Valsartan, a blood pressure medication in the angiotensin receptor blocker (ARB) class, carries several serious side effects beyond the common ones like dizziness and fatigue. The most significant risks involve dangerously low blood pressure, kidney damage, high potassium levels, and a rare but potentially life-threatening allergic reaction called angioedema. Valsartan also carries an FDA black box warning for fetal toxicity during pregnancy.
Low Blood Pressure
Symptomatic hypotension is one of the more common serious side effects. In heart failure trials, 7% of patients taking valsartan experienced low blood pressure compared to 2% on placebo, and an additional 2% had drops in blood pressure specifically upon standing. The risk is highest if you’re already dehydrated, taking diuretics (water pills), or on a low-salt diet, because valsartan lowers blood pressure further in people whose fluid volume is already reduced.
Symptoms of a dangerous blood pressure drop include feeling faint or lightheaded, sudden confusion or drowsiness, and in severe cases, passing out entirely. The risk tends to be greater when you first start taking the medication or after a dose increase. If you take a diuretic alongside valsartan, the combination can amplify this effect significantly.
Kidney Damage
Valsartan can impair kidney function, particularly in people whose kidneys are already under stress. In heart failure trials, 3.9% of patients on valsartan had significant increases in creatinine (a waste product the kidneys filter) compared to just 0.9% on placebo. In patients recovering from a heart attack, 4.2% showed a doubling of creatinine levels. About 1.1% of heart failure patients on valsartan had to stop the drug entirely because of declining kidney function.
The risk to your kidneys climbs when valsartan is combined with other medications. Taking an ARB like valsartan alongside a diuretic and a common pain reliever like ibuprofen or naproxen (NSAIDs) creates what researchers call a “triple whammy” for the kidneys. One study found this combination nearly doubled the 30-day risk of acute kidney injury compared to taking just the ARB and diuretic together. Reduced urine output is one early warning sign of kidney trouble.
High Potassium Levels
Valsartan can cause potassium to build up in your blood, a condition called hyperkalemia. In heart failure trials, 2% of valsartan patients developed elevated potassium versus 1% on placebo, and 0.5% had to stop the medication because of it. This matters because potassium regulates your heartbeat. When levels get too high, it can cause dangerous heart rhythm problems.
Warning signs include feeling unusually weak, noticing an irregular or pounding heartbeat, experiencing pins and needles sensations, or having muscle cramps. The risk increases if you have kidney problems, take potassium supplements, or use other medications that raise potassium. Current guidelines recommend checking blood electrolytes two to four weeks after starting valsartan or changing the dose, then at least annually.
Angioedema
Angioedema is a rare but potentially deadly allergic reaction where tissue beneath the skin swells rapidly, most often in the lips, mouth, throat, or tongue. When the airway is involved, it can make breathing difficult or impossible. The exact mechanism isn’t fully understood, but valsartan may raise levels of bradykinin, a substance that causes blood vessels to dilate and tissues to swell. Patients who previously took an ACE inhibitor (another blood pressure drug class) before switching to valsartan may face a higher risk of this reaction.
Angioedema from valsartan requires immediate emergency treatment. Signs to watch for include sudden swelling of the face, lips, or throat, difficulty swallowing, wheezing, or struggling to breathe.
Low White Blood Cell Count
Valsartan can reduce a type of white blood cell called neutrophils, which your body relies on to fight infections. In trials, 1.9% of valsartan patients developed low neutrophil counts compared to 0.8% on placebo. This may leave you more vulnerable to infections. Unusual bruising, reddish-purple spots on the skin, prolonged bleeding from minor cuts, or bleeding gums can signal blood cell problems and warrant prompt medical attention.
Liver Problems
Though less commonly discussed, valsartan can affect liver function. Yellowing of the skin or the whites of the eyes is a warning sign. This may be harder to spot on darker skin tones, so paying attention to changes in the color of your eyes is particularly important.
Fetal Toxicity During Pregnancy
Valsartan carries the FDA’s most serious warning, a black box label, for harm to a developing fetus. The warning states plainly: drugs that act on the renin-angiotensin system can cause injury and death to the developing fetus. Exposure during the second and third trimesters is especially dangerous because valsartan reduces fetal kidney function. This leads to a drop in amniotic fluid, which in turn can cause underdeveloped lungs, skeletal deformities including abnormal skull development, and kidney failure in the newborn. The medication should be stopped as soon as pregnancy is detected.
Interactions That Raise Risk
Several common drug combinations make valsartan’s serious side effects more likely. NSAIDs like ibuprofen and naproxen, which many people take casually for pain, reduce blood flow to the kidneys and can tip the balance toward kidney injury when combined with valsartan. Adding a diuretic to that mix raises the risk further. If you take valsartan and need pain relief, it’s worth discussing alternatives with a pharmacist.
Potassium supplements and salt substitutes (which often contain potassium chloride) can push already-elevated potassium levels into a dangerous range. Other blood pressure medications taken alongside valsartan increase the chance of hypotension, dizziness, and fainting.
Signs That Need Immediate Attention
Some side effects develop gradually, but others demand emergency action. Sudden swelling of your lips, mouth, tongue, or throat, or any difficulty breathing or swallowing, is a medical emergency. So is sudden severe dizziness, confusion, or loss of consciousness, which can indicate a dangerous blood pressure drop.
Less urgent but still serious symptoms to report promptly include persistent weakness with an irregular heartbeat or muscle cramps (suggesting high potassium), a noticeable decrease in how much you urinate (suggesting kidney problems), yellowing of the eyes or skin (suggesting liver involvement), and unusual bruising or prolonged bleeding (suggesting low blood cell counts). Most of these side effects are manageable when caught early, which is why blood work shortly after starting or adjusting the dose plays an important role in using valsartan safely.
