High blood pressure usually has no signs at all. That’s the core problem, and the reason it’s called the “silent killer.” The damage it causes to your heart, kidneys, brain, and blood vessels builds gradually, often without any noticeable symptoms until something serious happens. In 2023 alone, high blood pressure was a primary or contributing cause of more than 664,000 deaths in the United States.

Why Most People Feel Nothing

If you’re searching for symptoms that would tip you off to high blood pressure, the honest answer is that your body rarely sends a clear signal. You can walk around with dangerously high readings for years and feel perfectly fine. The damage happens at the level of your blood vessel walls, your heart muscle, and the tiny filtering units inside your kidneys. None of that produces pain or discomfort until it reaches a critical point.

This is exactly why routine screening matters more than symptom-watching. The U.S. Preventive Services Task Force recommends checking your blood pressure every year if you’re 40 or older, or if you’re at increased risk due to factors like being overweight, being Black, or having previous readings on the higher end of normal. If you’re 18 to 39 with no risk factors and a normal prior reading, screening every three to five years is reasonable.

Subtle Clues That Sometimes Appear

Some people with chronically elevated blood pressure do notice vague symptoms, though these overlap with dozens of other causes. When high blood pressure goes unmanaged for years, the first “symptoms” people notice are often signs that an organ has already been injured. These aren’t really signs of high blood pressure itself. They’re signs of its consequences.

In the heart, long-term high pressure forces the muscle to work harder, which can thicken the heart wall and eventually weaken it. In the brain, the effects range from subtle to catastrophic. Some people develop mild cognitive changes: slightly worse memory, trouble concentrating, or mood shifts. Others experience a transient ischemic attack (a mini-stroke) or a full stroke, with sudden weakness on one side of the body, slurred speech, or loss of consciousness. High blood pressure is also linked to vascular dementia, where repeated tiny strokes slowly erode thinking ability over time.

What the Numbers Mean

When Your Readings Tell Different Stories
