Carprofen is a nonsteroidal anti-inflammatory drug (NSAID) prescribed to dogs for pain and inflammation, most commonly from arthritis or surgery. In clinical trials, possible adverse events were reported in roughly 1.3% of post-treatment evaluations, making it generally well tolerated. But like all NSAIDs, it carries real risks to the stomach, liver, and kidneys that every dog owner should recognize.
Common Digestive Side Effects
The most frequent side effects involve the digestive system: vomiting, diarrhea or soft stool, and reduced appetite. In clinical field studies, these occurred at rates of about 3 to 4% of dogs. What’s notable, though, is that dogs on a placebo showed similar or even slightly higher rates of vomiting and diarrhea. In surgical pain studies, 10.1% of carprofen-treated dogs vomited compared to 13.4% on placebo, suggesting that some of this stomach upset comes from the underlying condition or surgery rather than the drug itself.
Other mild side effects reported in trials include lethargy (about 1.4%), behavioral changes (1%), and constipation (0.3%). These were also seen at comparable rates in dogs receiving no medication.
If your dog starts vomiting repeatedly, refuses food for more than a day, or develops dark, tarry stools (a sign of bleeding in the digestive tract), stop the medication and contact your vet. Occasional soft stool or one skipped meal is less concerning, but persistent symptoms deserve attention.
Liver Toxicity: Rare but Serious
The most worrisome side effect of carprofen is idiosyncratic liver injury. “Idiosyncratic” means it’s unpredictable and not dose-dependent. It can happen to any dog, but Labrador Retrievers appear to be at higher risk than other breeds.
When liver problems do occur, they typically show up within the first four weeks of treatment. The warning signs are loss of appetite, vomiting, and yellowing of the skin, gums, or whites of the eyes (jaundice). These signs can escalate quickly. Internally, the liver may show cell death, swelling, and inflammation, with bloodwork revealing sharply elevated liver enzymes and bilirubin levels.
Because this reaction is rare and unpredictable, monitoring is the main defense. Your vet should run baseline bloodwork and a urinalysis before starting carprofen. For dogs on long-term therapy, liver and kidney values should be rechecked two to four weeks after starting the drug, then every three to six months for as long as your dog takes it.
Kidney Effects and Dehydration Risk
In healthy, well-hydrated dogs, carprofen does not appear to significantly affect kidney function. An eight-day study found no meaningful change in kidney filtration rate or blood flow compared to placebo. That’s reassuring for most dogs.
The picture changes when a dog is dehydrated or losing fluids. In volume-depleted dogs (those also receiving a diuretic), carprofen caused a significant drop in kidney filtration along with rising creatinine and BUN, two markers of kidney stress. The good news: these changes reversed after the drug was stopped. The important takeaway is that carprofen becomes riskier for the kidneys whenever your dog is dehydrated, whether from illness, vomiting, not drinking enough water, or taking other medications that affect fluid balance.
Despite being classified as a COX-2 preferential drug (meaning it’s designed to be gentler on the body), carprofen’s kidney effects in dehydrated dogs were no different from those of nonselective NSAIDs like ibuprofen. COX-2 selectivity does not protect the kidneys when fluid volume is low.
Overdose Symptoms
Carprofen’s normal dose for dogs is around 2 mg per pound of body weight daily. Toxicity thresholds are well above that: gastrointestinal toxic effects begin at roughly 10 mg per pound (20 mg/kg), and kidney damage becomes a risk at about 20 mg per pound (40 mg/kg). This means accidental double-dosing is unlikely to cause serious harm, but a dog that chews into a bottle could be in danger.
Early overdose signs mirror the common side effects but are more severe: repeated vomiting, bloody vomit, diarrhea, black or bloody stool, abdominal pain, weakness, and loss of appetite. At higher doses, acute kidney failure, extreme lethargy, and dangerous shifts in blood potassium levels can develop. If you suspect your dog has consumed extra carprofen, call your vet or an animal poison control hotline immediately, even if your dog seems fine at first.
What Increases the Risk of Side Effects
Several factors raise the odds of problems with carprofen:
- Dehydration or fluid loss. Any condition that reduces your dog’s hydration, from kidney disease to prolonged vomiting, makes kidney side effects more likely.
- Other anti-inflammatory drugs. Combining carprofen with corticosteroids (like prednisone) or other NSAIDs (like aspirin or meloxicam) significantly increases the risk of stomach ulcers and bleeding. These combinations should be avoided.
- Breed. Labrador Retrievers have a documented higher susceptibility to carprofen-related liver damage.
- Pre-existing liver or kidney disease. Dogs with compromised organ function are less able to process and clear the drug safely.
- Age. Older dogs are more likely to have reduced kidney or liver reserves, even without a formal diagnosis, making monitoring especially important.
What to Watch For at Home
Most dogs tolerate carprofen well for months or years. The key is knowing which changes matter. Watch for persistent vomiting or diarrhea lasting more than a day, any sign of blood in vomit or stool, yellowing of the gums or eye whites, a sudden refusal to eat, unusual lethargy or weakness, increased thirst or urination (which can signal kidney stress), or dark-colored urine.
Keep your dog well hydrated, especially in warm weather or during illness. Make sure follow-up bloodwork happens on schedule. If your dog is a Labrador or has any history of liver issues, discuss whether more frequent monitoring makes sense. Many side effects are reversible when caught early, so routine blood panels are not just a formality. They’re the most reliable way to catch organ stress before your dog shows outward symptoms.
