To donate plasma in the United States, you must weigh at least 110 pounds, pass a brief physical screening, and bring valid identification. Beyond those basics, donation centers check your vital signs, blood protein and iron levels, medication history, and recent travel before every session. Here’s what each of those criteria involves and what could temporarily or permanently disqualify you.
Age, Weight, and ID Requirements
The FDA requires all plasma donors to weigh at least 110 pounds on the day of donation. Most collection centers set the age range at 18 to 69, though some states allow 16- or 17-year-olds with parental consent.
When you check in for the first time, you’ll need three documents: a government-issued photo ID, proof of your current address (a driver’s license or utility bill works), and proof of your Social Security number, such as a Social Security card, W-2, or pay stub. The name on your Social Security document must match your ID exactly.
Vital Signs Checked Before Every Donation
Staff will measure your blood pressure, pulse, and temperature before each session. Federal regulations set the acceptable ranges:
- Blood pressure: Systolic (top number) between 90 and 180, diastolic (bottom number) between 50 and 100.
- Pulse: Between 50 and 100 beats per minute, with a regular rhythm.
If your readings fall outside those windows on the day you show up, you’ll be turned away for that visit. Nervousness, caffeine, or dehydration can all push your numbers out of range temporarily, so arriving calm and well-hydrated helps.
Blood Tests You Must Pass
A finger-stick sample is taken before each donation to check two things: your hemoglobin (or hematocrit) level and your total protein level. These confirm your blood can safely lose plasma and still carry enough oxygen.
For hemoglobin, men need a minimum of 13.0 g/dL (or a hematocrit of 39%). Women need at least 12.5 g/dL (or a hematocrit of 36%), though some centers may accept women down to 12.0 g/dL under an FDA-approved alternative protocol. Total protein must also fall within an acceptable range, which the center checks to make sure you have enough circulating protein to recover quickly after donation.
Low iron is the single most common reason donors get deferred at the screening station. Eating iron-rich foods like red meat, spinach, and beans in the days before your appointment, and drinking plenty of water the day of, can help you pass on the first try.
Medications That Delay or Block Donation
Certain medications affect the safety of donated plasma, either for you or for the person who eventually receives it. Deferral periods vary widely depending on the drug.
Blood thinners carry waiting periods ranging from 2 days to a month after your last dose, depending on the specific medication. Common anticoagulants like warfarin and heparin require a 7-day wait. Antiplatelet drugs prescribed to prevent stroke or heart attack range from a 2-day to a 1-month deferral.
A few other notable medications and their waiting periods:
- Isotretinoin (severe acne treatment): 1 month after your last dose.
- Finasteride (for hair loss or prostate symptoms): 6 months.
- Oral HIV prevention drugs (PrEP/PEP): 3 months after the last dose. Injectable forms like cabotegravir require a 2-year wait.
- HIV treatment (antiretroviral therapy): Permanent deferral.
- Acitretin (for psoriasis): 3 years.
- Etretinate (older psoriasis drug): Permanent deferral.
If you take any prescription medication, bring a list of names and dosages. The center’s medical staff will cross-reference it against the current deferral list before clearing you.
Temporary Deferrals for Tattoos, Piercings, and Travel
Getting a tattoo or piercing doesn’t permanently disqualify you, but it can trigger a 3-month waiting period. The deferral applies if the tattoo was done in a state that doesn’t regulate tattoo parlors, was self-applied, done by a non-professional, or performed outside the United States. The same 3-month rule covers piercings not done with sterile, single-use equipment, and body branding.
Travel to malaria-risk areas requires a 3-month deferral from your return date. If you’ve actually had malaria, the wait extends to 3 years. A longer, sometimes permanent deferral applies to people who spent a cumulative 3 months or more in the United Kingdom between 1980 and 1996, or who received a blood transfusion in the UK or France during that era. This relates to the risk of a prion disease that standard blood tests cannot detect.
Conditions That Permanently Disqualify You
Relatively few medical conditions result in a lifetime ban. HIV infection is the most well-known permanent deferral, regardless of treatment status or viral load. Certain other conditions that affect the safety of the plasma product, such as a history of specific cancers treated with thalidomide or lenalidomide, also result in permanent disqualification.
The FDA updated its screening approach in 2023, moving away from blanket deferrals based on demographic categories. Donor centers now use individual risk-based questions about recent sexual behavior and other exposures, assessing each person’s specific situation rather than applying group-wide restrictions. This change affects eligibility for men who have sex with men, among other groups who were previously subject to time-based deferrals.
How Often You Can Donate
Plasma donation uses a process called plasmapheresis: your blood is drawn, the plasma is separated out, and your red blood cells are returned to you. Because you keep your red blood cells, you can donate plasma far more frequently than whole blood. Most centers allow up to two donations per week, with at least one day between sessions.
If you’ve recently donated whole blood or red blood cells through a standard blood drive, you’ll typically need to wait 8 weeks before donating plasma. Donating a double unit of red blood cells by apheresis extends that wait to 16 weeks. These rules exist to protect your red blood cell supply and overall iron stores.
What to Expect at Your First Visit
A first-time plasma donation takes longer than repeat visits, usually around 2 hours total. You’ll fill out a health history questionnaire, have your vitals and blood work checked, and go through a brief physical exam. The actual plasma collection takes roughly 45 to 60 minutes once you’re in the chair.
Return visits are faster because the extensive screening is already on file. You’ll still get a vitals check and finger stick each time, but the whole process typically runs 60 to 90 minutes. Eating a protein-rich meal and drinking at least 16 ounces of water before your appointment helps your body handle the fluid shift and makes it more likely you’ll pass the screening without a hitch.
