What Are the Long-Term Side Effects of Trazodone?

Trazodone is generally well tolerated for short-term use, but staying on it for months or years raises several concerns worth knowing about. The most clinically significant long-term risks involve heart rhythm changes, potential effects on memory, and for men, a rare but serious condition called priapism. Weight changes are relatively modest, and many common side effects like daytime drowsiness tend to stabilize over time, though they don’t always disappear.

Heart Rhythm Changes

Trazodone can lengthen something called the QT interval, a measure of how long your heart takes to reset between beats. When that interval stretches too far, the heart becomes vulnerable to dangerous irregular rhythms. Clinical studies have linked trazodone to QT prolongation even at standard prescribed doses, and in rare cases, patients have developed a potentially fatal type of rapid heartbeat called torsades de pointes.

This risk increases if you’re taking other medications that also affect heart rhythm, if you have low potassium or magnesium levels, or if you already have an underlying heart condition. For people on long-term trazodone, periodic heart monitoring may be appropriate, especially if other risk factors are present.

Effects on Memory and Cognition

One of the more subtle long-term concerns is trazodone’s impact on thinking and memory. Randomized controlled trials testing trazodone as a sleep aid in people with mild to moderate Alzheimer’s disease found no cognitive benefit. More concerning, participants in those trials experienced potentially harmful effects on short-term memory.

For people without dementia, the picture is less clear, but the sedating properties of trazodone can contribute to daytime mental fog, slower reaction times, and difficulty concentrating, particularly at higher doses. These effects can be easy to overlook because they build gradually and may be mistaken for normal aging or poor sleep quality.

How It Changes Your Sleep

Trazodone is one of the most commonly prescribed off-label sleep aids, and it does meaningfully change sleep structure. A meta-analysis of sleep studies found that trazodone increased total sleep time by roughly 40 minutes compared to controls. It also significantly boosted the amount of deep sleep (the restorative stage known as N3), which is the phase most responsible for feeling physically rested.

Interestingly, trazodone does not appear to suppress REM sleep, the dream-heavy stage important for emotional processing and memory consolidation. That’s a meaningful advantage over some other sedating medications. However, the long-term question is whether your brain becomes dependent on the drug to reach deep sleep stages, making it harder to sleep naturally if you stop taking it.

Weight and Metabolic Effects

Compared to many other antidepressants, trazodone’s effect on weight is relatively balanced. In clinical trials, about 5% of people gained weight while 6% actually lost weight. That’s a far cry from the significant weight gain associated with some other commonly prescribed antidepressants. Still, individual responses vary, and people on long-term trazodone should be aware that gradual changes in appetite or metabolism are possible, even if they’re not dramatic on average.

Priapism in Men

Trazodone carries a unique risk among antidepressants: priapism, a prolonged and painful erection unrelated to sexual arousal. This is rare but constitutes a medical emergency. If not treated within a few hours, priapism can cause permanent damage to erectile tissue. The risk exists at any dose and can occur early in treatment or after months of use. Men taking trazodone should know that any erection lasting four hours or more requires immediate medical attention.

Liver Health

Trazodone-related liver injury is rare but documented. Case reports in the medical literature describe both acute and chronic forms of liver damage. In one notable case, a patient developed reversible liver injury after 18 months of use. The injury was confirmed when liver enzymes spiked again after the patient was inadvertently restarted on the drug. Only a handful of cases have been formally reported, so this appears to be an uncommon reaction, likely driven by individual susceptibility rather than a predictable dose-related effect. For people on long-term trazodone, routine blood work that includes liver enzymes can catch problems early.

Fall Risk in Older Adults

Because trazodone causes sedation and can lower blood pressure when standing up (orthostatic hypotension), there’s a logical concern about falls, especially in older adults. A large retrospective study published in the Canadian Medical Association Journal compared trazodone to atypical antipsychotics in older adults with dementia. The fall and fracture rates were statistically similar between the two groups, with trazodone showing no increased risk of hip fractures or other major breaks compared to those antipsychotics.

That said, “similar to antipsychotics” is not the same as “safe.” Both drug classes carry meaningful fall risk in elderly populations. The sedation and dizziness from trazodone are most pronounced in the first few hours after taking it, which is why it’s typically dosed at bedtime. Nighttime trips to the bathroom remain a real hazard for older adults on this medication.

What Happens When You Stop

Trazodone has a short half-life, meaning it leaves your body relatively quickly. If you stop abruptly after long-term use, withdrawal symptoms can begin within 24 to 72 hours. Common experiences include dizziness, vertigo, and balance problems. Many people report heightened emotional sensitivity or mood swings that feel out of proportion to what’s happening around them.

Cognitive symptoms are also typical during withdrawal: mental cloudiness, trouble concentrating, and short-term memory lapses. Some people experience neurological symptoms like tingling, numbness, or the electric shock sensations often called “brain zaps,” a phenomenon familiar to anyone who has discontinued other serotonin-affecting medications. These symptoms are uncomfortable but temporary, and tapering the dose gradually rather than stopping all at once significantly reduces their severity.

Daytime Sedation and Functioning

The most common complaint with long-term trazodone use is next-day grogginess. While many people find this diminishes over the first few weeks, it doesn’t resolve for everyone. At higher doses used for depression (rather than the lower doses typically prescribed for sleep), daytime sedation can meaningfully impair your ability to drive, work, or stay alert during conversations. This hangover effect is dose-dependent, so people who find it disruptive often benefit from adjusting their timing or dose rather than pushing through it indefinitely.
