dog with hip dysplasia will push off with both back legs simultaneously. This keeps the hips from rotating through their full range of motion, which reduces discomfort. It’s easy to miss if you assume it’s just how your dog runs.

Other gait changes include a noticeable sway or wiggle in the hips when walking, and a tendency to shift weight forward onto the front legs. You might notice your dog’s front end looks more muscular than you’d expect while the thighs and hindquarters appear thinner. That imbalance happens because the dog is offloading work from the painful hips to the shoulders, gradually building muscle in the front while losing it in the back. If you stand behind your dog and compare the width of the thigh muscles on each side, or compare the rear legs to the shoulders, asymmetry or thinning can be an early clue.

Behavioral Red Flags

Dogs can’t tell you their hips hurt, so pain shows up as changes in behavior. In young dogs especially, these changes are easy to chalk up to personality or a lazy day. Watch for:

  • Slowness to rise from a sitting or lying position, particularly after napping
  • Tiring easily on walks and needing to sit and rest partway through
  • Reluctance to jump into vehicles, onto furniture, or over obstacles they previously cleared without hesitation
  • Trouble with stairs, whether going up, coming down, or both
  • Losing interest in prolonged play with other dogs or with you

As the condition progresses and arthritis develops, signs become harder to miss. Older dogs with hip dysplasia may limp outright, become sensitive to being touched around the rear end, vocalize (whining or crying) when moving, or show lower tolerance for contact with people and other animals. But by that stage, significant joint damage has usually already occurred. The earlier, quieter signs are the ones worth catching.

When Signs Typically Appear

Most dysplastic dogs first show symptoms between 6 and 12 months of age, during the rapid growth phase. Large and giant breeds are far more commonly affected, though hip dysplasia can occur in dogs of any size. In some dogs, joint laxity is mild enough that obvious symptoms don’t appear until middle age or later, when years of abnormal joint movement have produced enough arthritis to cause clear pain.

This two-phase pattern is important to understand. A puppy that shows early signs is dealing primarily with joint looseness and instability. An older dog showing signs for the first time is likely dealing with the arthritis that built up over years of that instability. The underlying problem is the same, but the treatment options and outlook differ depending on when it’s caught.

How Veterinarians Confirm It

If you’re noticing any of these signs, the next step is X-rays. Two main screening systems exist. The OFA method evaluates how deeply the ball sits within the socket, looking for a tight, well-seated fit. The PennHIP method takes a different approach, measuring how much the ball moves in and out of the socket under compression and distraction. One checks the shape of the fit; the other checks how stable it is. Both require the dog to be sedated or anesthetized for proper positioning.

A common misconception is that a single set of X-rays at a young age gives a final answer. Hip status can change as a dog matures, so results become more reliable as the dog gets older. Your vet will also physically manipulate the joint to check for looseness, grinding, or pain with specific range-of-motion tests.

Why Early Detection Changes the Outcome

Catching hip dysplasia early, particularly in puppies, opens a window for interventions that aren’t available later. One minimally invasive procedure works best when puppies are between 12 and 18 weeks old (up to about 22 weeks in large breeds). It fuses a small growth plate in the pelvis early, which changes the angle of the hip socket as the puppy continues to grow, creating a deeper, more stable fit for the ball of the thigh bone. After 20 to 22 weeks of age, this procedure no longer works because too much growth has already occurred.

For young dogs diagnosed between 6 and 12 months that don’t yet show arthritis on X-rays, a more involved surgery can reshape the pelvis to improve socket coverage. Both options aim to correct the mechanical problem before arthritis takes hold. Once significant arthritis has developed, treatment shifts to managing pain and maintaining mobility through weight management, controlled exercise, anti-inflammatory medications, physical therapy, and in severe cases, total hip replacement.

The practical takeaway: a bunny hop at four months old is not a quirk to monitor. It’s a sign worth investigating immediately, because the most effective interventions have very narrow age windows. If your dog is in a breed predisposed to hip dysplasia, paying close attention to movement during the first year gives you the best chance of catching problems when the most options are still on the table.
