Olive oil, particularly extra virgin olive oil, is one of the most well-studied foods in nutrition research. People who consume more than half a tablespoon per day have a 19% lower risk of dying from cardiovascular disease compared to those who rarely or never use it. Its benefits extend beyond heart health into blood pressure regulation, inflammation reduction, and potentially brain protection.
Heart Disease and Mortality Risk
A large study published in the Journal of the American College of Cardiology tracked olive oil intake across tens of thousands of U.S. adults and found that those consuming more than half a tablespoon daily had a 19% lower risk of cardiovascular death. Their overall risk of dying from any cause dropped by roughly the same margin. These results held after adjusting for other dietary and lifestyle factors, meaning the benefit wasn’t simply explained by people who use olive oil also eating more vegetables or exercising more.
The heart protection comes from several biological pathways working together. Olive oil’s plant compounds, especially one called hydroxytyrosol, stimulate the release of nitric oxide in blood vessels. Nitric oxide relaxes artery walls, improving blood flow and lowering blood pressure. These same compounds also act as potent antioxidants that prevent LDL cholesterol (the “bad” kind) from oxidizing. Oxidized LDL is what actually drives plaque buildup in arteries, so keeping it in check is a meaningful layer of protection.
Blood Pressure Reduction
Clinical trials within the large PREDIMED study found that people following a Mediterranean diet supplemented with extra virgin olive oil saw their 24-hour systolic blood pressure (the top number) drop by about 2.3 mmHg after one year, while a control group actually saw a slight increase. The gap between the olive oil group and the control group was even more striking: roughly 4 mmHg lower systolic and nearly 2 mmHg lower diastolic blood pressure.
At the three-month mark, the differences were even larger, with systolic pressure dropping 5.9 to 7.1 mmHg compared to the control group. Those numbers may sound modest, but at a population level, even a 2 to 4 mmHg reduction in systolic pressure meaningfully lowers the risk of stroke and heart attack over time.
Anti-Inflammatory Properties
If you’ve ever tasted a peppery, throat-catching extra virgin olive oil, you’ve felt oleocanthal at work. That compound inhibits the same enzymes targeted by ibuprofen, which is why high-quality olive oil can produce a similar sensation to swallowing a mild pain reliever. In lab studies, oleocanthal has shown genuine anti-inflammatory activity through this shared mechanism.
This doesn’t mean olive oil replaces pain medication. Oleocanthal is only found in small amounts in very high-quality, fresh, cold-pressed extra virgin olive oil that has been properly harvested and stored. But as part of a daily diet, it contributes to a lower baseline of chronic inflammation, the kind of slow-burning immune activation linked to heart disease, cancer, and metabolic problems.
Blood Sugar and Metabolic Effects
The evidence on olive oil and blood sugar is more nuanced than headlines sometimes suggest. A systematic review and meta-analysis found that extra virgin olive oil intake did not significantly change fasting blood sugar, insulin levels, or insulin resistance scores across pooled clinical trials. Subgroup analyses based on age, health status, dose, and duration didn’t change those results.
However, there’s an important caveat. When people replaced saturated fat sources (butter, cream, fatty meats) with extra virgin olive oil, their long-term blood sugar marker (glycated hemoglobin) dropped by about 0.3 to 2.0%, a reduction comparable to what some diabetes medications achieve. So the benefit may be less about olive oil adding something magical and more about what it replaces. Swapping butter for olive oil in cooking shifts the fat profile of your diet in a direction that favors metabolic health.
Brain Health and Cognitive Protection
Research from Yale School of Public Health identified 67 bioactive chemicals in extra virgin olive oil that could potentially play a role in preventing or slowing Alzheimer’s disease. Using machine learning, researchers simulated how these compounds might disrupt the protein networks involved in Alzheimer’s progression and found that 10 of them appeared to act similarly to approved pharmaceutical treatments.
Among those 10 compounds, quercetin showed the highest likelihood of clinical activity against Alzheimer’s. Earlier studies have demonstrated that quercetin reduces oxidative stress and inhibits the buildup of amyloid beta, the proteins that form the characteristic plaques in Alzheimer’s patients’ brains. This research is still in the modeling and simulation stage, not yet confirmed in human clinical trials, but it adds biological plausibility to population studies that have linked higher olive oil consumption to lower rates of Alzheimer’s death.
Extra Virgin vs. Regular Olive Oil
Not all olive oil delivers the same benefits. Extra virgin olive oil is pressed mechanically without heat or chemical solvents, which preserves its full complement of polyphenols and antioxidants. Regular (refined) olive oil undergoes processing with high heat and chemical treatments that strip away most of these protective compounds. The result is a milder-tasting oil with a longer shelf life but significantly less nutritional value.
The polyphenol content in extra virgin olive oil is high, while refined olive oil is low. The antioxidant activity follows the same pattern. Since nearly every health benefit discussed in this article traces back to those polyphenols, choosing extra virgin over refined is the single most important decision you can make when buying olive oil. The peppery bite of a good extra virgin is literally the taste of oleocanthal and related compounds doing their work.
Cooking With Olive Oil
A persistent myth holds that olive oil is unsuitable for cooking because it breaks down at high temperatures. In reality, olive oil smoke points range from 347°F (175°C) to 464°F (240°C) depending on the grade, quality, and freshness. That range is comparable to canola, rice bran, and high-oleic peanut oil. According to researchers at the University of California, Davis, good quality extra virgin olive oil has excellent cooking properties because its natural antioxidants resist heat and prevent the formation of harmful compounds.
For everyday cooking like sautéing vegetables, pan-frying eggs, or roasting at moderate oven temperatures, extra virgin olive oil works well and retains a meaningful portion of its beneficial compounds. Deep frying at very high temperatures for extended periods is the one scenario where a higher smoke point oil might be preferable.
How Much You Need
The American Heart Association highlights research showing that consuming more than half a tablespoon of olive oil daily is the threshold associated with lower heart disease risk and reduced rates of premature death from cardiovascular disease, Alzheimer’s, and other causes. That’s a low bar, roughly one generous drizzle over a salad or used in cooking a single meal.
Most Mediterranean diet studies use considerably more, often two to four tablespoons per day. Olive oil is calorie-dense at about 120 calories per tablespoon, so the practical approach is to use it as a replacement for other fats rather than adding it on top of your current diet. Swap it in for butter when cooking, use it as a salad dressing base instead of cream-based options, or dip bread in it rather than spreading on margarine.
Storing Olive Oil to Preserve Its Benefits
How you store olive oil dramatically affects how long its beneficial compounds last. Research published in the Journal of Agricultural and Food Chemistry tracked polyphenol degradation over 18 months and found that temperature is the dominant factor. At refrigerator temperature (5°C/41°F) and cool room temperature (15°C/59°F), the breakdown of key antioxidants was similar and relatively slow. But at typical room temperature (25°C/77°F), degradation rates jumped considerably, and at warm temperatures (50°C/122°F), they accelerated by five to seven times.
After three months in open bottles at room temperature, only about 77% of the key protective compounds remained. At warm temperatures, that figure plummeted to 28%. Sealed bottles fared much better than open ones because oxygen exposure roughly tripled the oxidation rate. One variety’s shelf life extended from 26 weeks at room temperature to 44 weeks when stored at 15°C, and at refrigerator temperature, none of the oils studied exceeded quality thresholds even after 72 weeks.
The practical takeaway: store your olive oil in a sealed, dark bottle in a cool spot, ideally a pantry away from the stove. If your kitchen runs warm, the refrigerator is a better option. The oil may turn cloudy and thicken when cold, but it returns to normal at room temperature with no loss of quality. Buy in quantities you’ll use within a few months rather than stockpiling large bottles that sit open for a year.
