The most common side effects of ciprofloxacin are nausea (affecting about 2.5% of users), diarrhea (1.6%), abnormal liver function tests (1.3%), vomiting (1%), and skin rash (1%). Most people tolerate ciprofloxacin without serious problems, but this antibiotic also carries several important safety warnings that set it apart from other antibiotics.
Digestive Side Effects
Stomach and gut problems are the reactions you’re most likely to notice. Nausea is the single most common complaint, showing up in roughly 1 in 40 people who take ciprofloxacin across clinical trials. Diarrhea follows, affecting about 1 in 60. Vomiting and abdominal pain or discomfort also occur, though each affects fewer than 1 in 100 users.
A more serious form of diarrhea can develop if ciprofloxacin disrupts the normal bacteria in your gut, allowing a harmful species called C. difficile to take over. This particular complication can appear up to two months after you’ve finished the antibiotic, so persistent or worsening diarrhea after a course of ciprofloxacin is worth reporting to your doctor even if you stopped taking it weeks ago.
Skin Rash and Sun Sensitivity
Rash occurs in about 1% of users. Beyond a simple rash, ciprofloxacin can make your skin significantly more sensitive to sunlight. This is a phototoxic reaction: the drug absorbs UV light and generates reactive molecules that damage skin cells, producing what looks and feels like an exaggerated sunburn. Unlike a true allergy, this can happen to anyone if they get enough sun exposure while the drug is in their system. Wearing sunscreen, covering exposed skin, and avoiding prolonged sun exposure while taking ciprofloxacin reduces the risk considerably.
Tendon Damage
Ciprofloxacin carries an FDA boxed warning for tendinitis and tendon rupture. The Achilles tendon is the most frequently affected, but damage has also been reported in the shoulder (rotator cuff), hand, biceps, and thumb tendons. The timing is unpredictable: tendon problems can appear within hours of the first dose, or surface several months after finishing the entire course. In some cases, both sides of the body are affected simultaneously.
Certain groups face higher risk. People over 60, those taking corticosteroids, and organ transplant recipients (kidney, heart, or lung) are all more vulnerable. Strenuous physical activity, kidney problems, and a history of tendon disorders such as rheumatoid arthritis also raise the odds. If you feel sudden pain, swelling, or tenderness in a tendon while taking ciprofloxacin, stopping the medication and resting the affected area is the standard advice.
Nerve Damage
The second boxed warning covers peripheral neuropathy, which means damage to the nerves outside your brain and spinal cord. Symptoms include tingling, numbness, burning, pain, or weakness, typically in the hands or feet. These sensations can start soon after the first dose. The concern here is that the damage may not reverse after stopping the drug. Some patients experience lasting changes in how they sense touch, temperature, or vibration.
Central Nervous System Effects
Ciprofloxacin can affect the brain, sometimes after just a single dose. The range of possible reactions includes dizziness, headache, insomnia, restlessness, and anxiety. In rarer cases, more severe reactions like confusion, tremors, hallucinations, or seizures have been reported. People with a history of seizures or conditions that lower the seizure threshold are at greater risk for these effects.
Fluoroquinolone-Associated Disability
The FDA recognizes a condition called Fluoroquinolone-Associated Disability, or FQAD, which describes a pattern of serious, lasting side effects that persist for 30 days or longer after stopping the drug. To meet the definition, a person must experience side effects in at least two different body systems, such as musculoskeletal problems combined with nerve symptoms, or neuropsychiatric issues paired with changes in vision or hearing. The effects must substantially disrupt normal daily life. FQAD is uncommon, but its recognition by the FDA reflects the fact that ciprofloxacin’s serious side effects can overlap and persist in ways that other antibiotics typically do not cause.
How Timing Affects Your Risk
Side effects from ciprofloxacin don’t follow a single predictable schedule. Digestive symptoms like nausea and diarrhea tend to show up early, often within the first few days. Nervous system effects, including dizziness and insomnia, can appear after the very first dose. Tendon problems are the most variable: they can develop within hours of starting, during a full course, or months after the last pill. Liver injury, when it occurs, typically begins within the first one to five weeks.
Food and Supplement Interactions
Ciprofloxacin binds to minerals like calcium, magnesium, zinc, and iron, which can prevent it from being absorbed properly. If you take a multivitamin, antacid, or mineral supplement, you’ll need to space it apart from your ciprofloxacin dose. The general guideline is to take ciprofloxacin at least two to four hours before, or four to six hours after, any product containing these minerals. Dairy products and calcium-fortified foods follow a similar rule: leave at least two hours on either side of your dose.
Age-Related Considerations
In children and adolescents, ciprofloxacin has historically been used cautiously because studies in young animals showed damage to weight-bearing joints. Real-world experience in pediatric patients has been more reassuring, with no pronounced increase in joint disease, but a small risk of joint pain can’t be ruled out. Pediatric use is still generally restricted to situations where no safer alternative exists.
In older adults, the risk profile shifts toward tendons and nerves. The over-60 age group faces a meaningfully higher risk of tendon rupture, especially when corticosteroids are also in the picture. Age-related kidney changes can slow the drug’s clearance from the body, which may intensify side effects overall.
