What Are the Side Effects of Pravastatin?

Pravastatin, a cholesterol-lowering statin, causes muscle and joint pain most frequently, along with stomach discomfort. Most people tolerate it well compared to other statins, and serious side effects are rare. Here’s what to expect and what to watch for.

The Most Common Side Effects

The side effects people notice most often with pravastatin involve the muscles and joints. These include muscle or bone pain, joint pain, stiffness, and difficulty moving. Stomach pain is also common and typically mild enough that it doesn’t require medical attention.

Less common but still reported side effects span a wide range: headache, nausea, diarrhea, dizziness, trouble sleeping, fatigue, and cold-like symptoms such as a runny nose, sore throat, or cough. Some people experience digestive issues like bloating, gas, heartburn, or constipation. Blurred or double vision, skin rash, and mood changes (including irritability or low mood) show up occasionally as well.

Many of these milder effects settle down on their own as your body adjusts to the medication. If they persist or become bothersome, your prescriber can often adjust the dose or timing.

Muscle Problems: What to Watch For

Muscle-related side effects are the hallmark concern with all statins, and they deserve special attention. Randomized controlled trials report statin-related muscle problems in roughly 1.5% to 5% of users, though observational studies that include high-dose regimens have reported rates as high as 10.5%. Pravastatin sits on the lower end of that spectrum. A large comparative study found that when researchers used pravastatin as the reference point, atorvastatin and simvastatin were both associated with higher rates of muscle complaints.

Muscle side effects can show up a few weeks or months after starting the medication. They typically feel like unexplained soreness, tenderness, weakness, or cramping, and they’re more likely at higher doses. In very rare cases, a severe form of muscle breakdown called rhabdomyolysis can occur, where damaged muscle fibers release their contents into the bloodstream. One large clinical trial of pravastatin found zero cases of rhabdomyolysis in the treatment group (the single case occurred in a person taking a placebo). Still, if you develop unexplained muscle pain that feels different from normal exercise soreness, especially if your urine turns dark, that warrants prompt medical attention.

Why Pravastatin Tends to Be Better Tolerated

Pravastatin is water-soluble, which sets it apart from fat-soluble statins like atorvastatin and simvastatin. This matters because water-soluble drugs don’t penetrate muscle cells or cross into the brain as easily. A network analysis published in Circulation: Cardiovascular Quality and Outcomes found that pravastatin and simvastatin had statistically better safety profiles than other statins, with higher-dose atorvastatin and rosuvastatin showing greater odds of people stopping treatment due to side effects.

Pravastatin’s water solubility also means it doesn’t cross the blood-brain barrier the way fat-soluble statins do. This is relevant to a common worry: memory problems. The PROSPER trial, the longest randomized study of pravastatin in older adults, found no difference in cognitive test scores, working memory, or declarative memory between the pravastatin and placebo groups. Broader analyses of statins and cognition have similarly found no evidence that these medications cause memory decline.

Liver Effects

Statins work in the liver, so there’s an inherent concern about liver stress. In practice, liver enzyme elevations with pravastatin are uncommon. In a large trial of people with kidney disease, abnormal liver enzyme levels occurred in just 0.6% of pravastatin users, virtually identical to the 0.7% rate in the placebo group. The FDA recommends a liver function test before starting pravastatin and then only when symptoms suggest a problem, not on a rigid monitoring schedule. Signs of liver trouble include unusual fatigue, loss of appetite, dark urine, or yellowing of the skin.

Blood Sugar and Diabetes Risk

Many statins carry a small increase in diabetes risk, which has been a concern since the FDA added a warning to the drug class in 2012. Pravastatin appears to be an exception. A large study from the West of Scotland Coronary Prevention Study found that pravastatin actually reduced the risk of developing diabetes by 30% compared to placebo. Out of nearly 6,000 participants followed over several years, 139 developed diabetes, and pravastatin treatment was independently protective even after adjusting for other risk factors like weight and blood sugar levels at the start of the study.

Drug Interactions

One practical advantage of pravastatin is that it largely bypasses the liver enzyme pathway (CYP3A4) that metabolizes many common medications. Most of the drug is excreted unchanged. This means pravastatin is far less prone to the drug interactions that complicate treatment with other statins. Medications like certain antibiotics, antifungals, and grapefruit juice that notoriously raise levels of atorvastatin or simvastatin are much less of a concern with pravastatin. That said, some drugs can still interact with it, so your pharmacist should review your full medication list.

People With Kidney Disease

Kidney disease is sometimes considered a reason to be cautious with statins, but pravastatin has been specifically studied in this group. A trial published in Circulation tracked people with moderate kidney impairment and found pravastatin was well tolerated, with no evidence of clinically meaningful toxicity. Rates of muscle enzyme elevation, liver enzyme abnormalities, and other adverse events were nearly identical between the pravastatin and placebo groups. Depression rates were actually slightly lower in the pravastatin group (4.0% versus 5.1% on placebo).
