Amiodarone is one of the most effective drugs for treating dangerous heart rhythms, but it comes with a uniquely long list of serious side effects that can affect the lungs, thyroid, liver, eyes, nerves, and skin. Up to 50% of people on long-term therapy experience at least one adverse effect. What makes amiodarone particularly tricky is its extremely long half-life: after you stop taking it, the drug lingers in your body for an average of 40 to 55 days, meaning side effects can persist or even appear weeks after discontinuation.
Lung Toxicity
Lung damage is the most dangerous side effect of amiodarone, affecting roughly 5% of patients. It takes two forms, and the distinction matters.
The acute form resembles sudden, severe respiratory distress. It’s rare, but it carries mortality rates as high as 50%. The chronic form is far more common and develops gradually over weeks to months. You might notice a dry cough that won’t go away, increasing shortness of breath, chest pain with breathing, unexplained fatigue, or weight loss. Chronic lung toxicity has a mortality rate of up to 10% in some studies, largely because the symptoms creep in slowly enough to be mistaken for other conditions.
One particularly frustrating reality: routine chest X-rays don’t reliably catch lung toxicity early, because the damage can develop rapidly with no prior abnormalities on imaging. This means paying attention to new respiratory symptoms is critical. If you develop a persistent cough or worsening breathlessness while taking amiodarone, that warrants prompt evaluation.
Thyroid Problems
Amiodarone contains a large amount of iodine, which directly interferes with thyroid function. Between 14% and 18% of people on long-term therapy develop thyroid dysfunction, either an underactive thyroid (hypothyroidism) or an overactive one (hyperthyroidism). At lower doses, the risk drops to around 3.7%.
Both conditions are serious but present differently. An underactive thyroid causes fatigue, weight gain, cold sensitivity, and sluggish thinking. An overactive thyroid can cause rapid heart rate, weight loss, anxiety, and heat intolerance. The overactive form is particularly concerning because it can worsen the very heart rhythm problems amiodarone was prescribed to treat. Thyroid function tests are recommended at least every six months during treatment.
Liver Damage
Amiodarone can cause liver inflammation and, in rare cases, serious liver injury. Liver function tests should be checked at least every six months while you’re on the drug. Most liver abnormalities are mild elevations in enzyme levels that resolve when the dose is reduced, but significant liver damage can occur without obvious warning signs. Because the drug stays in your system so long, liver monitoring should continue even after stopping treatment.
Eye and Vision Changes
Nearly everyone who takes amiodarone, between 70% and 100% of patients, develops tiny deposits on the surface of the cornea. These deposits are generally harmless and rarely affect vision. They typically clear up within 6 to 12 months after stopping the drug.
The far more serious eye concern is optic nerve damage, which can cause vision loss in one or both eyes. This is uncommon, but when it occurs, the consequences can be severe. In documented cases of amiodarone-associated optic neuropathy, 20% of patients had vision reduced to a level classified as legal blindness in at least one eye. Some cases progress to permanent vision loss. Any new blurriness, changes in color vision, or loss of visual field while on amiodarone should be evaluated urgently.
Nerve and Brain Effects
Neurological side effects are common and strongly tied to dose. At daily doses of 200 mg or less, fewer than 3% of patients report nerve-related problems. At doses above 400 mg per day, the rate can climb to 35% to 74%. Overall, a large prospective study found that 28% of patients experienced at least one neurological side effect.
The most frequent complaints are vertigo and gait imbalance, each affecting about 9% of patients. Tremor is also common, particularly in the hands, and can interfere with fine motor tasks like writing or buttoning clothes. Other neurological effects include tingling or numbness in the hands and feet (peripheral neuropathy), difficulty with coordination, memory problems, and sleep disturbances. These effects often improve after lowering the dose or stopping the medication, though the drug’s long half-life means improvement can take weeks.
Skin Discoloration and Sun Sensitivity
Amiodarone makes skin significantly more sensitive to sunlight, and prolonged use can cause a distinctive blue-gray discoloration, most visible on the face and other sun-exposed areas. The discoloration results from the drug or its breakdown products depositing directly in the skin.
Unlike the corneal deposits, which clear relatively quickly after stopping the drug, skin discoloration can take much longer to fade. In one documented case, the blue-gray color took 18 months to resolve after discontinuation. The drug’s slow elimination from fat tissue explains the delay. Wearing sunscreen and protective clothing during treatment helps reduce the severity of both the sun sensitivity and the discoloration.
Heart Rhythm Effects
It may seem counterintuitive that a drug used to treat abnormal heart rhythms can also cause them, but amiodarone can slow the heart excessively, leading to bradycardia (an abnormally slow heartbeat). It also prolongs a specific electrical interval in the heart, which in rare cases can trigger a dangerous rhythm called Torsades de Pointes. These cardiac effects are less common with amiodarone than with many other antiarrhythmic drugs, which is one reason it remains widely used despite its other risks.
Drug Interactions
Amiodarone interferes with how the body processes several other common medications. If you take a blood thinner like warfarin, starting amiodarone typically requires about a 25% reduction in your blood thinner dose to avoid dangerously elevated bleeding risk. In a large database study, patients’ blood-thinning levels rose significantly after beginning amiodarone, from an average INR of 2.6 to 3.1.
The drug also increases blood levels of digoxin, a medication used for heart failure and certain heart rhythms. If you’re on either of these medications and amiodarone is being added, your other drug doses will likely need adjustment, with close monitoring during the transition.
Why Side Effects Linger
What sets amiodarone apart from most medications is how long it stays in your body. The average elimination half-life is about 53 days in patients on long-term therapy, with some individuals taking over 100 days to clear the drug. This means that after you stop taking it, amiodarone levels drop by only half over the course of nearly two months. Side effects can persist, worsen, or even first appear well after discontinuation, and any replacement medication or dose adjustment takes weeks to show its full effect. This extended timeline affects every aspect of managing amiodarone’s risks, from monitoring schedules to treatment decisions.
