Adderall’s most common side effects are loss of appetite, trouble sleeping, and a faster heartbeat. These affect a significant number of people who take the medication, and they tend to get more noticeable at higher doses. Beyond these everyday effects, Adderall carries risks for your heart, your mental health, and, in children, physical growth. Here’s what to expect and what actually matters.

How Adderall Affects Your Body

Adderall works by increasing two chemical messengers in the brain: dopamine and norepinephrine. In the prefrontal cortex, the area responsible for focus and impulse control, this boost strengthens important signals while quieting background noise. That’s what helps with ADHD symptoms.

But dopamine and norepinephrine don’t just work in the brain. They’re active throughout your body, particularly in your cardiovascular system. That’s why Adderall’s side effects extend well beyond your head. The same chemical changes that sharpen your attention also raise your heart rate, suppress your appetite, and activate your body’s stress-response system.

Common Side Effects

Three side effects stand out as the most frequent, and all three are dose-dependent, meaning they get worse as the dose goes up:

  • Loss of appetite: This is the single most reported side effect. It often hits hardest in the first few weeks and can lead to noticeable weight loss. In a clinical trial of adolescents, those on 10 mg lost an average of 1.1 pounds in the first four weeks, while those on 20 mg lost 2.8 pounds.
  • Insomnia: Adderall is a stimulant, and it can keep you wired well past bedtime, especially if you take a dose too late in the day. This is one of the most common reasons people adjust their dosing schedule.
  • Weight loss: Closely tied to appetite suppression, but worth noting separately. Weight tends to be more affected than height in children, with weight changes roughly 2.4 times greater than height changes over the first couple of years.

Other commonly reported effects include dry mouth, headache, stomach pain, irritability, and nervousness. Most of these are mild and often improve after the first few weeks as your body adjusts.

Heart Rate and Blood Pressure

Adderall reliably raises both heart rate and blood pressure, and this happens even in healthy people taking it for the first time. A Mayo Clinic study found that a single 25 mg dose in healthy young adults with no prior exposure doubled the heart rate spike that normally occurs when standing, jumping from 19 extra beats per minute to 38. That’s a significant cardiovascular response from one pill.

Higher doses mean more cardiovascular strain. In a study of adolescents, 12% of those given 10 mg had isolated spikes in systolic blood pressure above the normal range for their age. At 20 mg, that number tripled to 35%. For most healthy people, these increases are temporary and manageable. But for anyone with an underlying heart condition, the risks are more serious. The FDA label warns that sudden death has been reported in patients with structural heart abnormalities or serious cardiac disease who took stimulants at standard ADHD doses.

Psychiatric Risks

This is the side effect category that surprises most people. Adderall can trigger psychosis or mania, even in people with no prior psychiatric history. A study published in the American Journal of Psychiatry found that people using prescription amphetamines in the past month had 2.7 times the odds of experiencing a first episode of psychosis or mania compared to non-users.

The risk climbs steeply with dose. At higher doses (above 30 mg of dextroamphetamine equivalents), the odds jumped to 5.3 times greater. In the most sensitive analysis, comparing against outpatient controls, the highest doses were linked to a 13.5-fold increase. Notably, methylphenidate (the active ingredient in Ritalin and Concerta) showed no increased risk of psychosis or mania in the same study, which suggests this is something specific to amphetamine-type stimulants rather than all ADHD medications.

These episodes are rare in absolute terms, but they’re not negligible. Symptoms can include paranoia, hallucinations, or an abnormally elevated mood with impulsive behavior. They typically resolve once the medication is stopped or the dose is lowered.

Growth Effects in Children

Children and adolescents on stimulant medications, including Adderall, grow more slowly than their peers. The effect is most pronounced in the first two years of treatment, with height deficits of 1 to 1.4 centimeters per year during that window.

The long-term picture comes from the Multimodal Treatment Study of ADHD, one of the largest and longest-running studies on ADHD treatment. After 14 months, children on stimulants were 3.7 cm shorter than those who never took medication (though those on medication were already 1.7 cm shorter at baseline, suggesting ADHD itself may play a role). By adulthood, children who consistently took their medication were about 4.7 cm (roughly 1.9 inches) shorter than those who rarely took it.

Whether this represents a permanent loss in adult height or a delay in growth that partially catches up remains somewhat unclear. The growth suppression is dose-dependent, and some clinicians recommend periodic breaks from the medication, particularly during summers, to allow for catch-up growth.

Long-Term Use

Decades of research on stimulant medications have not identified major negative health effects from long-term use at prescribed doses. The concerns that do exist center on sustained increases in blood pressure, which over many years could contribute to cardiovascular disease, and the potential for physical dependence.

The FDA’s strongest warning on Adderall is its black box label for abuse, misuse, and addiction. The label states that Adderall “has a high potential for abuse and misuse, which can lead to the development of a substance use disorder, including addiction.” That said, at therapeutic doses prescribed for ADHD, the dopamine increase is too gradual to produce a high. The addiction risk rises sharply with misuse, particularly crushing and snorting pills, or taking doses far above what’s prescribed.

Tolerance is a common concern. Some people find their current dose becomes less effective over time, which can lead to dose increases. This is worth discussing with a prescriber rather than adjusting on your own, since higher doses bring proportionally more side effects.

Managing Appetite Loss and Sleep Problems

Since appetite suppression and insomnia are the two most disruptive day-to-day side effects, there are well-established strategies for both.

For appetite, the simplest approach is timing meals around medication. Eating a solid breakfast before your dose kicks in, and planning a larger dinner as it wears off, can help maintain calorie intake. High-calorie snacks throughout the day fill in the gaps. If appetite suppression remains severe, switching between amphetamine-based and methylphenidate-based medications sometimes helps, since individuals often tolerate one better than the other. Medication breaks on weekends or school vacations are a last resort but can give appetite and weight a chance to recover.

For sleep, reducing the last dose of the day or taking it earlier often solves the problem. Low-dose melatonin (0.5 to 1 mg) taken about an hour before bedtime is a common first step. If that’s not enough, prescription options exist that can help initiate sleep without interfering with next-day ADHD treatment.

How Dose Affects Side Effects

Nearly every side effect of Adderall follows a dose-response pattern: more medication, more side effects. This is true for appetite suppression, insomnia, weight loss, blood pressure elevation, and psychiatric risks. The 5.3-fold increase in psychosis risk seen at high amphetamine doses, compared to 2.7-fold at any dose, illustrates how steep that curve can be.

This is why most prescribers start at the lowest effective dose and increase gradually. The goal is finding the dose where ADHD symptoms improve meaningfully without side effects becoming a problem. For many adults, that range falls between 10 and 30 mg daily, though it varies widely. The clinical trial data consistently shows that side effects at 10 mg are substantially milder than at 20 mg or above, so the difference between doses is not trivial.
