What Are the Five Signs of Metabolic Syndrome?

The five signs of metabolic syndrome are a large waistline, high triglycerides, low HDL cholesterol, high blood pressure, and high fasting blood sugar. You don’t need all five to be diagnosed. Having any three of these five qualifies as metabolic syndrome, a cluster of conditions that dramatically increases your risk of heart disease and type 2 diabetes.

Roughly one in four to one in three adults worldwide meets the criteria, depending on which diagnostic definition is used. None of the five signs cause obvious symptoms on their own, which is why metabolic syndrome often goes undetected until routine bloodwork or a blood pressure reading flags the problem.

1. A Large Waistline

Abdominal obesity is the most visible sign. The threshold is a waist circumference of 40 inches or more for men and 35 inches or more for women. This measurement matters more than overall body weight because fat stored deep around the organs in the abdomen is far more metabolically harmful than fat stored under the skin on the hips or thighs.

Belly fat produces inflammatory signals that interfere with how your body uses insulin, the hormone that moves sugar out of your blood and into your cells. That insulin resistance is the engine behind most of the other signs on this list. Excess abdominal fat also releases more fatty acids into the bloodstream, which disrupts cholesterol and triglyceride levels.

To measure your waist correctly, place a tape measure around your bare abdomen just above the hip bones, level with your navel. Keep the tape snug but not compressing the skin, and read the number after a normal exhale.

2. High Triglycerides

The cutoff is 150 mg/dL or higher on a fasting blood test. Triglycerides are the most common type of fat in your blood. Your body converts excess calories, especially from sugar and refined carbohydrates, into triglycerides and stores them in fat cells for later use. When levels stay elevated, it signals that your body is processing more energy than it needs or struggling to clear fat from the bloodstream efficiently.

High triglycerides rarely cause noticeable symptoms unless levels are extremely elevated. Most people find out through routine lipid panels. If you’re already being treated with medication for high triglycerides, that counts toward the diagnosis even if your current reading is below 150.

3. Low HDL Cholesterol

HDL is often called “good” cholesterol because it carries excess cholesterol away from your arteries and back to the liver for disposal. The diagnostic threshold is below 40 mg/dL for men and below 50 mg/dL for women. When HDL drops too low, cholesterol accumulates in artery walls more readily, accelerating plaque buildup.

Low HDL and high triglycerides tend to travel together. Insulin resistance shifts the body’s lipid processing in a way that raises triglycerides and suppresses HDL simultaneously. Physical inactivity, smoking, and diets high in refined carbohydrates all push HDL lower.

4. High Blood Pressure

A reading of 130/85 mmHg or higher meets the metabolic syndrome threshold. That first number (systolic) measures pressure when the heart beats, and the second (diastolic) measures pressure between beats. Either number being above the cutoff counts.

Insulin resistance contributes to high blood pressure through several pathways. When your body produces extra insulin to compensate for cells that aren’t responding properly, the excess insulin causes your kidneys to retain more sodium, which increases fluid volume in the blood vessels. Over time, blood vessel walls also stiffen. Like the other signs, elevated blood pressure at this level produces no symptoms for most people, so the only way to know is to have it checked. If you’re already on blood pressure medication, you meet this criterion regardless of your current reading.

5. High Fasting Blood Sugar

A fasting glucose level of 100 mg/dL or higher is the fifth sign. This is measured after at least eight hours without eating. At this level you don’t yet have diabetes (which starts at 126 mg/dL), but your body is already having trouble keeping blood sugar in a normal range. This zone between 100 and 125 is commonly called prediabetes.

When cells become resistant to insulin, the pancreas responds by pumping out more of it. For a while, that extra insulin keeps blood sugar in check. But as resistance worsens, even elevated insulin production can’t keep up, and fasting glucose starts to creep above 100. This is often the last sign to appear, meaning the metabolic disruption has been building for years before blood sugar becomes visibly abnormal.

How Insulin Resistance Ties All Five Together

These five signs might look unrelated on the surface: a tape measure around the waist, a cholesterol panel, a blood pressure cuff, and a glucose test. But insulin resistance is the common thread linking them. Excess calorie intake, physical inactivity, genetics, and aging all promote insulin resistance, and it cascades outward into each of these measurements.

Obesity is the leading driver. As fat tissue expands, particularly around the abdomen, it triggers low-grade chronic inflammation throughout the body. That inflammation further impairs insulin signaling, creating a cycle where resistance worsens, fat accumulates more easily, and the five markers drift further out of range.

Why the Diagnosis Matters

Having metabolic syndrome roughly triples a man’s risk of cardiovascular disease and roughly doubles it for women, compared to people without the syndrome. The impact on type 2 diabetes risk is even more dramatic. Both men and women with metabolic syndrome are about seven times more likely to develop type 2 diabetes.

These risks compound over time. Each individual sign carries some risk on its own, but the combination of three or more creates a synergistic effect where the total danger exceeds what you’d expect from adding each factor individually. That’s the reason clinicians group them into a single diagnosis rather than treating each measurement in isolation.

Reversing Metabolic Syndrome

The most effective intervention is lifestyle change, and the results can be surprisingly fast. Weight loss is the single strongest lever. Losing even a moderate amount of body weight improves nearly every component of the syndrome simultaneously because it directly reduces abdominal fat and improves insulin sensitivity. Calorie restriction that produces meaningful weight loss appears to have the most significant impact across the majority of metabolic markers.

Exercise helps independently of weight loss. Studies using structured programs of about 30 to 60 minutes of moderate-intensity activity three times per week show measurable improvements in blood pressure, triglycerides, HDL cholesterol, and fasting glucose. Importantly, participants who exercised without losing weight still saw benefits, suggesting that physical activity improves insulin sensitivity through pathways beyond just shrinking fat stores.

Dietary changes beyond calorie reduction also matter. Replacing refined carbohydrates and added sugars with whole grains, vegetables, and healthy fats lowers triglycerides and raises HDL. Reducing sodium intake helps bring blood pressure down. These aren’t separate strategies so much as different angles on the same underlying problem: giving your body less excess fuel to store and more reasons to use insulin efficiently.

Because you need three of five signs to qualify for the diagnosis, improving even one or two markers below their thresholds can technically resolve the syndrome. That’s a more achievable target than it might seem, since the lifestyle changes that work tend to move multiple markers in the right direction at once.
