What Are the Side Effects of Valium and Its Risks?

Valium (diazepam) most commonly causes drowsiness, fatigue, muscle weakness, and problems with coordination. These effects show up frequently and tend to be dose-dependent, meaning higher doses produce more noticeable symptoms. But the side effect profile extends well beyond simple sedation, especially with longer use or in certain populations.

The Most Common Side Effects

The four side effects reported most often with Valium are drowsiness, fatigue, muscle weakness, and ataxia (a loss of coordination that can make you feel unsteady on your feet). These are direct extensions of how the drug works: it enhances a calming chemical in the brain, which slows neural activity broadly rather than targeting anxiety alone. That means your reflexes, alertness, and muscle control all take a hit.

What makes these effects linger longer than you might expect is the drug’s unusually long presence in your body. Valium has an elimination half-life of up to 48 hours, and it breaks down into an active byproduct that can stay in your system with a half-life of up to 100 hours. So even after the calming effects feel like they’ve worn off, the drug and its byproducts may still be affecting your coordination and reaction time. This is why driving or operating machinery can remain risky well into the next day.

Psychological and Behavioral Effects

Valium can cause what clinicians call paradoxical reactions, where instead of calming you down, the drug triggers the opposite response. These reactions include increased aggression, agitation, irritability, hostility, hyperactivity, and depression. They can appear as a worsening of a behavior you already had or as something entirely new.

Certain groups face a higher risk of these paradoxical effects: children, older adults, people with poor impulse control, those with a history of behavioral problems, and people with brain injuries or developmental disabilities. If you or someone you care for falls into one of these categories, these reactions are worth watching for closely, particularly in the first days and weeks of use.

Serious and Potentially Dangerous Reactions

The most dangerous risk with Valium is respiratory depression, where breathing becomes dangerously slow or shallow. This risk is relatively low when Valium is taken alone at prescribed doses, but it escalates dramatically when combined with opioid painkillers. The FDA’s strongest safety warning, a black box warning, states that combining benzodiazepines like Valium with opioids “may result in profound sedation, respiratory depression, coma, and death.” This combination is one of the leading contributors to overdose deaths involving benzodiazepines.

Other serious but less common reactions include low blood pressure and, in rare cases with long-term use, drops in a type of white blood cell count (neutropenia) and jaundice. Periodic blood work and liver function tests are recommended during extended treatment for this reason.

Valium is not suitable for people with certain conditions. If you have myasthenia gravis (a condition causing muscle weakness) or sleep apnea, the drug can worsen the core symptoms of those conditions. Myasthenia gravis already impairs muscle function, and Valium’s muscle-relaxing properties can make breathing muscles dangerously weak. Similarly, sleep apnea involves interrupted breathing during sleep, and a sedative that suppresses respiratory drive compounds that risk.

Effects on Memory and Cognition

Short-term memory impairment is a well-known effect of Valium, but the longer-term cognitive picture is more concerning. Clinical studies have found that chronic benzodiazepine use increases the risk of dementia in older adults, and some research links long-term exposure specifically to a higher risk of Alzheimer’s disease.

Animal research helps explain why. In a study published in Frontiers in Aging Neuroscience, middle-aged mice given diazepam chronically showed significant impairments in spatial memory compared to controls. The researchers traced this to physical changes in the hippocampus, the brain region central to memory formation. Chronic diazepam reduced the density of dendritic spines (the tiny connection points between brain cells) and weakened the signaling process that underlies learning. Notably, even young mice showed these brain changes at the cellular level, even when their outward performance on memory tasks still appeared normal. This suggests the drug may be quietly eroding the brain’s memory infrastructure before obvious symptoms appear.

Risks for Older Adults

Valium poses outsized risks for people over 65. The body metabolizes the drug more slowly with age, so it accumulates to higher levels and lingers longer. The Beers Criteria, a widely used guideline for prescribing in older adults maintained by the American Geriatrics Society, lists benzodiazepines as medications to avoid or use with extreme caution in this age group. The rationale is straightforward: impaired metabolism leads to cognitive impairment and unsteady gait, and the harmful side effects generally outweigh the benefits.

Falls are a particular concern. The combination of drowsiness, muscle weakness, and impaired coordination that Valium produces is especially dangerous for older adults, who are already at elevated risk of falls and fractures. A hip fracture in an older adult can set off a cascade of complications, making what seems like a minor side effect potentially life-altering.

Dependence and Withdrawal

Physical dependence on Valium can develop even at prescribed doses, and it tends to build faster than many people expect. Once dependence sets in, stopping the drug abruptly triggers a withdrawal syndrome that follows a fairly predictable pattern.

Withdrawal symptoms typically begin 12 to 24 hours after the last dose, reflecting the drug’s long half-life. The early stage lasts roughly one to four days and often involves rebound anxiety, restlessness, and insomnia. The acute withdrawal phase follows, lasting from a few days to about two weeks, and this is when symptoms peak. Tremors, sweating, nausea, increased heart rate, and in severe cases seizures can occur during this window. Most physical symptoms taper off after one to two weeks, but psychological symptoms like anxiety, irritability, and difficulty concentrating may persist for weeks or months.

Because of the seizure risk, stopping Valium cold turkey after regular use is genuinely dangerous. A gradual taper, where the dose is slowly reduced over weeks or months, is the standard approach to minimize withdrawal severity.

Interactions That Increase Risk

Beyond opioids, Valium’s sedative effects are amplified by alcohol, other benzodiazepines, sleep medications, certain antidepressants, and anticonvulsants. Any substance that depresses the central nervous system will stack with Valium’s effects, deepening sedation and increasing the risk of respiratory problems. Even over-the-counter antihistamines that cause drowsiness can meaningfully intensify these effects. If you’re taking Valium, the safest assumption is that anything else making you sleepy will make the combination riskier than either substance alone.
