Hydralazine, a blood pressure medication that works by relaxing blood vessel walls, causes side effects that range from a racing heartbeat and headaches to a rare but serious lupus-like syndrome. The most common issues stem directly from how the drug lowers blood pressure: when your vessels suddenly relax, your heart compensates by beating faster, and your body tries to hold onto more fluid. Understanding which side effects are routine and which signal something more serious can help you know what to watch for.
Common Side Effects
The side effects people experience most often are essentially the drug doing its job too well or too fast. When hydralazine dilates your blood vessels, your nervous system detects the drop in pressure and triggers a reflex response, speeding up your heart rate. This is why a fast, pounding, or irregular heartbeat is one of the most frequently reported problems. Nausea, chest tightness, shortness of breath, and sweating round out the list of common reactions.
Headache, flushing, and dizziness also fall into this category. The dizziness tends to be most noticeable when you stand up quickly from sitting or lying down, because your blood pressure drops further with the position change and the drug makes it harder for your body to compensate. These effects are generally strongest when you first start taking hydralazine or after a dose increase, and they often ease as your body adjusts over the first week or two.
Fluid Retention and Weight Changes
Hydralazine prompts your kidneys to hold onto sodium and water. You may notice swelling in your feet or lower legs, or a few pounds of seemingly unexplained weight gain. This is why the drug is frequently prescribed alongside a diuretic (a water pill), which counteracts the fluid buildup. If you’re on a combination that includes a diuretic, you may be advised to eat potassium-rich foods like oranges or bananas, or take a potassium supplement, because the diuretic can flush potassium out along with the extra fluid.
Less Common but Notable Reactions
A range of less frequent side effects can appear at any point during treatment. These include:
- Nerve-related symptoms: numbness, tingling, “pins and needles,” or weakness in the hands and feet
- Skin reactions: rash, itching, or blisters
- General malaise: fever, sore throat, chills, muscle or joint pain, unusual tiredness
- Blood-related signs: unusual bruising or bleeding, pale skin, black or tarry stools
- Other: blurred vision, confusion, swollen lymph nodes, mouth sores
Tingling and weakness in the extremities deserve particular attention because they can indicate peripheral neuropathy, a type of nerve irritation. This side effect is more likely at higher doses and in people whose bodies process the drug more slowly (a trait determined by genetics, specifically how fast your liver breaks down certain compounds through a process called acetylation). People who are “slow acetylators,” roughly half the population, clear hydralazine more slowly and may be more vulnerable to both nerve effects and the lupus-like reaction described below.
Drug-Induced Lupus
The most serious side effect unique to hydralazine is a condition called drug-induced lupus. Unlike the autoimmune disease lupus (which develops on its own), this form is triggered specifically by the medication and typically resolves after you stop taking it. Symptoms usually don’t appear until you’ve been on hydralazine for at least 3 to 6 months, and the risk climbs significantly at daily doses of 200 mg or more.
The hallmark symptoms include joint pain and swelling, fever, general malaise, loss of appetite, a skin rash on sun-exposed areas, and a sharp chest pain that worsens when you breathe in (caused by inflammation of the lining around the lungs or heart). If you’ve been on hydralazine for several months and develop unexplained joint pain combined with fever or a new rash, those are the red flags worth bringing to your doctor promptly.
Diagnosis involves blood tests that look for specific antibodies, particularly antihistone antibodies, which are the characteristic marker of the drug-induced form. Once hydralazine is stopped, symptoms generally clear within weeks to months, though the antibodies themselves can linger in the bloodstream longer.
Rare Liver-Related Effects
In rare cases, hydralazine can affect the liver. Warning signs include dark urine, light-colored stools, pain in the upper right area of the abdomen, and yellowing of the eyes or skin. These symptoms suggest the liver isn’t processing waste normally and warrant immediate medical attention.
Why Side Effects Vary Between People
Your individual reaction to hydralazine depends partly on how quickly your liver metabolizes the drug. Everyone carries genes that control the speed of a specific enzyme responsible for breaking hydralazine down. If you’re a slow metabolizer, more of the drug stays active in your bloodstream for longer, which means stronger blood pressure effects but also a higher chance of side effects, especially nerve symptoms and drug-induced lupus. Your doctor can’t always predict this in advance, which is one reason follow-up visits and blood work matter while you’re on this medication.
Dose also plays a major role. Most of the serious complications, including lupus and peripheral neuropathy, cluster at the higher end of the dosing range. Keeping the dose as low as effective is one of the main strategies for reducing risk.
How Side Effects Are Managed
Because so many of hydralazine’s side effects stem from the reflex heart rate increase and fluid retention, it’s commonly prescribed as part of a combination. A beta-blocker slows the heart rate back down, directly countering the reflex tachycardia. A diuretic handles the fluid retention. This three-drug approach (hydralazine plus a beta-blocker plus a diuretic) is a well-established strategy that makes the medication far more tolerable for most people.
For the everyday discomforts like dizziness when standing, practical adjustments help: getting up slowly, staying well-hydrated, and avoiding alcohol (which amplifies the blood pressure drop). If headaches persist beyond the first couple of weeks, that’s usually a sign to discuss dose adjustment rather than something to push through.
Hydralazine in Pregnancy
Hydralazine is one of the medications used to manage dangerously high blood pressure during pregnancy, particularly in preeclampsia. A systematic review and meta-analysis found that pregnant women receiving hydralazine had a heart rate roughly 13 beats per minute higher than those on alternative medications. Overall, though, the same analysis concluded that hydralazine’s safety and effectiveness profile is comparable to the other commonly used options for treating hypertension in pregnancy. The reflex heart rate increase is the main side effect to be aware of in this setting.
